If you currently pay Income Tax or Capital Gains Tax, and are making a donation, Queen Elizabeth’s
Foundation for Disabled People (QEF) can claim back the tax on the sum, thus increasing the value
by 25p for every £1 you have donated. To make this possible, please complete and sign the
following declaration: I am a UK taxpayer and wish all gifts of money that I have made in the past 4
years and all future gifts of money that I make to QEF from the date of this declaration to be treated
as Gift Aid donations until I notify you otherwise. I confirm I have paid or will pay an amount of
income tax and/or capital gains tax for the current tax year (6 April to 5 April) that is at least equal
to the amount of tax that all the charities and Community Amateur Sports Clubs (CASCs) that I
donate to will reclaim on my gifts for the current tax year. I understand that other taxes such as VAT
and council tax do not qualify. I understand that the charity will reclaim 25p of tax on every £1 that I
have given.

is seeking £1m to provide the internal construction, fit out and
equipping of bedrooms and specialist therapy areas.
