Documents you must send us
We will only consider your claim for unclaimed company funds
if you send us the following documents.
These documents enable you to substantiate your claim and they
will help prevent liability to other parties.
Original share
certificates if
available
If you do not have the original share certificate send us either:
•
a certified copy of an official notice to you at the
former address as shown on the share register.
Your witness must certify the copy by writing on it I
certify this is a true and correct copy and sign it.
•
clear evidence of your shareholdings held with the
company.
or
Personal
identification of
company office
holder
Send us a copy of your drivers licence, passport or similar
document. Your signature must be clearly visible on the copy.
Your witness must certify the copy by writing on it I certify this is
a true and correct copy and they must sign it. See the sample
we’ve included.
Statutory
declaration
Fill in the statutory declaration using a blue or black pen. See the
sample we’ve included.
We may not accept your statutory declaration if it is not
complete. This means you must:
•
include all relevant information and
•
cross out irrelevant information and initial these changes.

Who can certify (sign) copies of your documents?
When you make copies of documents to send us they must be
signed by the person who witnesses your signature on the statutory
declaration. There’s a list of people who can witness your statutory
declaration on page 6. They should write these words on the copies:
I certify this is a true and correct copy
and they add their signature
If you live outside Australia
If you live outside Australia you must send us:
•
a statutory declaration under the oaths authority of the
country you are living in and
•
the other documents which are listed on the previous page.
Claims from money finding agencies
All claims from money finding agencies must also include a signed
original letter authorising the agent to act on behalf of the owner or
joint owners of the shares. We will send payments to the agent's
address.
