South County Hospital to give the best
possible care. Achieving this level of quality for
two and one-half years is really a distinction.”
The scores (A, B, C, D or F) are assigned
to hospitals based on preventable medical
errors, injuries, accidents, and infections.
The Hospital Safety Score was compiled under
the guidance of the nation’s leading experts
on patient safety and is designed to give the
members of the public information to protect
themselves and their families.
The Leapfrog Group is a nationally
recognized, not-for-profit, independent
healthcare transparency advocacy group.
The Hospital Safety Score uses national
performance measures established by the
Leapfrog Hospital Survey, the Agency for
Healthcare Research and Quality, the Centers
for Disease Control and Prevention, and the
Centers for Medicare and Medicaid Services.
A hospital’s overall performance in keeping
patients safe from preventable harm and
medical errors determines its score.
According to The Leapfrog Group,
“the Hospital Safety Score is one piece of
information consumers should use when
planning a hospital stay.” The group
recommends that healthcare consumers
seek out and use hospitals that receive an
“A” Hospital Safety Score.
According to Leapfrog, the safety
measurements considered represent the very
basic blocks of medical care, such as computerized medication control, nurse staffing levels,
and even hand-washing. One of the most
significant problems in today’s healthcare is the
failure to make safety and quality information
available to the public so that consumers can
make informed choices about where to receive
care. The purpose of the Hospital Safety Score
is to bring this information to light in a way
that is easy for consumers to understand.

Population Health Management:
A Shift in Perspective and a New
Role for Your Community Hospital

“Population health management” is a
term you’ve probably come across recently.
You may have seen it in the newspaper or
heard it during a radio or television news
report. Chances are, though, you don’t
fully understand it. While it’s a concept that may sound remote
and even cold, what it translates into is something good—and
far from remote—especially for the younger generation. But,
truly, population health management is good for all of us,
including folks with chronic health problems, no matter
what our age. It’s also the way of the future, particularly for
community hospitals. We are, therefore, embracing it.
At the heart of population health management is a belief
that the primary role of a healthcare system and/or hospital is
to improve the state of health of the individuals living within
its reach through education, preventive care, and direct active
support of an environment that allows and encourages members
of that community to engage in healthy lifestyles. A key tenet
of this philosophy is that hospitals must take deliberate steps to
move away from focusing only on the effects they can have on
health once a person is within the walls of their facilities.
This perspective represents a significant shift from the
traditional model of providing episodic care, usually at a crisis
point in a person’s state of health, and then sending him on
his way. While there will always be a need for urgent medical
attention, and in a hospital setting—serious fractures, systemic
infections, Cesarean section births, and so on—much of what
we do could become unnecessary if we did a better job of
fostering good health before disease gets a chance to begin.
Medical science and technology have progressed at an
amazing pace over the past generation. We marvel at what
good these advances can do. As a society, we are spending
more on healthcare and using healthcare more than ever. At
the same time, however, we’ve seen an increase in rates of
obesity, diabetes, cancer, heart disease, lung disease, and other
conditions that can be largely prevented. So, what’s going on?
What we have learned is that we are putting a lot of
resources in the wrong places. Now we marvel at the
recognition that we can take relatively simple steps to improve
and safeguard health. That view forms the heart of population
health management, and it is reshaping the mission of
organizations such as ours.
That’s why we’re sponsoring a series of cooking classes
that will teach healthful cooking on a low-income budget,
offered by Thundermist; why we’re meeting with local school
administrators on how we can impact the health of young
people; why we’re placing nurse care managers in primary care
practices to help patients with chronic disease (diabetes, heart
disease, and so on) learn how to improve their health and
avoid visits to the hospital; why we’ve launched a community
health team that includes “peer navigators” to work with
high-risk individuals who need help accessing behavioral
health, transportation, and financial counseling to break
the cycle of poor health.
We welcome this role, and I invite you to be part of it.

Wakefield Concert Band To Dedicate Spring Performance
to Hospital’s Cancer Rehabilitation Program
On Sunday, May 3, at 4:00 p.m. at South Kingstown High
School, the popular Wakefield Concert Band will dedicate all
proceeds from its “Kaleidoscope of Sound” concert to support
South County Hospital’s STAR™ Program, a multi-disciplinary
approach that helps individuals who are dealing with cancer.
The STAR Program was developed by Julie Silver, MD (herself
a cancer survivor and a member of the faculty at Harvard Medical School) to promote understanding of the healing process
and to help survivors regain strength and quality of life.
Since 2008, the Wakefield Concert Band has raised more
than $90,000 to benefit the Hospital. By supporting the Band’s
concert in May, you will be helping many in your community.

The orthopedic surgeons of the South County Hospital Orthopedics Center

Wakefield Welcomes New Primary
Care Group
Care for all ages, and every stage of life
The physicians of South County Medical Group Primary
Care, (L-R) Mark Zullo, MD, Gloria Sun, MD,
Hana Hagos, MD, and Heather Mackey-Fowler, MD,
are delighted with their new home, a spacious, newly
renovated suite of offices in the Medical Office Building
on the South County Hospital campus. All are accepting
new patients. To make an appointment with any of the

Dr. Gloria Sun, board certified in internal medicine, has been practicing primary
care medicine in Wakefield since 1995, so her name is not new to the community.
What is new is the South County Medical Group Primary Care practice, which
includes Dr. Sun and three other physicians, board certified in family medicine and
specializing in primary care: Drs. Zullo, Hagos, and Mackey-Fowler.
Dr. Sun has special expertise in women’s health. Dr. Zullo has a special
expertise in sports medicine. All focus on preventive care.
As a member of the South County Medical Group, SCMG Primary Care
can offer patients an exceptional level of coordinated care across 18 different
medical specialties—all sharing the same electronic health record system.
