AWARD NOMINATION FORM Deadline: 6DWXUGD\1RYHPEHU To nominate someone: Write a letter of nomination and ask a colleague to write a letter of support. The letters must be written by another member of the affiliate and/or from the institution where the nominee is employed. Once the deadline submission date has passed, the SCHA Executive Board will select an award recipient and send their recommendation to ACHA. The Charles F. Bohmann Award: Named in recognition of a true administrator, this award represents the highest honor of SCHA. Nominees should be outstanding leaders who have established impressive standards for excellence and service both in the field of college health as well as in SCHA. This award will be presented during the SCHA Annual Meeting. ACHA Affiliate New Professional Award: This award, established in 2002, recognizes individuals who have been in the field of college health less than five years and demonstrate the potential for significance and sustained leadership important to the mission of ACHA. Nominations for this award are through SCHA. Recipients will be notified after the meeting of the ACHA Board of Directors at the end of February. The recipient will be announced in the April issue of ACHA Action and presented at the ACHA Annual Meeting. This is for the: Charles F.
