The Millstone Township School District is eager to honor outstanding teachers. We invite you to nominate a teacher
and/or educational service professional who distinguished him/herself last year during the 2013-2014 school year by
completing the attached nomination form and returning it by Friday, December 5, 2014, in a sealed envelope marked
“Teacher Recognition Award,” to the Office of the Superintendent located at 5 Dawson Court, Millstone Twp., NJ 08535
or to the Principal's attention at one of the buildings. Your nomination may remain anonymous or you may include your
name in the space provided on the nomination form. If you wish to send electronically, please e-mail Scott Feder at
sfeder@millstone.k12.nj.us.
Educational service professionals may include, but are not limited to, school library media specialist, Child Study Team
members, reading specialists, school counselors, school nurses, occupational therapist, school psychologist, social worker,
and speech language specialist.
Nominations will be reviewed by a panel of teachers, administrators and parents. The panel will recommend to the Board
of Education one teacher from each school and one educational service professional from the district. The selected
teachers will receive a $500.00 grant for the purpose of purchasing classroom materials and will attend a luncheon in
recognition of this award.
The deadline for submitting your nomination to the Superintendent or Principal is Friday, December 5, 2014.
Teachers that have been honored over the past five years are not eligible to win this year. Congratulations to our previous

making a difference? If so, please take the time to recommend this person by completing this nomination form
and returning it to the building principal by the specified deadline. Administrators, staff members, parents and
community members are encouraged to participate.

Each candidate will be scored based on the criteria below:
 Have a superior ability to inspire students of all backgrounds and abilities to learn;
 Be an exceptionally skilled and dedicated educator in a public or charter school;
 Anticipate actively continuing in his/her teaching status for at least five more years;
 Have the respect and admiration of students, parents, and colleagues; and
 Demonstrate a strong ability to foster excellence in education as evidenced by ongoing contributions to
the improvement of student learning and the learning environment.
