Confused? Don’t sweat it—
we’re here to help you.
QCDR Reporting
The IRIS Registry can help you report
on quality measures in accordance with
the latest regulations. The Academy has
updated the IRIS Registry in accordance with
the most current regulations and submitted
the data required to meet new criteria in
2015, with no extra work on your part.
• Report nine measures from the PQRS
measures list. (If fewer than nine
measures apply, report IRIS Registry
QCDR measures too).
• Successfully map your EHR system to
the IRIS Registry by Aug. 1, 2016.
• Include measures from three quality
domains and at least two outcomes
measures.
• Submit your EHR data release consent
form to send your information to CMS by
Sept. 1, 2016.
How to Participate in the IRIS
Registry in 2015
If you have an EHR system, the easiest
PQRS options are the automated extraction
reporting mechanism for the IRIS Registry.
However, the deadlines for EHR-based
reporting have passed for 2015. (But if you
sign up for 2016 participation now, you’ll be
ahead of schedule for next year.)
You still have a little time left to report PQRS
manually (no EHR required) via the IRIS
Registry Web portal. There are two options:
Qualified Registry reporting or Qualified
Clinical Data Registry reporting. Use of
the Web portal is free for U.S. Academy
members and their staff.
Qualified Registry Reporting
• Deadline to sign up is Oct. 31, 2015.
• Report nine measures from the PQRS
measures list. (If you report fewer than
nine measures, you may be subject to
a Measurable Applicability Validation
review.)
• Include measures from three quality
domains and at least one cross-cutting
measure.
• Report each measure for at least 50% of
the Medicare patients who you see this
calendar year for whom the measure
applies.
• Deadline to sign up is Oct. 31, 2015.
• Report each measure for at least 50%
of all patients (including non-Medicare
patients) you see this calendar year for
whom the measure applies.

in your practice.
3. We will send you a questionnaire about
your EHR system and two agreements to
review and sign.
4. Once you return the questionnaire and
signed agreements, you will be placed
on a reservation list.
5. Your practice will be sent an email for
software installation to map your EHR
data to the IRIS Registry.

What is age-related macular
degeneration?
Age-Related Macular Degeneration
(AMD) is a problem with your retina. It
happens when a part of the retina called
the macula is damaged. With AMD you
lose your central vision. You cannot see
fine details, whether you are looking at
something close or far. But your
peripheral (side) vision will still be
normal. For instance, imagine you are
looking at a clock with hands. With
AMD, you might see the clock’s
numbers but not the hands.

Retina: Layer of nerve cells lining the
back wall inside the eye. This layer
senses light and sends signals to the
brain so you can see.
Drusen: Tiny white or yellow
particles that form under the retina.
They seldom cause vision loss. But
many (or very large) drusen can be a
sign of AMD.

Glaucoma is a disease that damages your eye’s
optic nerve. It usually happens when fluid builds
up in the front part of your eye. That extra fluid
increases the pressure in your eye, damaging the
optic nerve.
Glaucoma is a leading cause of blindness for
people over 60 years old. But blindness from
glaucoma can often be prevented with early
treatment.
With AMD, dark areas can appear in
your central vision.

What is glaucoma?
AMD is very common. It is a leading
cause of vision loss in people 50 years
or older.

Macula: Small but important area in
the center of the retina. You need
the macula to clearly see details of
objects in front of you.

A nerve at the back of your eye
that connects to your brain. The optic nerve
sends light signals to your brain so you can see.
Aqueous humor (“aqueous”): Clear liquid

inside the front part of our eyes. It nourishes
the eye and keeps it inflated. (Aqueous
is different from tears, which are outside
the eye.)
Drainage angle: The area of the eye where
the aqueous humor drains from the front of
the eye.
Iris: The colored part of your eye. It controls
the size of your pupil to let light into your eye.

In a healthy eye, excess fluid leaves the eye
through the drainage angle, keeping pressure
stable.
What causes glaucoma?
Your eye constantly makes aqueous humor. As
new aqueous flows into your eye, the same
amount should drain out. The fluid drains out
through an area called the drainage angle. This
process keeps pressure in the eye (called
intraocular pressure or IOP) stable. But if the
drainage angle is not working properly, fluid
builds up. Pressure inside the eye rises, damaging
the optic nerve.

The optic nerve is made of more than a million
tiny nerve fibers. It is like an electric cable made
up of many small wires. As these nerve fibers die,
you will develop blind spots in your vision. You
may not notice these blind spots until most of
your optic nerve fibers have died. If all of the
fibers die, you will become blind.
