permission for information collected on this form, possible audio/video recordings and any photos taken during the Festival to be used for the following specific
purposes without any monetary gain: Toronto Festival program, Toronto Festival website, National Music Festival, corporate sponsors, newsletters, press

BEFORE FILLING IN YOUR ENTRY FORM, PLEASE READ THE FOLLOWING:
1. Entries must be on official Entry Forms or photocopies of the official Entry Form. Please feel free
to make as many copies as you require. For duets, duos, trios, quartets, quintets, ensemble or
group entries, please use the ENSEMBLE & GROUP ENTRY FORM - MUSIC.
2. Entries must be completed IN FULL and signed. Entries that are not complete will be declined by
the Association. All information (including valid and clearly legible e-mail) is required to facilitate
scheduling and to determine eligibility for the Ontario Music Festivals Assocation Provincial
Finals.
3. Please list any possible scheduling conflicts. List all Kiwanis Music Festival classes entered
by class number which are not included on this entry form. We will make every effort to avoid
scheduling conflicts where possible. Please note: competitions may take place AT ANY TIME
during the Festival except on Sundays - this includes the possibility of being scheduled during
school hours. Music lessons, after school and weekend activities do not qualify as a scheduling
conflict.
4. It is the responsibility of the competitor to read ALL rules (http://www.kiwanismusictoronto.org/
music_rules.html) and enter the correct class by CLASS NUMBER. Class changes will
not be permitted. Please ensure that you use THE 2015 syllabus as some
class numbers have changed.
5. The entry fee MUST accompany each entry. The entry fee is indicated under each class number
as listed in the syllabus. (http://kiwanismusictoronto.org/music_syllabus.html)
6. Entry fees cannot be refunded, except under circumstances where the entry is refused by the
Association.
7. Please consider making a donation along with your entry. The Kiwanis Music Festival receives
no funding from government agencies and relies on the support of its donors to ensure its
existence. All donors will be listed in the Festival programme and tax receipts will be issued for
amounts of $10.00 or more.
8. ENTRIES CLOSE – November 19, 2014. Entries postmarked later than the above date will be
accepted for one week only but with a penalty equal to the fee(s) for all class(es) entered. In all
other cases, entries received after that date or postmarked after that date will be returned.

Faxed entries will be accepted ONLY if paying by VISA and Mastercard (American Express not
permitted) and as long as the forms are clearly written. Our fax number is 416-487-5784.
