From Hwy #416 south, take the Fallowfield Road Exit and proceed on Fallowfield Road East until you come to Greenbank Road. Turn right and proceed to the second right, which is Malvern Drive. There is ample free parking surrounding the facility. Bus Route Information Walter Baker Sports Centre is serviced from the Fallowfield Transit Station by bus #173 and #170. Walter Baker Sports Facility was built and opened on October 4, 1980 then called South Nepean Centre. In November 1983, it was re-named Walter Baker Sports Centre after the late Walter Baker MP (Progressive Conservative Party) for Nepean–Carleton for his outstanding accomplishments for this area. Please have your credit card number and expiry date ready (Visa/MasterCard). You will need your Family PIN number and Client Barcode(s). 2 Touch Tone Telephone: Online/Touch Tone 10 pm August 8. at 613-580-2588. Please have your credit card number and expiry date ready (Visa/MasterCard). You will need your Family PIN number and Client Barcode(s). Receipts will be available upon request. Payment can be made by cheque, credit card, debit card or in cash. Cheques are made payable to the City of Ottawa. Cancellations Courses with insufficient registration will be cancelled. If a class or lesson is cancelled, every attempt will be made to re-schedule. If the class or lesson cannot be rescheduled, a refund will be issued. Refunds Visit your local recreation facility or any City of Ottawa Client Service Centre to withdraw from a program. If you would like a refund*, you must fill out and submit a Refund Request Form. No processing fee will be charged if the refund is credited to your account. A $25 processing fee will be charged for refunds by cheque or credit card. Refunds will be issued within 21 days of the receipt of the Refund Request Form. * If more than 50% of the course/membership has elapsed, no refund or credit on account will be offered. There are no refunds or credits for private lessons. For admission and goods purchases, refunds will be processed at the Facility with proof of purchase. Waiting Lists If the course you are interested in is full, you have the option of making another course selection or being put on a waiting list for the course of your choice (however, there is no guarantee that a space will become available). Participants will be contacted only if a space becomes available. Returned Cheque / Declined Credit Card Fee A service fee will be charged after two declined credit card payments and on all cheques returned as Non Sufficient Funds (NSF); Stop Payment; or Funds Not Cleared. Seniors’ Discount City of Ottawa residents who are 65 years of age and over will receive a discount of 10% when registering for a City program. A proof of age is required. Harmonized Sales Tax (HST) The Harmonized Sales Tax (HST) of 13% applies to recreation programs, services and materials where applicable and is included in the fees. Existing programs that were GST exempt remain HST exempt – for example, qualifying recreation programs for children 14 and younger and individuals with disabilities. Admittance Statement Users of City facilities are personally responsible for ensuring they are fit to participate in physical activities. Anyone with a condition affecting his/her ability to participate must take appropriate safety precautions to ensure his/her own safety and is encouraged to be accompanied by an adult who is knowledgeable about their condition. Medication Administration If the participant requires that City of Ottawa staff be responsible for storing, administering, or supervising the administration of any medication, the participant or parent/guardian is required to sign a release waiver and submit it to the facility where the program is being offered. Contact the facility for additional information. Severe Allergies It is the responsibility of the participant or parent/guardian to identify if there are severe allergies that require an auto-injector. The City of Ottawa attempts to ensure the safety and well-being of all participants with “allergen-safe zones and practices”. Participants or parents/ guardians are required to provide two autoinjectors and sign a release waiver to be submitted to the facility where the program is offered. Special Needs Participants or parents/guardians of participants are asked to provide information regarding special needs requirements before the start of the program. Ottawa Hand in Hand Recreation & Culture Fee Support The City of Ottawa offers financial support to ensure that all residents can participate in recreation and culture programs and activities offered by the City, regardless of their ability to pay. For additional information or to obtain a fee support application, visit your local recreation facility or any City of Ottawa Client Service Centre. Applicants will be asked to provide proof of financial need. Keep Your Receipts Children’s Fitness Tax Credit Many City of Ottawa Parks, Recreation and Cultural Services programs may be eligible for the Children’s Fitness Tax Credit. The Children’s Fitness Tax Credit is a federal, non-refundable, personal tax credit that lets parents claim up to $500 per year for eligible fitness expenses paid for each child who is under 16 years of age at the beginning of the year in which the expenses are paid. If a child qualifies for the disability tax credit, parents can claim up to $500 per year in eligible fitness expenses paid for the child who is under 18 years of age at the beginning of the year. Also, if at least $100 in eligible fitness expenses has been paid for the child, an additional amount of $500 can be added to the eligible fitness expenses actually incurred. Fees paid by parents must relate to the cost of a facility membership or registration in an eligible physical activity program. Your receipt will indicate whether the program that your child is registered in is eligible (see receipt wording). For more information on this credit, please see the Canada Revenue Agency website www.cra.gc.ca/fitness. The authority to determine whether any claim for a credit will be approved rests with the CRA. The City of Ottawa disclaims any responsibility or liability in connection with any determination of the CRA. 5 Heart Wise Exercise Programs Safe, fun, enjoyable and accessible classes In partnership with the University of Ottawa Heart Institute and other community agencies, Heart Wise Exercise programs are intended for participants who are interested or concerned about their heart health. To find Heart Wise Exercise programs look for the Heart Wise Exercise logo shown above. Heart Wise programs meet the following criteria: •Encourages regular, daily aerobic exercise •Incorporates and encourages warm up, cool down and self-monitoring with all exercise sessions •Allows participants to exercise at a safe level, and has progressive options to increase intensity if appropriate •Accepts participants with a known history of cardiac disease – provided they have physician approval •Provides health screening for all participants •Has an emergency plan that is documented and known to all exercise leaders, including the requirement of current CPR certification, phone access to EMS and presence of an AED These criteria could cover a variety of different levels and type of programs. Preschool Gymnastics 6 months to 5 year olds Our preschool programs are offered seven days a week mornings, afternoon and evenings. Dynamo & Spartan 5 & 6 year old boys and girls classes are 1.5 hours Young gymnasts are introduced to the fundamentals; the program focuses on basic skills, strength, flexibility, coordination and confidence. Our unique badge incentive program has been enhanced to include the first two levels of the CANGYM program, Burgundy and Red. Novice 7 & up boys and girls classes are 1.5 hours Gymnasts, on the various apparatus will focus on skill development, physical fitness and FUN! Your membership card must remain on your person throughout your stay. All City of Ottawa Class A Memberships are transferable to other Class A and B facilities (with exception to the Weight Loss and Conditionining Membership). Memberships on Hold You may put your membership on hold for a minimum of one month and only once during the course of your membership term. We apologize for any inconvenience to our clients. 28 Safety and Supervisory Standards To ensure the safety of residents swimming in City pools, certain supervisory standards will now be enforced when you visit recreational facilities. The Ontario Ministry of Health and Long-Term Care strongly supports these safety standards. The standards apply to pool swim sessions only – they do not apply to swimming lessons or to wading pools. Parents/ guardians with younger children are encouraged to view the Wading Pool Safety and Admission Requirements where the standards are different. PLEASE NOTE: If you are unsure of how these requirements apply to you and your family, please contact your facility of choice BEFORE your visit. • In order to ensure adequate supervision and to identify children at greater risk, the City of Ottawa is now issuing wristbands during swim sessions. All participants require a wristband to access swimming. • All Children 7 years of age and under, regardless of swimming ability, must be directly supervised (within arms’ reach) by a responsible person 16 years of age or older. • Children 10 years of age and under who are nonswimmers or who cannot pass the facility swim test must be directly supervised (close proximity and able to render assistance) by a responsible person 16 years of age or older. • Non-swimmers and those who cannot pass the facility swim test must remain in shallow water. Each ratio below indicates the maximum number of children that one responsible person would be able to supervise. For example: Children 5 years of age and under: maximum 2 children per responsible person 16 years of age and older. The person responsible must be in the water and within arms reach. Facility Swim Test: Participant must be able to demonstrate comfort in the water and swim 25 meters in shallow water without assistance or a lifejacket. Participant must complete the entire distance, swim continuously without stopping to rest, show strong forward movement and ability to take breaths and not touch the bottom of the pool. *High Risk Participants are defined as those who have a condition or illness, which may put them at risk in an aquatic environment (e.g. frequent seizures, fainting condition, etc) and those who are unable to control behaviour or impulses and require direct supervision. **Supervisory Ratios for Groups of High Risk Participants to be determined by pool staff based on the specific needs of the participants and associated risks. The Nepean Masters Swim Club (NMSC) is 32 years strong and the largest Masters swim club in Canada.
