About Maryhaven
Maryhaven is a nonprofit, Long Island based
agency committed to helping persons with
physical disabilities, mental disabilities and
substance abuse/dependence issues without
regard to age, race, and/or religious beliefs to
realize their full potential. The agency’s purpose is to serve this population by establishing,
maintaining, and operating progressive programs and services which are residential, educational, and vocational in nature. The ideal goal
is the attainment of normalization, in all
aspects of life, for each individual in our care.

• All individuals with developmental disabilities
have a right to timely and appropriate health
and mental health rehabilitation and social
support services to maintain their well-being
in the community.
• All individuals with developmental disabilities
should be able to identify their own valued
outcomes and be supported in attaining these
outcomes.

OUR MISSION
The mission of Medicaid Service Coordination is
to link and assist individuals with developmental
disabilities to have access to the services and supports needed to enhance and maintain themselves
at a level of functioning where they have personal
satisfaction and live in the environment of their
choice. The following values are inherent in this

community in which they live.
MEDICAID SERVICE COORDINATION
Medicaid Service Coordination is an initiative of
the New York State Office for People with
Developmental Disabilities Office (OPWDD). It is
targeted to offer services to individuals with a
diagnosis of a developmental disability.
Utilizing a person-centered philosophy and approach,
services are determined by the circumstances, needs
and desires of each individual. Together with the
individual and the advocate of their choice, the service coordinator assists in the development and
coordination of services and supports. These
individualized services will empower the person
towards independence, inclusion, and productivity.
The Individualized Service Plan is a guide for
self-determined outcomes. The “ISP” is reviewed at
regular intervals and incorporates input from team
members who will assist the individual to improve
the quality of their life.
Coordination of services is an ongoing process.
Contact with the individual at home, at day activities or in the community, are part of this process.
This contact, along with input received by ancillary
contacts, allows the individual to discuss their
ongoing progress with the service coordinator and
allows the service coordinator to address any personal or safety issue needs.
ELIGIBILITY
Medicaid Service Coordination serves individuals
who have a documented diagnosis of a developmental disability. Individuals may refer themselves
to Service Coordination by contacting Maryhaven
Center of Hope directly or by having a vocational
counselor, school counselor or therapist contact
Maryhaven directly. Proof of a developmental disability must be provided in the form of a current
psychological evaluation, current psychosocial evaluation along with a current physical.
Individuals will be assisted to obtain and maintain
financial entitlements, such as Medicaid. Medicaid
Service Coordination is funded by Suffolk County
Medicaid. Individual financial circumstances are
discussed privately.
