Change begins with you - Benefits
2013 Change begins with you 2013 Open Enrollment is Oct. 15–Nov. 15, 2012 With your help, we can get a handle on health care costs. Change begins with you. Our hectic lives seem to leave us with less time and energy to focus on our health, and as a result, we are spending more time in the doctor’s office and more money on prescriptions to manage our health issues. Our health care costs seem to keep going up. But we should remember that ultimately, we can make an impact in the way we utilize health care by considering the cost and quality of the care we receive. In this guide, read about Doug, Cathy, Ed and Amy, who have taken small steps toward changing their health using the tools and support J.B. Hunt provides. They did it, and you can too. Six years ago, Doug Gibbs, an assistant foreman at J.B. Hunt in Austell, Georgia, thought he would have to take insulin for the rest of his life. The prospect of the daily injections prompted him to start riding a bicycle, and he hasn’t looked back. Not only has he lost a lot of weight, he doesn’t have to take any medication for high blood sugar. So this year, take the first step toward feeling better AND save a little money in the process. You MUST go online during Open Enrollment Oct. 15–Nov. 15, 2012, to see if you can get discounts on your 2013 medical premiums. There are two ways for you to save in 2013: ASSESS YOUR HEALTH 1 You’ll save $5 per week on your medical premiums if you take an online Health Assessment to learn more about your overall health status. If you cover your spouse, and he/she also takes the assessment, you’ll receive an additional $5 discount each week. BE TOBACCO-FREE You’ll receive a weekly $5 discount on your medical premiums if you are tobacco-free for at least 30 days prior to Nov. 15, 2012. If you cover your spouse, and he/she is also tobacco-free, you’ll receive an additional $5 discount every week. 2 If you and/or your covered spouse need help quitting tobacco, call your Personal Health Team at 877-459-9896. What happens if you don’t go online and enroll? If you and your family are currently enrolled in a J.B. Hunt medical plan but don’t go online and enroll, you will continue to have coverage but will pay the higher, non-discounted medical rates in 2013. Don’t forget to review and update your personal information when you go online and enroll this year. (Provided through Cigna) Your 2013 medical plans J.B. Hunt offers three medical plans that will provide you and your covered family members the protection you need for everyday health issues or when the unexpected happens. All our medical plans have these same features: • Certain preventive care services are covered at 100%. If you go to a network provider, you can get services like immunizations, mammograms, Pap tests and flu shots at no cost to you. • Choose from a wide variety (a “network”) of medical providers and you’ll save. Not only will network providers charge you less for their services, you’ll never pay more than your out-of-pocket maximum.* • Prescription drug coverage at no additional cost. Managing your weight is about more than just looking good. According to the Centers for Disease Control, obesity is responsible for 90 to 95% of all Type 2 diabetes. The HSA Medical Plan is your best value This plan offers the best value at a lower premium, AND you can contribute dollars to a Health Savings Account to help pay your portion of qualified medical expenses such as your deductible and coinsurance, and you can change your contribution amount at any time. You can also use your HSA dollars to pay for expenses that aren’t covered by health plans such as some over-the-counter medications. One great feature of a Health Savings Account is if you have money left at the end of the year, it rolls over year-to-year—you don’t lose it. But that’s not all! You get a TRIPLE tax advantage when you enroll in and contribute to a Health Savings Account. Here’s how: 1 Pre-tax payroll deduction—you don’t pay income tax on your Health Savings Account contribution amount Use this mobile app to: Submit claims, view your account balance and much more at www. connectyourcare.com/ cyc2/technology.html. Android 2 Tax-free earnings—interest and monies earned on your Health Savings Account are tax-free 3 When you use money in your Health Savings Account, you don’t have to claim it on your tax return iPhone Plan Employee only If you cover dependents HSA Medical (in-network) • $1,325 annual deductible • $5,500 annual out-of-pocket maximum* • $2,650 annual deductible • $11,000 annual out-of-pocket maximum* HSA Medical (out-ofnetwork) • $1,325 annual deductible • If you go to a provider who is not in the network, there is no limit to what you will pay. • $2,650 annual deductible • If you go to a provider who is not in the network, there is no limit to what you will pay. The most you can contribute each year** • Up to $3,250 (you can change your contribution amount anytime) • Up to $6,450 (you can change your contribution amount anytime) * There is an annual maximum of $2,000,000 but no lifetime maximum. ** If you’re 55 years or older, you can make additional catch-up contributions to your Health Savings Account of up to $1,000 in 2013. 2 3 PPO/OAP Medical Plan This plan has a lower annual deductible than the HSA Medical Plan, but the premium you pay will be significantly higher each pay period. You’ll pay a copay each time you see a network primary care provider even after you’ve met the deductible. Plan Cathy Bolton is a senior Information Systems project manager at the corporate office. Her husband, Scott, was diagnosed with a rare condition of the diaphragm. Cathy reached out to Advance Medical. She says, “Having this benefit was phenomenal and gave us the opportunity to have the quality of life we want.” Good news! Effective Jan. 1, 2013, J.B. Hunt’s FSA waiting period will be reduced to 90 days! That means your FSA will become effective on your 91st day of full-time employment, and if you meet the eligibility criteria, your FSA will become effective on Jan. 1, 2013. Employee only If you cover dependents PPO/OAP (in-network) • $35 copay* • $700 annual deductible • $6,000 annual out-of-pocket maximum** • $35 copay* • $1,400 annual deductible • $12,000 annual out-of-pocket maximum** PPO/OAP (out-ofnetwork) • $1,400 annual deductible • If you go to a provider who is not in the network, there is no limit to what you will pay. • $2,800 annual deductible • If you go to a provider who is not in the network, there is no limit to what you will pay. * Copay amount applies to in-network primary care providers only. Specialists will be treated as all other covered services. ** There is an annual maximum of $2,000,000 but no lifetime maximum. Kaiser Medical Plans Kaiser offers HMO plans in certain areas. Go to myjbhunt.com/benefits to find out if you are eligible to enroll in an HMO and to view details of the HMO medical plans. Use pre-tax dollars and save A Flexible Spending Account, or FSA, is a great way to use pre-tax dollars to pay for child care or for qualified medical expenses that may not be covered by your medical plan. The way an FSA works is pretty simple. You determine how much you want to put in your account each week to pay for services like daycare or other qualified health care expenses. You’ll get a debit card to pay for any expenses. It’s that easy. There are a couple things you need to know about FSAs: • You get the same triple tax advantage that a Health Savings Account provides. • You can’t change your contribution amount during the year unless you have a “qualifying” event like having a baby or if you get married. Maximum contribution amounts for 2013 are: • $2,500 Health Care FSA • Up to $5,000 Dependent Care FSA (based on your tax filing status) • If you don’t use all of the money during the year, you lose it. You can enroll in a Health Care FSA if you think you’ll need additional tax-free dollars to pay for medical expenses. If you enroll in a Health Savings Account and a Health Care FSA, your FSA would be limited to use for dental and vision expenses only. For more information on FSAs, go to the Benefits tab on myjbhunt.com or call the HR Solution Center team at 800-777-4968. You can enroll in the Limited Dental Plan, a “bridge plan” that will help bridge the gap until you are eligible to enroll in the Full Dental Plan. Keep in mind, your coverage in the Limited Dental Plan will end on your first anniversary of employment. In order to ensure you don’t have a lapse in coverage, you can enroll in both the Limited and Full Dental Plans during your initial eligibility period, and premiums and coverage for the Full Dental Plan will not start until the day following your one-year anniversary. Hunt will match $.50 for each dollar you save up to a maximum of 3% of your pay. You must contribute at least 6% of your pay to receive the maximum company contribution. And since your contributions are taken out of your paycheck before taxes are calculated, you will save even more. Your annual pay is: J.B. Hunt will give you: $20,000 $30,000 $40,000 For example, if you save 6% of your pay and… $600 $900 $1,200 THINK YOU CAN’T AFFORD TO SAVE? ACTUALLY, YOU CAN’T AFFORD TO WAIT. You don’t have to save a lot each paycheck to build a nice nest egg for your future. That’s because the money you save now has time to grow. Even if the economy is uncertain, investing your savings over a long period is one way to make sure you have money to help during your retirement. Let’s look at an example of Arnie who is 30 years old and makes $40,000 a year. If Arnie saves a small percentage of his pay in the J.B. Hunt 401(k) plan: 2% 3% 4% 6% A cup of coffee each work day (about $15/week) Cable TV with premium channels (about $25/week) Two large pizzas with double pepperoni (about $30/week) Shopping trip (about $45/week) In 30 years, Arnie could have saved as much as: ** $100,000 $150,000** $200,000** $300,000** Just think—if you skipped your weekly pizza and saved some money instead, you could have a nice nest egg for the future.
