Ablative Fractional Laser Treatment
F
ractional laser treatments (where lasers treat just a fraction
of the skin’s surface) have been extremely successful at
improving skin texture, minimizing scars, and reducing
stubborn pigmentations. Recent refinements in fractional
laser technology (called ablative fractional laser treatment)
make the technology even more exciting. Ablative lasers
actually remove microscopic tubules of skin, while leaving
the surrounding area untreated, thereby allowing the skin to
heal more quickly. (A slightly discomforting analogy might
be to think of ablative laser treatments as lawn aeration.
Tiny plugs are selectively removed, thereby enhancing the
overall health and appearance of the remaining surface.)
How do ablative treatments compare to standard fractional
treatments and laser resurfacing? Let’s take a look.
Certainly laser resurfacing (laser peel) treatments remain
the most effective alternative when it comes to removing
aged, wrinkled, scarred, pigmented and sagging skin. Ablative
treatments follow in second place; and standard fractional
laser treatments come in third, based on improvement
noticed per treatment.
Shouldn’t everyone choose laser resurfacing then, since it’s
the most effective? Not exactly. While resurfacing offers the
most noticeable results, it also requires
the most downtime—one to two weeks.
Hence the reason for the positive buzz
surrounding new ablative treatments.
While they are not quite as effective as
laser resurfacing peels, the associated
downtime is just 2-4 days before makeup
can be applied. Skin may stay red or
discolored for two weeks or more after
treatment, but downtime reduction is
significant.
While
regular
fractional
treatments are the least
effective of the three,
there is no downtime
at all, only redness and
mild swelling.
In addition to downtime,
it’s also important to consider the number of treatments you
will need to achieve the desired results. Laser resurfacing
requires just one treatment for maximum results. Ablative
treatment requires one, perhaps two, depending on the
conditions being treated. Traditional fractional treatment
requires approximately five spaced treatments to reach
maximum effectiveness (but with no downtime).
Which treatment is right for you? In truth, there is a place
for each, depending on skin condition, desired results, and
recovery schedules. Ablative treatments are exciting because
they offer greatly reduced downtime over laser resurfacing.
However, all of these technologies, if used correctly, can give
excellent improvement in skin texture, and significantly
reduce wrinkles, scars, prominent pores and other skin
irregularities.
At the Gateway Aesthetic Institute & Laser Center we
have all three options available—standard fractional lasers,
ablative fractional lasers, and traditional laser resurfacing.
(We just purchased the leading edge Fraxel Repair laser.) In
a private consultation, we can discuss your goals and how
best to achieve them with a skin treatment that is perfect
for you. ©

Our Most Popular Cosmetic Services
Acne Treatment
Acne affects millions of Americans every year, but there is no reason to worry. With advancements
in lasers, light, and phototherapy treatments, you have more options to control acne today than
ever before.
Laser Hair Removal
Are you tired of shaving, plucking, waxing, electrolysis, or other painstaking methods of hair
removal? Consider laser hair removal. In addition to reducing the presence of visible hair, laser hair
removal will also treat and prevent ingrown hairs.
Tumescent Liposuction, Smartlipo, LipoDissolve
Do you have love handles, tummy bulges, saddle bags, or other areas of localized
fat that just don’t seem to go away, no matter how much you diet or exercise? There
are options. Tumescent Liposuction is the most commonly performed cosmetic
procedure in the United States. The tumescent technique reduces post-operative
bruising, swelling, and pain, without hospitalization and without general anesthesia.
Smartlipo is an innovative, laser-assisted procedure used in combination with
tumescent liposuction to tighten skin and achieve superior results. LipoDissolve is
a non-surgical fat removal treatment for body contouring. Fat is removed through
a series of microinjections of PCDC, a naturally occurring substance that breaks up
and dissolves unwanted, localized fat.
Varicose & Spider Vein Treatment
Our varicose and spider vein treatments with laser and sclerotherapy are more effective
than traditional saline injections or surgery. Even one or two treatments can help to
relieve the aching, tired, cramping sensations often associated with varicose and spider
veins—and your legs will look terrific.

Cosmetic Laser Eyelid Surgery (Blepharoplasty)
In the past, baggy, saggy eyelids were treated with traditional scalpel
surgery. Now there is a better choice. Laser eyelid surgery (also called
Blepharoplasty or Bleph, for short) uses a pulsed laser to remove excess skin,
fat, and wrinkles. No scalpel or scissor cutting is involved. The procedure is
performed with mild oral sedation and local anesthesia and in addition to
removing excess skin and fat, the laser reduces wrinkles.
Botox® Cosmetic
Botox Cosmetic is a simple and virtually painless procedure performed with very
small injections into facial muscles that cause them to relax and wrinkles to fade. The
injections are very precise and affect only a few muscles of the face. The result is a
relaxed, less-wrinkled appearance to the skin. Safe. Simple. Beautiful.
Soft Tissue Fillers & Lip Augmentation
Smooth skin and full lips are associated with youth and beauty. Soft tissue
fillers give your lips fullness and vanquish depressed facial wrinkles. We offer
several safe FDA-approved products. At the Gateway Aesthetic Institute, we
have performed thousands of these procedures with fabulous results and no
serious side effects.
Light and Laser Facials
These treatments represent one of the most dramatic modern
advances in the ongoing war against aging skin. High-intensity
light and laser energy can now be used to easily and effectively treat
age spots, wrinkles, freckles, spider veins, facial redness, moles,
birthmarks, pores, uneven skin tone, texture, and more. Light
and laser facial treatments rejuvenate the skin to give you a more
youthful appearance. Best of all, they are affordable, effective, and
easy.
Laser Scar Treatment
Unsightly scars have presented a problem for men and women for
centuries. In the past, treatment options have been limited. With today’s
laser technology, scars can be smoothed, filled, and softened to make
them less noticeable.
Laser Tattoo Removal
Laser tattoo treatments use the latest technological advancements in lasers
to fade and remove unwanted tattoos with little risk of scarring or of
damaging normal skin.

Healthy Food, Healthy Skin
I
f the saying
is true that we
are what we
eat, that may
explain why
so many of us
have the skin
of a Twinkie.
Nutritionists
today
are
confirming what
common
sense
has dictated for
decades—if
you
want healthy skin (or
any other part of your
body), you need to
eat healthy foods.
While a balanced
diet—including
plenty of vitamin-rich
fruits and vegetables—
is key, there are certain
foods that contain essential
vitamins and minerals that
are particularly helpful to
your skin.
Skin foods to consider:
• Carrots, sweet
potatoes, broccoli,
spinach & cod-liver
oil are all high in
vitamin A, which
not
only
helps to
8
g a t e w a y

strengthen the skin, it prevents acne.
As a powerful antioxidant, vitamin A
also helps rid your body of toxins. And be
cautious on your intake of alcohol, coffee, and
excessive iron, all of which can deplete your body’s
supply of this essential vitamin.
•
Hazelnuts, sunflower seeds, and raw almonds are rich
in vitamin E. Numerous research studies have shown
that vitamin E, applied topically to the skin, can
prevent UV damage. A diet of vitamin E-rich foods
exerts the same protective effect.
•
Flaxseeds are an excellent source of omega-3 fatty acids.
A good breakfast granola, sprinkled with flaxseed, is a
great way to begin each day.
• Citrus fruits are laden with high levels of vitamin C,
a nutrient believed to add both luster and softness to
your complexion and to also fight wrinkles. In one of
the first studies to examine skin aging and the impact of
getting nutrients from foods rather than supplements,
researchers reported that people who ate foods high in
vitamin C had fewer wrinkles than those whose diets
contained minute amounts of the vitamin.
• Excellent
sources of
B vitamins
include whole grains, such as wheat and oats, dairy
products, like milk and yogurt, leafy green vegetables,
beans, and peas. The B family includes B1, B2, B6,
B12, niacin, folic acid, biotin, and pantothenic acid.
Vitamin B1 aids skin health by improving circulation.
Proper amounts of vitamin B2 or riboflavin can reduce
brown pigmentation and aid oily skin. Vitamin B6 or
pyridoxine will improve dermatitis or eczema. The B
group of vitamins is also involved in making red blood
cells, which carry oxygen throughout your body—
critical in ensuring your skin retains its pink, healthy
tone.
While diet and healthy skin go hand-in-hand, balance is
the key. Over-emphasizing one food in preference to others
may actually deprive your skin of essential nutrients and do
more harm than good. Eat healthy and let common sense—
and beautiful skin—prevail. ©
• In addition to fruits and vegetables, don’t overlook the
importance of protein, which is necessary for cell repair
and to help keep aging in check. One of the best choices
is fish, which contains essential oils that will help
nourish your skin. Studies seem to indicate that
wild-caught, cold-water fish are best, including
salmon, mackerel, albacore tuna, and sardines.
Other protein-packed animal products include
egg whites, skinless chicken, and turkey breast.
And remember, don’t shy away from all fat.
The right kinds are not only healthy,
they are essential.

liposuction and Smartlipo offer
consistent, quick, and satisfying results
with little pain or downtime.
When we perform liposuction
in our office, we use tumescent
anesthesia (local numbing of the
fat) so there is no need for IV
sedation or general anesthesia.
Because of this, the risk of
serious side effects is decreased
significantly. A recent review of
the medical literature published
in the American Journal of
Cosmetic Surgery, evaluated over
10
375,000 cases of tumescent liposuction
performed according to the standards of
the American Society of Dermatologic
Surgery (ASDS) were evaluated. In these
cases, there were no deaths, no serious side
effects, and no hospitalizations required.
In contrast, when this procedure is done
under general anesthesia, the rate of
death and serious complications can be as
high as 5-20%. The tumescent technique
also produces less bleeding, less pain, and
more even removal of fat with smaller,
less aggressive surgical instruments.
We pride ourselves on providing a safe
procedure with amazing results when
we perform liposuction at the Gateway
Aesthetic Institute.

calves,
and ankles. In fact, according to the
American Society of Plastic Surgeons
(ASPS), it is estimated that at least
half of the adult female population is
plagued with this common cosmetic
problem. A number of factors may
contribute to the development of
the spider veins, including heredity,
pregnancy and other events that
cause hormonal shifts, weight gain,
occupations or activities that require
prolonged sitting or standing, and the
use of certain medications.
Beautiful Facial Volume
with Sculptra
Gateway Aesthetic Institute offers
injection treatment (sclerotherapy)
for the veins, with a safe sclerosing
detergent that causes the veins to
collapse and fade from view. For
those with needle-phobia, we also
offer laser treatment with the Candela
Gentle Yag laser. Both procedures
work great and may also remedy
other bothersome symptoms that
can be associated with spider veins,
such as aching, burning, itching, and
swelling. After each sclerotherapy
session, the vessels will appear lighter
as the body breaks down the collapsed
vessels. Compression wraps or support
stockings are usually recommended to
be worn for a couple of weeks after the
procedure to promote healing. Soon,
your legs will look younger, clearer,
and healthier.
The procedure is done in our clinic,
takes about 45 minutes, and there
is no downtime. Your legs will need
some time to heal before they look
better, so call for an appointment
right away. Then unpack all those

(injections
of poly-L-lactic acid), a procedure that
has been used in Europe for years and is
now becoming popular in the U.S.
Sculptra provides reliable, largevolume facial filling that can last two to
three years with minimum maintenance.
Keep in mind that the procedure does
take some artistry, since the contours of
each unique face vary, especially in the
cheek areas. Depending on the patient,
some areas of the face will need more
and others less. When performed by
an experienced provider, the results are
truly impressive.
While patients will notice an
immediate difference with newly
-defined cheekbones, Scuptra’s full
effects generally take several months
to develop. A few days after the
treatment, the water in the Sculptra
mixture is absorbed by the body and
initial swelling disappears. Then, over
the next few days and weeks, the body
begins to create collagen in response to
the tiny particles of Sculptra which act
as an irritant.
And then it’s time for another
treatment! Typically, even with mild
lipoatrophy, a patient needs at least two
treatment cycles. While it may feel like
a drawn-out process of gradual filling,
the procedure is very safe, reliable, and
well worth the time invested.
The most impressive thing about
this product is that after performing
hundreds of Sculptra procedures over
the past five years, I have not had an
unhappy Sculptra patient yet!

Tried and True Skin Innovations
by Elisa Wilcox, PA-C
O
ne of the perks of my job at the Gateway Laser
Center is being privy to the modern advances in
skin care. These advances usually take the form of light
and/or laser treatments, injections, or most often, topical
creams and serums. Skin care is always evolving, with
new products and treatments being introduced almost as
frequently as news on Britney Spears. And oftentimes, the
advances in skin care divide the top experts–one person’s
miracle in a jar is another’s flash in the pan. However, I
recently stumbled upon a very interesting magazine article
(Allure, December 2007), featuring eight skin innovations
that have received unanimous praise from the most
prominent dermatologists and cosmetic chemists in the
country. These products live up to their promises and have
truly changed the face of skin care. It’s hard to imagine life
without them!
1. Retinoids: “Retinoids are the one topical treatment
actually proven to regenerate collagen” says Ranella Hirsch,
president-elect of the American Society of Dermatologic
Surgeons. There is also evidence that they smooth the
surface of the skin, even skin tone, increase blood flow,
and prevent clogged pores.
2. Botox: FDA-approved in 2002 to treat frown lines,
Botox is now most often used to make movement softer
and to diminish persistent wrinkles. Botox can also be
used around the eyes to smooth out crow’s feet, to elevate
the corners of the mouth, to lift the tip of the nose, and to
minimize excessive sweating in the armpits, the palms of
hands, and the soles of the feet.
3. Accutane: This drug is a vitamin A derivative
that is taken orally to treat severe cystic acne. Most
dermatologists agree that this drug has miraculous
potential, but it is not to be taken lightly and
requires close monitoring.
4. Alpha Hydroxy Acids: Alpha Hydroxy
Acids (AHAs) are a family of acids that
include mandelic acid, malic acid, glycolic
acid, and citric acid. They smooth out skin
roughness, increase skin thickness, and
diminish fine lines.
14
g a t e w a y
aesthetic Institute & Laser Center
5. Hyaluronic Acid: As an injectable filler, hyaluronic
acid is considered one of the safest available and the
effects usually last from 6 to 9 months. Restylane and
Juvederm are the most popular brand names. They may
even reverse aging: According to a University of Michigan
study published in the Archives of Dermatology, Restylane
was shown to create new collagen and may even inhibit
the breakdown of existing collagen.
6. Hydroquinone: Most often called a “bleaching
cream”, hydroquinone is a tyrosinase inhibitor which
prevents new skin cells from darkening. Potentially it’s
a good solution for those suffering from melasma or
hyperpigmentation.
7. Antioxidants: Antioxidants combat the free radicals
that are generated by the sun, cigarette smoke, and
environmental pollution. Antioxidants protect the skin
against DNA damage, discoloration, and wrinkles.
Common topical treatments with this protection are
vitamin C serums, CoffeeBerryTM cream, green tea serums,
and idebenone products.
8. Sunscreen: This is the biggest innovation of them
all. Without protection from UV rays, all other skin care
efforts may well be a waste of time. According to Doris J.
Day, clinical assistant professor of dermatology at NYU,
sun exposure is responsible for 95 percent of wrinkles
and is the single most preventable cause of aging. Broadspectrum chemical or physical
sunscreens are the most
advantageous, as
they deflect
both UVA
and UVB

Combined Treatments. Amazing Results. |
LipoDissolve & Gentle Yag Laser
A
t a recent consultation with Inna Prokopenko, her
Gateway Aesthetic provider, patient Linn Thorsen Jensen
expressed concerns about her sagging neck. “We looked at
several treatment options,” said Inna, “including liposuction
and a suture lift—neither of which seemed appealing.” During
their visit, Inna recommended a combination treatment—
LipoDissolve and a Gentle Yag laser—to address Linn’s jowls,
undefined jaw line, and skin texture and tone. After just three
treatments, each a month apart, both provider and patient

were thrilled with the results. “Her jowls were gone, she had a
defined jaw line, and her neck was beautiful.”
To achieve such remarkable results without surgery is
impressive and after the treatment was featured on KUTV
Channel 2 News, the demand skyrocketed.
“If you’ve reached the age where your neck is beginning to
drop and you’re just not ready,” says Inna, “call for a consultation

Great Skin at Any Age |
Lifelong Skin Care Beauty Secrets
by Mark Taylor, M.D.
W
hen we see someone our
own age with beautiful
skin, it’s natural to compare our
problem skin to theirs and then
blame Mother Nature. No so
fast. Beautiful skin is no accident.
How many babies do you know,
for example, with bad skin? The
question that should be asked is,
what can we do to keep our skin
“baby” beautiful?
Start Protection Early
The most important key
to lifelong beautiful
skin is sun protection.
Without it, the
damage
of
ultraviolet light
on the skin
begins early.
(For example,
freckles
in
children are
an early
sign of
s u n
damage.) Then,
as we age, the
damage continues
to accumulate.
Over a lifetime,
the result is
premature aging,
brown
spots,
thinning skin,
broken
blood
vessels, damaged
collagen
and
elastin,
and
wrinkled,
sagging, rough,

and coarse skin—and in its worst form, skin cancers,
including deadly melanomas. Don’t think if you opt for a
tanning booth that you outsmart nature. While the sun’s
rays have a broader spectrum, the artificial rays produced
in tanning booths can be equally damaging.
When choosing a sunscreen or sunblock, protection
from both UVA and UVB light is necessary to avoid sun
damage. We recommend products from both SkinCeuticals
and La Roche-Posay, since they cover a broad spectrum
of both the UVA and UVB ranges (something drugstore
lotions don’t). And remember, when you are outside, it
is recommended that sunscreen be applied liberally every
two hours to give maximum protection. Wide-brimmed
hats, sun-protective clothing, and shade are also important
weapons in your skin’s battle against the sun.
Control Acne
One of the most devastating skin problems people
experience is facial scarring caused by acne. Some children
will start to get acne lesions as early as 10 years of age.
The condition typically worsens after puberty and may
affect some people for years into adulthood. To keep skin
beautiful, treat acne early to prevent or reduce it and the
permanent scarring that may otherwise result.
Simple acne may respond to daily cleaning or over-thecounter products like benzoyl peroxide or salicylic acid.
More stubborn cases are best treated by a dermatologist
who can prescribe stronger medications. For many patients,
aggressive control of acne during their teenage years will be
sufficient to prevent scarring. However, others who have a
strong family history of acne may need to undergo potent
acne treatments for many years.
Remove Dots and Spots
As we age, many of us will develop age spots, freckles,
melasma (uneven patches of pigmentation), scars, cysts,
and papules. In addition, pores often become more
prominent as oil glands age.
If any of this sounds familiar, no worries. The good news
is that these unsightly skin conditions can be efficiently
treated with the right treatments or products. Using a
product with hydroquinone can help control melanin
production. We really like the Obagi Skincare Line which
has a variety of products with hydroquinone. However,
hydroquinone should not be used long term so please see
your skin care professional before you begin. Additionally,
the use of vitamin A and/or alpha-hydroxy acid can be
beneficial. Not only will these products help correct and
treat, but they will also help maintain and prevent.
Moisturize
Maintaining the integrity and protective quality of the
skin can be achieved through frequent use of moisturizers
and barrier repair products. In a study comparing Retin
A to good moisturization, vigorous use of moisturizer
and sunscreen was almost as effective as the prescription
Retin A treatment. You should look for a moisturizer
with hyaluronic acid to help replenish water loss which
occurs with age, stress, dry climates, high altitude, etc.
A good suggestion is layering a hyaluronic acid gel, like
SkinCeuticals B5 gel, underneath your moisturizer to help
lock in moisture. Products with ingredients like aloe vera,
shea butter, and glycerin (to name a few) can help. We
really like Immuderm, which is a glycerin and shea butterbased moisturizer that not only hydrates, but helps to heal
and soothe the skin as well. It also serves as a great postprocedure moisturizer.
In Conclusion
Starting early with good skin care is important; however,
getting on a good regimen and sticking to it is what
matters most. It can be a bit overwhelming, as there are so
many products on the market today. However, the great
news is that skin care has advanced dramatically over the
last few years. Many of today’s current products are truly
wonderful and easy to use. See a skin care professional to
help point you in the right direction.

And remember, beautiful skin is not an accident—it is a
commitment. ©

See the Light (“Omni” light, that is) |
No Reason to Hide
A
cne affects over 85% of adolescents and up to 50% of adults. Experts agree that acne is
not caused by greasy foods, but rather by the skin’s sebaceous glands producing excess
oil that combines with normal skin bacteria to clog pores.
The methods for fighting acne are many; however, a treatment that works for one
person may not always work for another. It all depends on one’s body chemistry. That said,
one treatment that is garnering the praise of many doctors and patients is OmniluxTM light
therapy.
What is Omnilux light therapy and how does it work?
Omnilux light therapy consists of alternating sessions of exposure to red and blue wavelength
light. For each session, makeup or sunblock is removed and then the Onmilux light will
be positioned close to the face for 20 minutes of exposure. It’s simple, safe, and easy with
a complete treatment course usually consisting of 8-12 visits. Omnilux treatment may also
be combined with prescription acne medications for greater benefit.
How does red and blue light treat acne?
The bacteria in the skin that causes acne also produces natural
chemicals called poryphyrins which are sensitive to light
at certain wavelengths. The Omnilux blue light stimulates
these chemicals to eradicate the bacteria that cause acne
inflammation. Then in the following session, the red
wavelength light helps reduce inflammation and
stimulate cell repair.
Is it painful?
No. Omnilux light treatments are totally noninvasive. There is no damage to the skin, no pain,
and no internal medication or drugs that need to be
taken. It’s arguably the safest acne treatment on the
market.
How well does it work?
It doesn’t work equally well for everyone, but a
recent independent study showed that Omnilux light
treatments, performed 2 to 3 times per week for a
month (8 to 12 treatments), improved acne by as
much as 81% within 3 months of the final treatment.
What are my Omnilux treatment options?
Until now, Omnilux light therapy had to be done
in a physician’s office. However, Photo Therapeutics,
the company that produces the Omnilux light, has
recently introduced a hand-held model that offers light
treatment from the convenience of your home. While
it may not cover as large an area as the office equivalent
used by your doctor, the handheld version is definitely
more convenient for spot treatment or flare-ups.
If acne has been an issue (or if you’d simply like to
stimulate healthy skin cell growth), consider the Omnilux
home unit, currently available at Anne-Michele Apothica,
located at the Gateway Mall. ©

A Note from Your Doctor
I
t is always enjoyable to write this column and offer my perspective on new skin treatments.
Certainly aesthetic dermatology is an area where we see “waves of hysteria.” What do I mean by
hysteria? As new procedures become available, it’s common for facilities to promote them at frantic
levels before the complete value of the procedures is understood. In my professional experience,
which spans over 30 years, I have seen many touted treatments survive, and many that have not.
Let’s look at a quick example. In the mid-1990s, laser skin resurfacing swept the country like wildfire.
After a few years, however, it fell into disrepute because too many untrained physicians entered the
market offering poor treatment that left the skin waxy, hypopigmented, and scarred. The procedure
wasn’t to blame, but rather the inexperience of those performing it.
Sometimes, even with an experienced provider, a much-hyped procedure simply doesn’t live up to
its potential. For example, laser treatment of spider veins. Though the procedure received significant
attention when it was first introduced, it didn’t outperform standard sclerotherapy (injections).
While there is a place for the new technology (veins too small to inject), the “old” treatment still
offers the best results for most patients.
What I hope everyone understands is that the key to a successful treatment is not found in chasing
after the latest procedure, but rather in relying on a provider with both knowledge and experience
to know when to chase and when to wait. In the case of laser resurfacing, for example, we have
performed over 1,000 procedures. It remains the gold standard for our patients who want to rid their
faces of the maximum number of wrinkles, scars, and sun damage.
Today, body shaping devices are the latest craze to hit the industry. These are machines that claim
to manipulate fat beneath the skin’s surface to give you the body shape you have always wanted.
Soon, every mom & pop medi-spa will be touting the machine’s benifits. And while some shaping
may be possible, I haven’t seen any of these devices that, when used by themselves, come close to the
excellent results we are seeing with LipoDissolve, SmartLipo, and tumescent liposuction. Instead, I
believe these machines are best when used as a follow-up treatment to LipoDissove, SmartLipo, and
other lipo treatments. If you believe the hype and expect life-changing results from the machines
alone, you will be dissapointed.
What is the moral of the story? Rely on experience, not hype. With two board-certified dermatologists
and a qualified staff of experts, the Gateway Aesthetic Institute & Laser Center will help you put
every procedure (new or old) into perspective.
Sincerely,
Dr. Mark Taylor
