Article 6. The form of Request for Issuance and Delivery for Revision of Official Passport to be submitted under the provisions of Article 9 paragraph 3 of the Law, when issuance and delivery for revision of an official passport is requested for, shall conform to the annexed Form 7. Article 7. The form of Application for Re-issuance and Delivery of Ordinary Passport to be submitted under the provisions of Article 10 paragraph 1 of the Law by a person who, after having been delivered an ordinary passport, desires re-issuance and delivery because the said passport has been lost, burnt, or badly damaged, or in the case of outside the country the space for receiving visa has been used up, shall conform to the annexed Form 8. Article 8. The form of Application for Re-issuance and Delivery of Official Passport to be submitted under the provisions of Article 10 paragraph 3 of the Law, when re-issuance and delivery of an official passport is requested for, shall conform to the annexed Form 9. Article 9. The form of Passport Receipt as provided for in Article 20 paragraph 2 of the Law shall conform to the annexed Form 10. ORDINANCES Ministry of Foreign Affairs Ordinance No. 26 December 1, 1951 In accordance with the provisions of Article 22 paragraph 1 of the Passport Law (Law No. 267 of 1951), the Ministerial Ordinance concerning the Forms of Application for Issuance and Delivery of Ordinary Passport and Other Documents shall be established as follows: Minister for Foreign Affairs YOSHIDA Shigeru Ministerial Ordinance concerning the Forms of Application for Issuance and Delivery of Ordinary Passport and Other Documents Article 1. The form "of Application for Issuance and Delivery of Ordinary Passport and the form of Statement of Personal Status to be submitted under the provisions of Article 3 paragraph 1 of the Passport Law (Law No. 267 of 1951, hereinafter referred to as "the Law") by a person desiring to have an ordinary passport issued and delivered to him, shall conform to the annexed Forms 1 and 2 respectively. Article 2. The form of Request for Issuance and Delivery of Official Passport to be submitted under the provisions of Article 4 of the Law, when an official passport is requested for, shall conform to the annexed Form 3. Article 3. The form of Application for Addition of Destinations in Ordinary Passport to be submitted under the provisions of Article 8, paragraph 1 of the Law by a person desiring to make addition of destinations after having applied for issuance and delivery of an ordinary passport, shall conform to the annexed Form 4. Article 4. The form of Request for Addition of Destinations in Official Passport to be submitted under the provisions of Article 8 paragraph 4 of the Law, when an addition of destinations in an official passport is requested for, shall conform to the annexed Form 5. Article 5. The form of Application for Issuance and Delivery for Revision of Ordinary Passport to be submitted under the provisions of Supplementary Provisions : This Ministerial Ordinance shall come into force as from the day of its promulgation. 2 The following Ministerial Ordinances shall be abolished : Overseas Passport Regulations (Ministry of Foreign Affairs Ordinance No. 8 of 1935) Ministerial Ordinance concerning the Forms of Application for Issuance of Passport and Other Documents (Ministry of Foreign Affairs Ordinance No. 2 of 1950) Ministerial Ordinance concerning the Forms of Application for Issuance and Delivery of Overseas-issued Passport and Other Documents (Ministry of Foreign Affairs Ordinance No. 15 of 1951) 1 £ / (Size: Form1 Application Attach side of graphs. spelling also. Those who with respect to anexport contract concluded prior to the coming into force of this Ministerial Ordinance have received a letter of credit (including a document equivalent thereto) or any other kinds of foreign currency payment means as of the day of the coming into force of this Ministerial Ordinance and who, moreover, have yet made no report as mentioned in Article 3, proviso, item (1) of the Regulation for Restriction on Use of Nickel, etc. (hereunder called "Regulation"; hereunder this shall apply) prior,to the amendment as of the day of the coming into force of this Ministerial Ordinance may receive the confirmation by reporting to the Chief of the Bureau of International Trade and Industry on the use of nickel, etc. to manufacture articles based on the contract concerned. With respect to articles on the report on the use of nickel, etc. as made in accordance with the provision's of Article 3, proviso, item (1) of the Regulation prior to the amendment prior to the day of the coming into force of this Ministerial Ordinance or to articles on the report as confirmed in accordance with the provisions of the preceding paragraph the provisions of Article 3 of the Regulation shall not apply for the 90 days of the day of the' coming into force of this Ministerial Ordinance. No person shall use articles as manufactured on reporting as mentioned in the preceding paragraph (including semi-finished articles) for other purposes than those as described in the report concerned. The provisions of Article 6, proviso of the Regulation shall apply mutatis mutandis to the cases as mentioned in the preceding paragraph. The provisions of Article 7 of the Regulation shall apply mutatis mutandis to those using nickel, etc. to manufacture articles as described, in the report as mentioned in paragraph 3. 7 With respect to cases and outer frames of the time-piece under manufacture as of the day of the coming into force of this Ministerial Ordinance the provisions of Article 3 of the Regulation shall not apply for the 30 days of the day of the coming into force of this Ministerial Ordinance.
