A deposit of $500. per camper must accompany each
application. Postdated cheques for 50% of the fee are
due March 15, 2016. Balance is due May 4, 2016. If
payment is not made as per the schedule, Camp
Agudah may cancel the enrolment.
Pre-tipping is required. If tipping by credit card mark the
box on reverse side.

Camp Agudah will accept payment by Visa or Mastercard until
February 25th, 2016 and only if paying the full fee. Balance of
fees will be charged automatically to your credit card on or
about February 25th, 2016. Credit card payments made after
February 25th, 2016 will be subject to a charge to cover credit
card costs.

GENERAL AGREEMENT, CANCELLATION AND REFUND POLICY “Camp” means Camp Agudah of Toronto
A deposit of $500 per camper must accompany each application. Balance of payment as per schedule on reverse side. A full refund will be issued if application is
not accepted.
Cancellations until February 25th are subject to a $100 forfeiture. After February 25th refunds will be issued at the camp’s discretion.
CANCELLATION MUST BE MADE IN WRITING.
Camp Agudah will accept payment using Visa or Mastercard until February 25th, 2016 and only if paying the full fee. Credit card payments made after February
25th will be subject to a charge to cover credit card costs. NSF cheques and declined Visa/Mastercard will incur a $45 fee.
Pre-tipping is required. (Totalling $125 as per schedule on other side of form). If tipping by credit card, mark the box below .
All out of province campers and staff members must provide effective health coverage prior to being admitted into camp. No one will be permitted onto camp
grounds without proof of coverage. Upon acceptance, campers will receive a medical form which must be completed and returned immediately as part of your
acceptance to camp.
Even if paying by cheque Camp requires a credit card information, which must be filled out below, to cover unforeseen charges such as prescription and/or
medical costs, unexpected travel expenses and negligent purposeful damage to camp property caused by your child and NSF cheques. You will be notified of any
charges levied against the card.
WAIVER & DECLARATION - “Camp” means Camp Agudah of Toronto
Camp reserves the right, at its sole discretion, to withdraw any camper's enrolment at any time before or during the camp season. Going to the waterfront or
utilizing camp vehicles or assets without supervision or authorization or damaging camp property are examples of causes for dismissal. All travel and other
expenses incurred will be the parents responsibility. Camper lateness, absence, early departure or dismissal from camp is not a cause for a refund.
By completing this application, I hereby authorize Camp to take my child(ren) off camp grounds to go on trips organized as part of the camping program. This may
include swimming and/or boating activities. In addition my child(ren) may participate in any on or off ground activity organized by camp, including but not limited
to land sports, aquatic activities, ropes course, indoor activities, bicycling, hiking, cookouts etc. I understand the inherent risk of such activities and camp programs.
Camp will not be liable in the event of injury or property damage or loss as a result of such activities. I also agree to abide by all the rules and regulations set forth in
the application, parents guide and by the administration. Camp has the right to use photos of my child(ren).
I understand that Camp is not responsible for any damage, theft or loss of personal property for any reason. I agree to the conditions of enrolment,
payments, and refund policy stated on this application. Enclosed please find my deposit (application NOT VALID unless accompanied by full
deposit). I have read and accepted the enrolment, cancellation and refund policies .
