The Plan outlines the major goals and activities that will be implemented at the local community level and builds on the direction and guidance provided by the NSW Ministry of Health, Population Health Framework and SESLHD's overarching Strategy 2012-2017 and the Health Care Services Plan 2012-2017. Developed by SESLHD Health Promotion Service staff, the selected strategies were based on best available evidences, knowledge, funding and human resources.
