I, (We) a resident(s) of Mesa Village Homeowners Association (are) in agreement that upon the
installation, use and removal of an astro jump or Pony ride within the Association and release
Mesa Village of any obligation or liability that may arise. I will place/operate it in an agreed
location, which upon approval will be designated on this release. Further more I, (We) will not
install it any earlier than 9AM and remove it no later than 5PM on the approved day.
I, (We) a resident(s) of Mesa Village Homeowners Association understand that the management of
the association may cancel my request at any time.

must be at least 18 years old and have a valid Mesa Village Pool ID. Bring this form to the
office Monday Thru Friday between the hours of 8AM-3PM to submit your request.
