On-site forms For the purpose of proper and timely construction as specified, designed and to the Employer‘s satisfaction, onsite quality control procedures shall be applied from the commencement to the completion of the Works. Attached forms are blank – filling forms implemented to large spectrum of construction from delivery of materials to site to hand-over formalities. However, the most important form is ITP (inspection & testing plan) which is utilized for the closed step-bystep inspection and testing for individual works on site. Prior to be fabricated, installed, poured, covered etc., all parts of work must be scrutinized against construction drawings, specifications and approved in writing by both FDC ‘s supervisors and the Employer ‘s representative. III. Project Quality Plan This Project Quality Plan is project specific and incorporates the Client’s requirements with the FDC system procedures. The plan is a working document that is used to control all aspects of quality for this project. The distribution list shows the distribution for controlled copies of documents and ensures the removal of obsolete documents from use. There are four main parts to the plan: Section 1: General Section 2: PQP Control Section 3: Personnel responsibilities Section 4: Project Procedures The first section, General, provides information in regard to how the Plan is structured and operates, along with definitions of terms used throughout the Plan and reference documents that relate to the Plan. The second section, PQP Control, provides an index and serves as the distribution and revision status control register, and shows the authorization for the Project Quality Plan. The third section, Personnel responsibilities, identifies the FDC personnel involved with the project and describes their roles and responsibilities. The fourth section, Project Procedures, provides summarized information on standard FDC system procedures for this project to provide a user-friendly interpretation for everyday use. This section therefore takes precedence over the FDC systems procedures. FDC procedures project record and applicable forms are included as Appendix A. Procedures Manual FDC System Procedures: the documented system that FDC applies across all its operations Project Quality Plan (PQP) The project specific quality plan that incorporates the specific requirements of the clients and FDC’s systems procedures. Work packages are made up of work lots and are typically the responsibility of one organisation. Work Lot A “work lot” that can be completed from start to finish without dependence upon any other work lot once commenced. The work lot is the basic unit for programming and quality assurance. Internal Audit A quality system audit by FDC personnel in regard to FDC and their subcontractors. 2nd Party Audit a quality system audit by the Client in regard to FDC operation on this project. 3rd Party Audit a quality system audit by an accredited organisation. These document form as a basis in which all our projects are executed. With established procedures, consistent methods of proper execution are obtained resulting in the quality level desired by the Client. The key to any procedure is its use and the individuals executing them. We take pride in the qualities of the individuals we have assembled that provide the services discussed in this section. Highlights of quality control methods and systems employed are as follows: IV. Geographical, cultural and social conditions pertaining in Vietnam are also considered in the implementation of process for occupation health and safety. Accupational health and safety is regarded as a prerequisite in all its company operations . No employee of FDC is required to work in an unsafe work place. Continual Improvement FDC policy is to involve all its workforce in the implementation, measuring and continual improvement of safe work practice. Responsibility and involvement at all levels means employment ownership of safety and the desire for its effective implementation. PERSONNEL SAFETY Each and every person (including all visitors) working on a project should expect to take home from work in the terms of bodily functions What they brought with them. Each and every person should understand that the person who is injured or whose health is damaged in any way could very well be them self. 3. THE BROAD PLAN The plan is set out in the main with dot points for inclusion/ expansion as necessary or exclusion on a “needs basis”. The safety system should be simple and practical to manage. There are thousands of issues and sub issues concerning safety consequently is would be impractical to provide coverage for all in a broad based safety plan as items will always be missed. The question is What to “put in” and What to “leave out”. This plan includes for safety procedures that should be carried out on all FDC projects. The system within the plan is open ended: the items can be condensed or expanded to suit each sites needs. 4. MEASURABLE Site safety must be measurable. General Safety and environmental protection Program FDC has a long tradition of job safety in the workplace and on the site. We are very aware that Job Safety is mandatory in this Project. All project procedures and working shall be in compliance with the all safety rules and statutory requirements applicable at the job site. The requirements for proper safety apparel including, but not limited to, safety helmets, boots, eye protection, hand protection, hearing protection and respiratory protection shall be complied with. The same standards will be written and specified to subcontractors and imposed on them carrying out work in this Project. The environmental protection during the course of construction, such as: noise, dust, etc., at the site is the first considerations in operating our business. Environmental protection during the course of construction must form as part of every operation and are a part of every employee's responsibility at all levels. We intend to comply with all laws concerning the environmental protection during the course of construction of Owner and the public. II.1 General Objectives No employee is required to work at a job known to be unsafe or dangerous. The willingness to co-operate in detecting, reporting and controlling dangerous situations and workplace hazards is required, as safety is the responsibility of each employee. Each individual is required to notify their supervisor immediately of any dangerous situation, which is beyond their ability or authority to correct. II.2 General safety measures Highlight Highlights of the safety measures are as follows: - Chief of Security & Safety Control Group with similar project experience. - Chief of Security & Safety Control Group do not assume, but verify if there is a question concerning a safety issue. - Chief of Security & Safety Control Group make the Owner's Safety Rules and Regulations an integral part of the overall site safety program. - Chief of Security & Safety Control Group conduct initial meetings with the subcontractor's field staff to clearly outline project expectations for safety. These discussions include expectation of the subcontractor to adhere to their respective safety programs. - Chief of Security & Safety Control Group attend daily, weekly and monthly meetings with all workmen to emphasize safety in the work place. - Chief of Security & Safety Control Group continually check safety in the work place and document deficiencies - Chief of Security & Safety Control Group are expected to spend a portion of their time in the work areas reviewing on-going work, work in place and asking the right question prior to an, activity commencing so that the probability of it being done in safe manner is greatly enhanced. - All staffs and workers who have been engaged by FDC must firstly attend the safety induction to be got used to with the safety regulations and procedures of the projects as well as Client’s special requirements before starting. Page 1/3 HEALTH & SAFETY MANAGEMENT III. - Security & Safety Control Group at site is required to make Job Safety Analyse each work of worker teams, in which it is needed to identify the potential hazard and control measures. - To equip warning for work safety at vulnerable positions such as dangerous signs, no smoking signs, and safety first signs… - To equip fire-fighting tools such as air extinguisher, powder extinguisher… for vulnerably inflammable areas. - Temporary first aid service will be installed on the site to provide first aid cares in case of work accidents. The staff in charge of first aid service shall be competent enough for such work. They shall be present at site round the clock so that works accidents whatsoever occurring at any time can be dealt with properly and professionally. - A Local hospital with ambulance car equipped with suitable bed and stretcher shall be put on convenient records for immediately calling to transport the wounded after first – aid performance. - The wounded and dead due to construction works shall be insured and would be indemnified in compliance with currently valid legislation issued from Socialist Republic of Vietnam. Construction site safety measures Highlight These highlights shall be read in conjunction with the FDC Construction Company general safety procedures hand book III.1. Keep workplace clean A clean workplace is safer for everybody on the site, so all people must have responsibility to keep their workplace and amenities areas clean including: III.2. III.3. Be alert to other workers, machinery and others on the operating areas Scaffolds – working platforms ♦ All scaffolds and working platforms must be erected on the level and solid ground, and must all times kept confirming to these requirements III.7. ♦ Bracings must be fully fixed prior to allowing the persons working on the scaffolds. ♦ Mobile scaffolds must not be moved while persons are on them ♦ Proper access must be provided to all scaffolds and working platforms ♦ Adequate and proper protective guard or hand rails must be provided for wherever it is required Concrete pumping ♦ Operators must be trained in the use and operation of the machinery involved and is aware of the dangers with associated with the pumping procedures. PROJECT MANAGER Project Manager is accountable for and has the overall control of all HSE matters on site, such as: Providing the HSE plan and exercise good HSE discipline over all personnel participating in SCQC. Ensuring that all levels of supervisors are qualified with appropriate training. Describing and applying similar responsibilities as outlined above for their organization and their HSE plan shall be approved by SCQC. b. Training and Communication 8.1 - Safety induction Before working on site, full project personnel including staff, existing employees, new employees, visitors are required to attend the safety induction course conducted by safety team. Site medical room shall be equipped with necessary facility and drugs for first aid and medical cares. To make it easy for employee to identify the location of the medical station on site, one sign boards marked with Red Cross in big size shall be posted. 8.11 Accident, incident reporting - - Successful incident/accident prevention requires a minimum of four fundamental activities: + A study of all working areas to detect and eliminate or control physical or environmental hazards that contributes to incident/accidents. + A study of all operating methods and practices. + Education, instruction, training, and discipline to minimize human factors that contributes to incident/accidents. + For cause analyses, a thorough investigation of at least every incident/accident, which results in a disabling injury to determine contributing, circumstances. + Accidents that do not result in personal injury (so-called “incidents” or “near- misses”) are warnings. They should not be ignored. + The fourth activity, cause analysis, is a secondary defense against hazards, which may have been overlooked, in the first three. + In connection with this activity, an investigation should be made of every + Incident/accident, which results in a Lost Time Injury (LTI) of more than 3 work day losses. Other incidents/accidents can be investigated, especially if there is frequent recurrence of certain types of injuries, or if the frequency of incidents/accidents is high in certain areas or operations. Written reports include detail the nature of the accident, the time and date of the occurrence and the precise location, as well as other pertinent factors that may have contributed to the occurrence. All statutory notices sent to the enforcing authorities concerning the occurrence shall be copied to Owner. Trang 14/30 HEALTH, SAFETY & ENVIROMENT PLAN - - Copies of reports of all incidents reported to the governmental safety authorities shall also be sent to Owner. Full details of any witnesses must also be obtained and any breach of statute or permit to work arrangements must be recorded. Where personal injury has occurred, a medical report shall be obtained and included with the written report or submitted immediately afterwards. The site manager and of the Subcontractor involved in an accident/near miss will be required to investigate and give a detailed account of the circumstances of the incident. All accidents / near misses resulting in or having potential for injury, damage or loss shall be reported to Owner and, where an injury - however slight - occurs, it must be recorded in both FDC and Owner's accident books. Any incident or dangerous occurrence involving plant, workers, Equipment shall be reported immediately to Owner's Safety Officer, both verbally and in writing. 8.12 Hazard Analysis and Risk Management - - - - For each phase of the major work a Job Safety Analysis (JSA)/Risk Assessment (RA) shall be developed with usage of the form S040 and S041. Job safety analysis or risk assessment analysis shall be maintained by the job supervisor and be available for review by employees involved with task. Copies of completed JSA/RA shall be kept in the site HSE file. The analysis shall be job specific and address at a minimum the following areas: + Activity being performed including all of the major tasks + Sequence of work + Health and safety hazards include chemical, physical, and ergonomic stressors in the hazard analysis + PPE must be included on the JSA and reviewed with personnel prior to starting work. + Control measure or precaution actions include PPE, fall protection measures, fire protection measures, barricades, work practices etc. Safety Engineer, Safety Officer and site engineers are responsible for JSA and preventive actions for all site operations. After being approved by site engineer, the documents will be notified to all staffs as official documents. Safety Engineer, Safety Officer and site engineer will review JSA/RA and revisions together with relevant staffs to ensure relevant staffs understand the requirements and expectation. The JSA/RA shall be updated whenever there are significant changes in the work plan, materials used, work environment, or a new crew or subcontractor is assigned to conduct the work. Any changes/revisions to the JSA/RA shall undergo the review process as the original JSA and assigned a revision number. 8.13 Safety Lock-Out/Tag Out program - If any employees are required to perform hazardous maintenance or repair of machines and equipment, a formal, written Lockout/Tag out Program shall be developed and implemented. This program must include machine-specific procedures for shut-down and re-energizing equipment and training for "affected" employees (i.e., those who operate, service or work near the equipment in question). Trang 15/30 HEALTH, SAFETY & ENVIROMENT PLAN - - These procedures must be followed during the servicing or maintenance of machines, to avoid the unexpected start up of the machinery or equipment, or the release of stored energy, which could cause injury to employees. All employees should be familiar with lockout/tag out procedures for their company's operations and a review of these should take place periodically. Procedures will vary, depending upon whether the source of hazardous energy is and how many employees are affected. 8.13.1 The following are a review of steps for safely de-energizing equipment - Notify all "affected employees" that the equipment will be shut down. Shut down the equipment by normal stopping procedures. Isolate" all the equipment's energy sources. Lock out and/or tag out the energy isolating devices with assigned, individual locks. Release or restrain any stored energy by grounding, blocking, bleeding down, etc. Assure that no personnel are exposed, then test the equipment to assure that it will not operate. 8.13.2 Restoring Equipment to Service: - Check to assure that all employees have been safely positioned or removed from the area. Verify that equipment controls are in neutral. Remove lockout devices and/or tags and re-energize the machine or equipment. Notify affected employees that servicing is complete and the equipment is ready for use. 8.14 Emergency Response Plan - This plan is to be organized and operated when emergency conditions have occurred. The period, size, and other details of the plan shall be determined by the Site Manager assisted by Safety Manager. All staff and workers shall act according to the emergency instructions from the plan except for the approval of Project Manager. 8.14.1 Emergency Contact List. - - - Emergency Contact list including phone number, fax, number, and other information shall be posted on the notice Board at all times. Any one who discovers a fire or other emergency situation at the work site shall inform Safety Manager. Safety Officer, or by phone or other available fast way, pressing a siren at the nearest point in the vicinity of the emergency. Safety officer is responsible for organizing company’s resources for fighting the fire. Safety officer is responsible to make a report (form) about the fire and submit it to the Site Manager and authorities. 8.14.3 Emergency state from toxic release - If toxic gas leakage or toxic liquid release takes place, take emergency action is as follows: + Action to be taken in case of possible gas leakage or toxic release: immediately stop working and using gas, a person who finds the possibility of damage from high- pressure gas or toxic liquid should immediately report to the site office and call for aid necessary for fire fighting activity. If the emergency state threatens to cause explosion of high pressure gas, all persons there should escape from the dangerous area under the guidance of the Safety Officer or other related personnel. When doing hot work, such as welding or gas cutting, stop hot work and switch off the welding generator and shut off gas valve at the gas cylinder. + Action in case of gas leaks. In case of leakage from between the cylinder valve and the pressure regulator, close the valve immediately. In case of leakage from the cylinder valve, take the following steps: • Leak from the charge port with the valve closed: this case means that the valve seat has been damaged or restricted. Open it ¼ turn and the close completely. If the leak continues, cover the charge port with a blind plug or cap and then give a notice to the seller. • Leak from between the spindle and gland nut: This case means that the gland packing has been loosened. Close the valve and tighten the gland nut. If the leak still continues, first close the valve and then give a notice to the seller. • • Take care when moving a leaking gas cylinder as it is likely to topple aside and cause danger of fire; Leakage from hose Immediately close the cylinder valve in the cylinder storage 2) Promptly ventilate the room and watch for flame. 3) Replace the hose with new one; As gas leak may cause a fire, use extreme care so that working clothes of chemical fiber may not generate static electricity or fire, or that working shoes may generate metal impact, friction, or sparks. The following action is prohibited within 10m from the high pressure gas cylinder storage area: 1) Using flame; 2) Leaving dangerous substances ; 3) Stockpiling explosives; 4) Using bare electric wires; 5) Other dangerous behaviors corresponding to the above prohibitions 8.14.4 Emergency state from injury accident. Bleeding from injury + Apply direct pressure on various points on the body to stop bleeding. It is possible to stop arterial bleeding by pressing the artery with finger or thumb against the underlying bone. To control bleeding by direct pressure, apply a pad of sterilized dressing, bandage firmly, Trang 17/30 HEALTH, SAFETY & ENVIROMENT PLAN - - - - - - - adding, if need be, sterilized cotton wool, finally apply a triangular bandage. If bleeding cannot be controlled by direct pressure, immediately send to the first aid room for treatment by the nurse. Bone fracture + Do not attempt to move casualty with fractured bones or injured joints. Secure the injured parts with triangular bandages so that they cannot move. An injured leg may be tied to the uninjured one, and an injured arm tied to the body. Immediately send to hospital for treatment. Emergency resuscitation & external cardiac massage: + Electrical Shock: Switch off the power supply. If this not possible, free the victim by using something made of rubber, cloth or wood or a folder newspaper. Do not touch the victim’s body before the current is switched off. If breathing is failing or has stopped, start CPR procedure. Get help and send for a nurse and ambulance. Burns and Scalds + If serious, send promptly for first aider / nurse and ambulance. Put sterilized dressing on the burn or scald. Never use an adhesive wound dressing. If extensive, cover with clean towels and secure loosely. Do not burst blisters or remove clothing and flush the burn with plenty Falling from height + When a person was fallen from a height, anyone who is nearby the scene shall try to do the following: Call First Aider who will decide on the level of first aid required; If the victim is unconscious, lay the victim on his side to prevent tongue obstructing him/her stop breathing. Pay special attention to the wounds at his neck. Using antiseptic to rub and clean the surround of the wound from inside to outside. If the limbs are injured, the injured limbs must be lift up to reduce blood Use the bandage to cover the wound; Tie bandage but not too tight, Use tourniquet to stop loss of blood if other method fail; After that send victim to the nearest hospital or health service for further treatment. Excavation Collapse Rescue + In the event of personnel becoming trapped by a collapsed excavation, emergency medical and rescue services, such as the local fire department, shall be contacted immediately. + Site personnel shall not be allowed to enter the excavation to attempt rescue as they may become trapped if the excavation is continues to collapse. + Mechanized equipment shall not be used to attempt rescues as additional injury to victims may occur. Rescue from Heights + In the event a rescue from heights is necessary the local fire brigade be contacted to conduct the rescue. + Information about the specific location and height at which personnel are trapped should be provided at the time of the call to ensure that the local fire brigade arrives on site with the specific ladder truck. + If the employee can be reached safely using a secured ladder/ Scaffoling on site rescue may be conducted. Lightning + Safety Officer shall monitor weather conditions. An inexpensive portable weather radio will provide regular weather condition updates. + In the event of a forecast for thunderstorms and especially severe storms, the weather forecast should be monitored throughout the day. Trang 18/30 HEALTH, SAFETY & ENVIROMENT PLAN + + - - If severe weather is imminent, outdoor operations should be ceased and request all employees on site to take shelter. First Aid Recommendations for Lightning victims First - Call 115 if available to provide directions and information about the likely number of victims. Evacuation • In an active thunderstorm, the rescuer needs to choose whether evacuation from very high-risk areas to an area of lesser risk is warranted and move the victim rapidly if necessary. • Rescuers are cautioned to minimize their exposure to lightning as much as possible. Resuscitation + If the victim is not breathing, start mouth-to-mouth resuscitation. If it is decided to move the victim, give a few quick breaths prior to moving them. Determine if the victim has a pulse by checking the pulse at the carotid artery (side of the neck) or femoral artery (groin) for at least 20-30 seconds. If no pulse is detected, start cardiac compressions as well. In situations that are cold and wet, putting a protective layer between the victim and the ground may decrease the hypothermia that the victim suffers which can further complicate the resuscitation. In wilderness areas and those far from medical care, prolonged basic CPR is of little use: the victim is unlikely to recover if they do not respond within the first few minutes. If the pulse returns, the rescuer should continue ventilation with rescue breathing if needed for as long as practical in a wilderness situation. However, if a pulse does not return after twenty to thirty minutes of good effort, the rescuer could stop resuscitation. Safety Rules + Go quickly inside a completely enclosed building. + If no enclosed building is convenient, get inside a hard-topped all-metal vehicle. + Do not take shelter under a tree. + If there is no shelter, avoid being the tallest object in the area. If only isolated trees are nearby, crouch on the balls of your feet in the open, keeping twice as far away from a tree as it is tall. + Avoid leaning against vehicles. + Avoid open spaces, wire fences, metal clothes lines, exposed sheds and electrically conductive elevated objects. + Do not use metal objects like golf clubs, fishing rods, tennis rackets or tools. + Do not work on fences, telephone or power lines, pipelines, or steel fabrications. + Stop tractor work and heavy construction equipment, especially when pulling metal equipment, and dismount. All drivers of vehicles will have current driving license relating to the class of vehicle being driven. No person may drive a vehicle unattended while in possession only of a provisional license. Before the equipment come to the Site, all driver license and inspection certificate of the machine must be checked and approved. Therefore, all vehicles will be regularly inspected and maintained in a condition for use on public roads whether they are to be used on such roads or not. No item of plant, mobile or otherwise, will be allowed to operate on the Site by Owner unless the user can supply evidence of its inspection, maintenance, or repair where appropriate, to the standard required by the relevant legislation or the manufacturer's requirements. All vehicles entering or leaving the Site carrying loads which constitute a potential dust or dirt nuisance such as aggregate, sand, gravel, soil or peat, shall be secured such that no material shall leave the vehicle. FDC shall be responsible for keeping highways adjacent to the Site free from mud, droppings or other materials that are due to its operations and safeguard Owner from any liability. Contractor is expected to take reasonable care to ensure that its vehicles entering the Site are free from mud on the wheels or tracks by washing out mud at washing station at the site gate, and that loads are secure. In the event of fouling of the roads due to its operations, Contractor will clean them up expeditiously at its own expense to the satisfaction of Owner. FDC shall be responsible for enforce its sub-contractors to comply the following requirement during refueling of mobile equipment and vehicles: + Prohibition of smoking + Turning off ignition during refueling + Prevention of fires and spillage + Provision of "No Smoking" and "Turn Off Ignition" signs 8.18 Safe working practices - 8.18.1 Excavation Stormwater & underground water drainage systems must be available at excavation area Surveilance of excavated holes must be carried out continuously. Shoring system will be applied (if necessary) Access to excavated holes must be installed properly (provision of at least 2 permanent access) Install safety hard barricade (top rails, midrails, toeboards) or safety bands with warning signs 1m away from excavated holes. Trang 22/30 HEALTH, SAFETY & ENVIROMENT PLAN - - Do not work in excavated holes when excavators are operating or somebody is working on ground surround excavated holes Excavated soil must be minimum 0.5 m away from holes and slopes of excavated oil are less than 45 degree. Excavation permit from authorities must be obtained before excavating areas where there are underground services beneath. Underground service plan and protection method must be available. Whenever underground piping system is different from the plan or finding bomg, landmine, amunition, excavation must be stopped for proper solution. Excavation must be stopped and workers evacuate whenever encountering toxic gas Double-check hole sides before pouring concrete. Strengthening with shoring system if land slide appears. Attention to electrical safety. Cables must be hung above ground - 8.18.2 Welding and Welding Machines Sufficient fire extinguishers shall be equipped at workplace. A ground return cable shall be in good condition and have a safe current carrying capacity. All arc welding and cutting machines shall be earthed. Where necessary arc and welding operations, shall be shielded. The welder must inspect all ground connections to ensure its conditions. Sparks, hot metal or slag must not be allowed to fall on any combustible materials or personnel. The employee doing welding works must determine the location of the nearest fire extinguisher. Housekeeping around the welding and cutting operations shall be carried out. Cables shall be laid out properly not to obstruct passageway. - - - 8.18.3 Electrical Safety Electrical equipment shall be suitably earthed. All external electrical connections/plugs are to be of weatherproof design. Use of ELCB for all switchboards. Leads shall be run so as not to present a trip hazard. Weather protection shall be provided to all exposed electrical connections. Only cables in good serviceable conditions shall be used. Cables shall be disconnected and coiled when not in used. Cables running across vehicular access must be protected from damage. Cables shall not be kinked, knotted, cut or crushed as this may damaged the insulation. Only qualified electricians will repair or fix damaged electrical equipment. Maintain a lock-out/tag-out system for all isolated machinery and distribution systems. Maintenance Electrician shall remain responsible for the control of all switchboards/fuse panels associated with the yard’s system during the entire period. Supply cables shall not be laid out on flooded areas (min.height of cables raise 2.5 meter). If feasible temporary cable tray or hangers shall be provided. 8.19 Lifting work safety - Plan the move before lifting: remove obstructions from your chosen pathway. Test the weight of the load before lifting by pushing the load along its resting surface. Trang 23/30 HEALTH, SAFETY & ENVIROMENT PLAN - If the load is too heavy or bulky, use lifting and carrying aids such as hand trucks, dollies, pallet jacks and carts, or get assistance from a co-worker. If assistance is required to perform a lift, coordinate and communicate your movements with those of your co-worker. Position your feet 6 to 12 inches apart with one foot slightly in front of the other. Face the load. Bend at the knees, not at the back. Keep your back straight. Get a firm grip on the object with your hands and fingers. Use handles when present. Never lift anything if your hands are greasy or wet. Wear protective gloves when lifting objects with sharp corners or jagged edges. Hold objects as close to your body as possible. Perform lifting movements smoothly and gradually; do not jerk the load. If you must change direction while lifting or carrying the load, pivot your feet and turn your entire body. Do not twist at the waist. Set down objects in the same manner as you picked them up, except in reverse. Do not lift an object from the floor to a level above your waist in one motion. Set the load down on a table or bench and then adjust your grip before lifting it higher. Slide materials to the end of the tailgate before attempting to lift them off of a pick-up truck. Do not lift over the walls or tailgate of the truck bed. Do not use load hooks that are cracked, bent or broken. 8.20 Crane or Tower Crane - Only operators with license and safety conducted are allowed to operate cranes. Do not use cranes that do not have their rated load capacity indicated on each side of the crane or on its load block. Passengers are not permitted to ride inside the operator's cab of a truck crane. Keep crane windows clean. Do not use a crane if its windows are broken. Do not exceed the rated load capacity as specified by the manufacturer. Do not operate a crane on soft ground without using cribbing and mats. Fully extend outriggers before attempting a lift. Stay outside the barricades of the posted swing radius. Do not perform any crane refits or modifications without the manufacturer's approval. Do not leave the crane unattended with a hoisted load. Do not hoist loads over people. Do not drive on the road shoulders. Wear a high visibility vest when working as a signal man. Only follow the signals of the person designated to give you signals when operating a crane. Replace the belts, gears or rotating shaft guards after servicing a crane; do not use the crane if guards are missing from these areas. Strictly follow manufacture guideline for crane in terms of load charts, overload alarms, reverse alarms, lifting slings, ropes, hooks, shackles, latches, etc. In order to avoid every body can cranes/machinery, the authorized operators shall wear orange cloths to for easy identification. 8.21 Work at height Trang 24/30 HEALTH, SAFETY & ENVIROMENT PLAN - Safety belt must be used for working at the height of more than 2m. Hooks must be secured to firm structures. Use full body safety harness (using big hook and without shock). Training must be on ducucted on how to use properly safety harness, regular inspection of harness conditions. Moving along designated routes and locations. Climbing up and down is prohibited. No walking on wall top, beam top, rafter, roof truss and structures under construction. Going up and down through firm-fixed ladder. Do not carry heavy, bulky objects when using ladder. No playing, Do not drink alcohol before and during time of working at height. Tools must be kept in bags. Do not drop any tools or any objects from the height Do not work on high scaffolds, chimney, water tower, high colums, bridge stumps, roofs of building with more than 2 storeys ect. when heavy rain, stormy, strong winds of more than type 5. Fall protection procedures (use of lifeline/ static lines, safety net, canopies,etc ) 8.22 Scaffolding safety - - Scaffold must be designed, explained, calculated and approved by site manager Do not use scaffolds which deform, have cracks, erosion or miss structural elements. Gap between working platform and project is less than 0.3m (otherwise, additional solution must be applied). Distance from scaffold to transporting equipment is not less than 0.6m. Scaffold must not be rested on unstable surfaces. Do not load on scaffolds at un-specified locations. Do not load on scaffold ladders. Working platform must be equip with 1.0m high handrail with 02 horizontal bracing to avoid falling. Access under scaffolds must be protected. Electrical safety method must be applied when scaffolds are installed adjacent to LV cables. Before working hours, site engineer and safety officer have to check scaffolds to ensure safety conditions. Then “safe scaffold” tags will be hooked up for working. Only competent scaffolders shall erect/dismantle scaffoldings. Do not working on scaffolds under heavily rainy or stormy conditions. 8.23 Hoist safety - Hoist must be checked and maintained regularly. Hoist operator must be trained on hoist operation. Before operating, hoist must be checked (cables, cogs, power supply) to ensure safety requirements are met. Install fence/mesh around hoist. Do not over-load hoist Do not stop hoist on sky when the stoppage last for long time. After working hours, clean working area and turn off power supply. 8.24 Power hand tools safety - Workers who chip/drill metal or concrete with hand tools must wear safety glasses, gloves, masks, ear-plugs. Wokers who use power/pneumatic hand tools do not stand on ladders. They have to stand on floor or safe working platfom. Trang 25/30 HEALTH, SAFETY & ENVIROMENT PLAN - Workers who use electrial welding machines must wear welding shield and glasses as regulated with fire extinguishers aside. When power failure, CB must be turned off. Do not supply powers to un-used hand tools. Clean hand tools daily and maintain regularly. All hand tools with metal cover must be earthed or connected to ELCB. Steel Erection - Steel erection works for this project consists of motorcycle shed, pipe rack and temporary storage yard. - The steel erector shall provide with documentation that footing, piers, and walls have sufficient strength to support loads imposed during steel erection. - Information on any repairs, replacements, and modifications to anchor bolts (if any) shall also be provided. - Adequate access roads are maintained and that a firm and properly graded is provided for storage of materials and operation of equipment. - All hoisting and rigging operations shall be preplanned including: + A pre-shift visual inspection of the crane and rigging equipment shall be completed by a competent person. + Routes for suspend loads shall be preplanned to ensure that employees are not working directly below a load. + Working and walking surfaces shall be maintained free of tripping hazards including shear connectors, reinforcing bars, anchors, or threaded studs which may project vertically from beams or other work surfaces. + Metal decking shall be secured against displacement at the end of the shift or when environmental conditions warrant. Decking shall not be removed until immediately prior to installation of permanent equipment or fixtures. + All materials, equipment and tools used at heights shall be secured from falling and flied with wind when not in use. + Fall protection shall be required for all employees working at heights greater than 6 feet (2m) (Refer working at height). Demolition - Competent Person will perform an engineering survey and establish a demolition plan to determine the condition of structure(s), location of existing utilities, protection, isolation, removal and/or rerouting of such utilities and safe demolition sequence. - Work must only begin after a JSA/RA has been completed and communicated to anyone who could be impacted by the planned activity. - However, in this project the demolition only dealing with pile head cutting off shall require safety practice for concrete cutting. 8.25 Fire Prevention and Protection - Office, welfare facility, accommodation, store, laydown area or temporary facility of any kind will be erected or moved with proper reasons FDC shall provide and maintain 16 fire extinguishers for office area and 16 more for hot work areas. Trang 26/30 HEALTH, SAFETY & ENVIROMENT PLAN - - All items of plant and equipment - e.g., cranes, welding sets - will each have a dedicated fire extinguisher of a suitable type. All work carried out under a "Hot Work" permit will have dedicated, suitable fire extinguishers. All fire extinguishers shall be inspected by a certified company on yearly bases and shall be in accordance with local fire codes. Fire extinguishers shall also be visually inspected for any changes by FDC on a monthly basis and records kept of the visual inspection. 8.26 Signs and Barricades - - - Post/erect signs, signals or barricades necessary to advise personnel of hazardous conditions in work areas. Signs and warnings shall be in the Vietnamese and English when required and/or use pictograms to communicate hazard. The barrier shall be constructed to withstand adverse weather conditions and site construction traffic. Hard barricades will be needed for excavations, near roof edges, around overhead work and similar areas. Hole covers or hard barricades (guard rail with toe boards, safety net) must be placed at all floor openings. All holes or openings through floors or decking at all elevations must be provided with hole covers or barricades immediately. 8.27 Housekeeping - Clean working space is safer working space. All must be responsible for keeping working space clean by the following methods: + Move rubbish out of site regularly + Stack/store material tidily at specified locations + Keep access, staircase clear of obstacles + Do not put combustible materials adjacent to fire sources + Remove protruding nails + Keep W.C clean 8.28 Permit to work procedure - - 8.28.1 Purpose To provide, via the formal application of a permit system, an effective control of specific construction operations that have a high potential to cause injury to persons, damage to property, or an adverse reaction on the environment. This document is to be regarded as an integral part of the HSE Plan. The use of the work permit: This permit to work is a checklist to ensure that all the hazards are identified, and the necessary precautions are taken to eliminate or isolate the hazards of the work to be performed. 8.28.2 Scope - - This Work Permit System covers the following construction tasks: + Hot work + Excavation works + Scaffolding (working at height) 8.28.3 Issuing Procedure Application for a work permit has to be prepared by a working group of FDC / FDC subcontractor. Trang 27/30 HEALTH, SAFETY & ENVIROMENT PLAN - - - It is the responsibility of SO to examine the permit and to ascertain what work has to be carried out together with the hazards involved after which he will complete the necessary parts covering HSE measures and precautions to be taken. SO will give his authorization for the work to start by signing the appropriate permit. The maximum validity of a FDC work permit will be one day from the date of issuance. 8.28.4 Disciplinary procedures Penalty System + Staff and workers of FDC / FDC subcontractors who exhibit a poor attitude toward safe work and safety procedures will not be allowed to continue to work. + Employees removed from this job due to safety violations will not be allowed to Return to work without proper authorization. + The Company and its representatives will impose a penalty system for people violating the rules and regulations as described in this document by means of the Yellow / Red Card system. + A violator will receive a yellow card, and he/she will be immediately expelled from site for that day and one following day. And right after the employer of the Violator will be fined depending on the level of risk posed by the unsafe acts or unsafe conditions. See HSE-19Offence & Disciplinary Actions. + A person who has received 2 (two) yellow cards, will be given a red card. A red card involves expulsion from the site. Depending on the seriousness of the violation, the Site Manager will decide the duration of the expulsion with minimum of one week. The FDC 8.29 Independent inspection of equipment on site - - Tower cranes, rubber-wheel mobile cranes, track-wheel mobile cranes, lifting appliances (hoist), excavator, slings, suspended working platforms (cradles, gondolas, etc), mechanical vehicles working on site are to be inspected and approved by authorised agents of Vietnam Accessories only require C.O. Tools only require approval from the site management (include: material platform, etc). It must be properly designed and should adequately carry its intended load, safety working load/ capacity designs must be approved by professional engineer before use 8.30 Work Permit - 8.30.1 Purpose To provide, via the formal application of a permit system, an effective control of specific construction operations that have a high potential to cause injury to persons, damage to property, or an adverse reaction on the environment. This document is to be regarded as an integral part of the HSE Plan. 9. FDC shall ensure his subcontractors’ crews shall comply with the Waste Management Plan by binding his obligations under subcontract wording. All waste containers and waste storage will be clearly labeled and posted. 9.3 Implementation Plan On-Site/Pre-construction + Reuse any and all possible construction materials on-site. + Take materials to one of the reuse stores in the area. + Work with subcontractors to prevent waste. + Ask subcontractors to reuse or recycle their own materials. + Ask suppliers to deliver supplies using returnable pallets and containers. Have them pick up the empty containers when delivering new supplies. + Set up central cutting areas for wood, fabricated reinforcement and other materials. Make sure the crew uses all the reusable pieces before cutting a new piece. + Store materials in places that prevent loss from weather and other damage. Trang 29/30 HEALTH, SAFETY & ENVIROMENT PLAN + - Re-evaluate estimating procedures to make sure the correct amount of each material is delivered to the site. Continuous supervision of bin for timely emptying. Special Note - This Health – Safety – Environment Plan for Project is written in English and Vietnamese. In case there are any discrepancies between English version and Vietnamese version, Vietnamese shall prevail. 10. Welfare and sanitation - All employees and workers’ health shall be examined in regular period as required by laws and insured under insurance policy for worker’s compensation. - FDC shall establish and maintain basic hygiene provisions for all employees in all working place as specified as follows: + All employees shall maintain a good personal hygiene and shall not wash, bath or drink using water from unauthorized sources. + Water container for drinking water shall be kept clean and washed regularly. They shall be clearly marked and shall not be used for any other purpose. + Defecation and urination outside the toilets are strictly prohibited. + The FDC will provide on-site toilets in sufficient numbers and will ensure that they are cleaned daily and serviced regularly. 11. Control substances hazardous to Health - Toxic - Toxic materials or chemicals are generally considered as poisons that can cause short-term or long-term sickness or even death. Exposure can be through inhalation, physical contact. High level work for structural steel/roofers/scaffolders: soft shoes may be worn while working “in the air”. Returning to the base level soft shoes must be changed to hard enclosed footwear. 12.
