This care will be provided within the scope of practice granted by the SWSLHD Medical and Dental Appointments Advisory Committee. To actively participate in all of the activities required to provide efficient and effective medical care for the Aged Care Purpose Of Position : patients within the Liverpool/ Fairfield LGAs with a focus on acute and continuity of care at Liverpool Health service. Duties include clinical service delivery, administrative duties, quality and clinical governance activities and the provision of education. There is an opportunity to pursue research interests. 1. Clinical l Have clinical privileges in geriatric medicine including admitting rights and undertake geriatric medicine inpatient care at Liverpool Hospital l Provide clinical management and treatment of patients in a SWSLHD facility under your care, within the scope of practice granted by the SWSLHD Medical and Dental Appointments Advisory Committee. l Ensure detailed patient treatment plans are in place to support the timely management of patients. l Ensure a high standard of clinical record documentation is maintained. This includes the completion of the patient’s clinical records (including medication charts / orders) to reflect clinical decisions and the keeping of appropriate databases up-to-date. l Provide a consultant service as required for patients of other senior medical staff. l Participate in the on-call roster for your department as required. l Provide care to non-inpatients attending department ambulatory clinics as required. l Be an active participant in the clinical activities in your Department. l Adapt clinical practice in accordance with contemporary evidence based best practice. l Preparedness to participate in the provision of health services at an Area level, if credentialed. l Participate in the on-call roster for your department as required. l Participate in rosters for out of hours Emergency Department admissions and Departmental on call and relief as appropriate l Provide geriatric medicine care to non-inpatients attending department ambulatory clinics as required. l Participate in clinical rotations within the department of aged care including orthogeriatric care. l Provide an inpatient geriatric medicine consultant service as required for patients of other senior medical staff at Liverpool hospital. l Undertake community aged care consultations in the Fairfield and Liverpool sectors l Support the Liverpool/Fairfield ACAT in domiciliary assessments l Work closely with multidisciplinary teams in aged Care to improve medical care for Aged Care patients l Liaise and consult with GPs and other health professional l Participate in hospital community interface development, and planning, policy and procedure development, as required l Be an active participant in the clinical activities in your Department. l Adapt clinical practice in accordance with contemporary evidence based best practice. 2. Clinical Governance and Quality Improvement l Comply with SWSLHD and facility clinical governance policies and programs. Key Accountabilities : l Participate in the facility quality improvement program. l Ensure effective clinical handover processes in accordance with facility and SWSLHD guidelines. l Participate in the facility and / or SWSLHD clinical governance requirements for ongoing maintenance of professional competence and clinical performance l Participation in peer review and other personal development activities consistent with the relevant specialist medical college requirements to maintain professional standards. 3. Education and Training l Participation in teaching and training at all levels of undergraduate and postgraduate medical education where required by your Head of Department or Clinical Director in accordance with the requirements of the NSW Clinical Education and Training Institute (HETI), SWSLHD and NSW Health Department policies; and, if applicable, the relevant specialist medical College vocational training program. l Contribute to the education and training of other clinical staff as required by your Department Head. l Participate in Departmental training programs and any other training courses specified by facility and/or SWSLHD management that will enhance personal development, skills and knowledge, and practice requirements. l Participate in the education of health service staff, patients and carers, and the broader community in multifactorial syndromes of ageing, medication use and health care for the aged 4. Supervision l Ensure supervision of junior medical staff (intern to postgraduate Fellow) in accordance with facility, SWSLHD, HETI, NSW Health Department and relevant specialist medical College polices and guidelines. 5. Performance Management l Participate in the SWSLHD Performance Management and Development Program. l Demonstrate clinical competency within the clinical privileges granted by theSWSLHD Medical and Dental Appointments Advisory Committee. 6. Other Duties l Attend, participate and support administrative meetings as required by the Department Head or Director of Medical Services. l Abide by the SWSLHD Code of Conduct. l Ensure familiarity with, and adherence to, all NSW Health Department, SWSLHD and Facility / Service policies and procedures that are relevant to the performance of the duties specified in this Position Description. 7. Research and Development l Provide leadership in Health Services Research l Provide leadership in Area and Sector Aged Care Services development l Pursue health services research grants l Providing clinical care. l Delivery of medical services within your Department to ensure the Department provides agreed service delivery and meets relevant performance benchmarks and expenditure and revenue targets. Selection Criteria Current medical practitioner, registered, or eligible for registration, with the Australian Health Practitioner Regulation Agency (AHPRA) and Fellow of the Royal Australasian College of Physicians and/or other specialist recognition as provided for under the staff specialist state award.
