AFFIDAVIT FOR SEARCH WARRANT

following property constitutes evidence of the commission of a
crime:
1.
DVD Video surveillance footage from Jane Phillips
Hospital Emergency Room, Examination Room #13 for the
night time hours of 0100 Hours to 0400 hours on Sunday
morning September 18, 2011.
and that the above mentioned property is now located at, and is
now being kept, possessed, and concealed by the resident, or by
other persons in whose possession the resident has placed it for
the purpose of concea~ent, at or upon or within a certain vehicle
and/or house, building or premises, the curtilage thereof and the
appurtenances thereunto belonging, in Washington County, Oklahoma,
described as follows:
Jane Phillips Medical Center, located 3500 SE Frank Phillips
Boulevard. Jane Phillips Medical Center is located north of
Frank Phillips Boulevard and east of Burch Street ~ It is a
multi-story medical facility, it is. located. in· the City of
Bartlesville, Washington County, State of Oklahoma,
based upon the following facts:
1. Affiant States that on Saturday night September 17 and 18,
2011 Mr. James Woods went to the Jane Phillips Medical Center
with Suicidal thoughts. At that time Medical Staff began the
process of evaluating Mr. Woods.
2. While Medical Staff began evaluating Mr. Woods, he became
agitated and the Jane Phillips Medical center Security called
the Bartlesville Police Department for assistance.
3. Affiants states that a Concerned Citizen was present in the
Emergency room at the time Mr. Woods was there. They reported
that several Bartlesville Police Officers responded to the
Emergency Room and to room #13 where Mr. Woods was.
4. Affiants states that the Concerned Citizen told me that Mr.
Woods had been handcuffed and became combative with the
officer. The Concerned Citizen told me that the officers
slammed Mr. Woods on the cot in the room, on the floor in the
room and that at one point one of the officers had choked him
to the point of unconsciousness.
5. Affiant states that the Concerned Citizen also told me that
after Mr. Woods was transported away from the Hospital a
..•..
second Emergency Detention Order came into the Emergency
Room. They told me that this EDO was a 17 year old male who
had assaulted two (2) females, torn a sink off a wall and was
threatening to kill or harm the Medical Staff if they entered
the room.
6. The Concerned Citizen told me that when Bartlesville Police
Officers arrived, they entered the room and had the Security
Guard handcuff the 17 year old. The Concerned Citizen also
told me that when the Officers discovered the male was 17
years old they told them there was nothing they could do and
left the 17 year old handcuffed with the security Guard.
7. Affiant spoke with Jane Phillips Medical Center Security
Supervisor Dale Lien. Mr. Lien reviewed the Video and told me
that be believed there evidence on the video that I would be
interested in viewing to determine whether a cr~e had been
committed.
8. Affiant states that the time of interest is between 1230
hours and 0400 hours on Sunday morning, September 18, 2011.
Based upon the above foregoing information your affiant requests
this court to determine there is probable cause for the issuance
of a search warrant for the above descr~ed property at the above
descr~ed location.

before me this day, based upon the facts stated therein, probable
cause having been found, I command you, during the hours of 6:00
A .M. to 10:00 P.M. (or anytime), to make immediate search of the
following described property and things, and if you find the same
or any part thereof, to bring it forthwith before me at my office
at Bartlesville, Oklahoma or before a magistrate who presides in
the 11th Judicial District:
