Recent refinements in fractional laser technology (called ablative fractional laser treatment) make the technology even more exciting. Ablative lasers actually remove microscopic tubules of skin, while leaving the surrounding area untreated, thereby allowing the skin to heal more quickly. (A slightly discomforting analogy might be to think of ablative laser treatments as lawn aeration. Tiny plugs are selectively removed, thereby enhancing the overall health and appearance of the remaining surface.) How do ablative treatments compare to standard fractional treatments and laser resurfacing? Let’s take a look. Certainly laser resurfacing (laser peel) treatments remain the most effective alternative when it comes to removing aged, wrinkled, scarred, pigmented and sagging skin. Ablative treatments follow in second place; and standard fractional laser treatments come in third, based on improvement noticed per treatment. Shouldn’t everyone choose laser resurfacing then, since it’s the most effective? Not exactly. While resurfacing offers the most noticeable results, it also requires the most downtime—one to two weeks. Hence the reason for the positive buzz surrounding new ablative treatments. While they are not quite as effective as laser resurfacing peels, the associated downtime is just 2-4 days before makeup can be applied. Skin may stay red or discolored for two weeks or more after treatment, but downtime reduction is significant. While regular fractional treatments are the least effective of the three, there is no downtime at all, only redness and mild swelling. In addition to downtime, it’s also important to consider the number of treatments you will need to achieve the desired results. Laser resurfacing requires just one treatment for maximum results. Ablative treatment requires one, perhaps two, depending on the conditions being treated. Traditional fractional treatment requires approximately five spaced treatments to reach maximum effectiveness (but with no downtime). Which treatment is right for you? In truth, there is a place for each, depending on skin condition, desired results, and recovery schedules. Ablative treatments are exciting because they offer greatly reduced downtime over laser resurfacing. However, all of these technologies, if used correctly, can give excellent improvement in skin texture, and significantly reduce wrinkles, scars, prominent pores and other skin irregularities. Nutritionists today are confirming what common sense has dictated for decades—if you want healthy skin (or any other part of your body), you need to eat healthy foods. The B family includes B1, B2, B6, B12, niacin, folic acid, biotin, and pantothenic acid. Vitamin B1 aids skin health by improving circulation. Proper amounts of vitamin B2 or riboflavin can reduce brown pigmentation and aid oily skin. Vitamin B6 or pyridoxine will improve dermatitis or eczema. The B group of vitamins is also involved in making red blood cells, which carry oxygen throughout your body— critical in ensuring your skin retains its pink, healthy tone. While diet and healthy skin go hand-in-hand, balance is the key. Over-emphasizing one food in preference to others may actually deprive your skin of essential nutrients and do more harm than good. These advances usually take the form of light and/or laser treatments, injections, or most often, topical creams and serums. Skin care is always evolving, with new products and treatments being introduced almost as frequently as news on Britney Spears. And oftentimes, the advances in skin care divide the top experts–one person’s miracle in a jar is another’s flash in the pan. However, I recently stumbled upon a very interesting magazine article (Allure, December 2007), featuring eight skin innovations that have received unanimous praise from the most prominent dermatologists and cosmetic chemists in the country. These products live up to their promises and have truly changed the face of skin care. It’s hard to imagine life without them! 1. Retinoids: “Retinoids are the one topical treatment actually proven to regenerate collagen” says Ranella Hirsch, president-elect of the American Society of Dermatologic Surgeons. There is also evidence that they smooth the surface of the skin, even skin tone, increase blood flow, and prevent clogged pores. 2. Botox: FDA-approved in 2002 to treat frown lines, Botox is now most often used to make movement softer and to diminish persistent wrinkles. Most dermatologists agree that this drug has miraculous potential, but it is not to be taken lightly and requires close monitoring. 4. Alpha Hydroxy Acids: Alpha Hydroxy Acids (AHAs) are a family of acids that include mandelic acid, malic acid, glycolic acid, and citric acid. Hydroquinone: Most often called a “bleaching cream”, hydroquinone is a tyrosinase inhibitor which prevents new skin cells from darkening. Potentially it’s a good solution for those suffering from melasma or hyperpigmentation. 7. Antioxidants: Antioxidants combat the free radicals that are generated by the sun, cigarette smoke, and environmental pollution. Antioxidants protect the skin against DNA damage, discoloration, and wrinkles. Common topical treatments with this protection are vitamin C serums, CoffeeBerryTM cream, green tea serums, and idebenone products. 8. Sunscreen: This is the biggest innovation of them all. No so fast. Beautiful skin is no accident. How many babies do you know, for example, with bad skin? The question that should be asked is, what can we do to keep our skin “baby” beautiful? Start Protection Early The most important key to lifelong beautiful skin is sun protection. Without it, the damage of ultraviolet light on the skin begins early. (For example, freckles in children are an early sign of s u n damage.) Then, as we age, the damage continues to accumulate. Over a lifetime, the result is premature aging, brown spots, thinning skin, broken blood vessels, damaged collagen and elastin, and wrinkled, sagging, rough, 30 g a t e w a y aesthetic Institute & Laser Center and coarse skin—and in its worst form, skin cancers, including deadly melanomas. Don’t think if you opt for a tanning booth that you outsmart nature. While the sun’s rays have a broader spectrum, the artificial rays produced in tanning booths can be equally damaging. When choosing a sunscreen or sunblock, protection from both UVA and UVB light is necessary to avoid sun damage. We recommend products from both SkinCeuticals and La Roche-Posay, since they cover a broad spectrum of both the UVA and UVB ranges (something drugstore lotions don’t). And remember, when you are outside, it is recommended that sunscreen be applied liberally every two hours to give maximum protection. Wide-brimmed hats, sun-protective clothing, and shade are also important weapons in your skin’s battle against the sun. Control Acne One of the most devastating skin problems people experience is facial scarring caused by acne. Some children will start to get acne lesions as early as 10 years of age. The condition typically worsens after puberty and may affect some people for years into adulthood. To keep skin beautiful, treat acne early to prevent or reduce it and the permanent scarring that may otherwise result. Simple acne may respond to daily cleaning or over-thecounter products like benzoyl peroxide or salicylic acid. More stubborn cases are best treated by a dermatologist who can prescribe stronger medications. For many patients, aggressive control of acne during their teenage years will be sufficient to prevent scarring. However, others who have a strong family history of acne may need to undergo potent acne treatments for many years. Remove Dots and Spots As we age, many of us will develop age spots, freckles, melasma (uneven patches of pigmentation), scars, cysts, and papules. In addition, pores often become more prominent as oil glands age. If any of this sounds familiar, no worries. The good news is that these unsightly skin conditions can be efficiently treated with the right treatments or products. Using a product with hydroquinone can help control melanin production. We really like the Obagi Skincare Line which has a variety of products with hydroquinone. However, hydroquinone should not be used long term so please see your skin care professional before you begin. Additionally, the use of vitamin A and/or alpha-hydroxy acid can be beneficial. Not only will these products help correct and treat, but they will also help maintain and prevent. Moisturize Maintaining the integrity and protective quality of the skin can be achieved through frequent use of moisturizers and barrier repair products. In a study comparing Retin A to good moisturization, vigorous use of moisturizer and sunscreen was almost as effective as the prescription Retin A treatment. You should look for a moisturizer with hyaluronic acid to help replenish water loss which occurs with age, stress, dry climates, high altitude, etc. A good suggestion is layering a hyaluronic acid gel, like SkinCeuticals B5 gel, underneath your moisturizer to help lock in moisture. Products with ingredients like aloe vera, shea butter, and glycerin (to name a few) can help. We really like Immuderm, which is a glycerin and shea butterbased moisturizer that not only hydrates, but helps to heal and soothe the skin as well. Experts agree that acne is not caused by greasy foods, but rather by the skin’s sebaceous glands producing excess oil that combines with normal skin bacteria to clog pores. The methods for fighting acne are many; however, a treatment that works for one person may not always work for another. It all depends on one’s body chemistry. That said, one treatment that is garnering the praise of many doctors and patients is OmniluxTM light therapy. What is Omnilux light therapy and how does it work? Omnilux light therapy consists of alternating sessions of exposure to red and blue wavelength light. For each session, makeup or sunblock is removed and then the Onmilux light will be positioned close to the face for 20 minutes of exposure. It’s simple, safe, and easy with a complete treatment course usually consisting of 8-12 visits. Omnilux treatment may also be combined with prescription acne medications for greater benefit. Certainly aesthetic dermatology is an area where we see “waves of hysteria.” What do I mean by hysteria? As new procedures become available, it’s common for facilities to promote them at frantic levels before the complete value of the procedures is understood. In my professional experience, which spans over 30 years, I have seen many touted treatments survive, and many that have not. Let’s look at a quick example. In the mid-1990s, laser skin resurfacing swept the country like wildfire. After a few years, however, it fell into disrepute because too many untrained physicians entered the market offering poor treatment that left the skin waxy, hypopigmented, and scarred. The procedure wasn’t to blame, but rather the inexperience of those performing it. Sometimes, even with an experienced provider, a much-hyped procedure simply doesn’t live up to its potential. For example, laser treatment of spider veins. Though the procedure received significant attention when it was first introduced, it didn’t outperform standard sclerotherapy (injections). While there is a place for the new technology (veins too small to inject), the “old” treatment still offers the best results for most patients. What I hope everyone understands is that the key to a successful treatment is not found in chasing after the latest procedure, but rather in relying on a provider with both knowledge and experience to know when to chase and when to wait. In the case of laser resurfacing, for example, we have performed over 1,000 procedures. It remains the gold standard for our patients who want to rid their faces of the maximum number of wrinkles, scars, and sun damage. Today, body shaping devices are the latest craze to hit the industry. These are machines that claim to manipulate fat beneath the skin’s surface to give you the body shape you have always wanted. Soon, every mom & pop medi-spa will be touting the machine’s benifits. And while some shaping may be possible, I haven’t seen any of these devices that, when used by themselves, come close to the excellent results we are seeing with LipoDissolve, SmartLipo, and tumescent liposuction. Instead, I believe these machines are best when used as a follow-up treatment to LipoDissove, SmartLipo, and other lipo treatments. If you believe the hype and expect life-changing results from the machines alone, you will be dissapointed. What is the moral of the story? Rely on experience, not hype.
