Its specialist field is investigating and learning from adverse events resulting in moderate to severe or catastrophic harm. Its managing director Maria Dineen was a key contributor to the National Patient Safety Agency’s (NPSA’s) original investigation model and framework for investigating adverse events in England. Her ‘how to’ investigate manual ‘Six Steps to RCA’ is a referenced text in the recently revised SI framework for England. CUK has been training NHS and Social Care professionals in ‘how to investigate effectively’ using systems analysis principles (including root cause analysis) since 2002, making CUK one of the longest standing providers of investigation training to the NHS since the inception of the NPSA. Why is this workshop important? This workshop will: Adverse Incidents are part and parcel of everyday life in all NHS Trusts, private providers and Social Care organisations.
