You can get reward cards worth up to $75 for taking steps to improve your health. Challenge yourself and earn up to $75: g $50 for completing a Total Health Assessment This online questionnaire gives you a picture of your overall health, plus an action plan that’s based on your answers. g $25 for completing an online healthy lifestyle program Get help to reduce stress, quit smoking, lose weight, and more. You can take a program when it’s convenient for you, at your own pace. Starting January 1, 2015, visit kp.org/feds to get started. There you’ll find information and links to the Total Health Assessment and healthy lifestyle programs. The rewards program runs from January 1, 2015, through December 31, 2015, and is open to all Kaiser Permanente FEHB subscribers and their enrolled spouses who are 18 and older. You can take the Total Health Assessment as often as you’d like and as many healthy lifestyle programs as you’d like, but you can only earn rewards for them once during the reward period. You must complete the activities during the calendar year. Rewards will be issued 4 to 6 weeks after you complete your activity. 6 Choose the plan that’s right for you Select from the High Option, the Standard Option, or the Basic Option. With each, you’ll get a wide range of services and resources to help you stay healthy. g High Option g g g g Standard Option g g g Basic Option g You pay higher premiums but lower copays for most health care services and prescription drugs. You don’t pay a deductible. You’ll get a 25 percent discount on eyeglasses and contact lenses at Vision Essentials by Kaiser Permanente optical centers.4 You pay lower premiums compared to the High Option. You’ll pay higher copays for office visits and urgent care visits, compared to the High and Basic Options. You pay a lower deductible and then copays for certain services, like inpatient hospital services, labs, and X-rays. You pay the lowest premiums. Compared to the Standard Option, you’ll pay lower copays for office visits and urgent care visits. You pay a higher deductible and then coinsurance for certain services, like inpatient hospital services, labs, and X-rays. Your supplemental dental options You can also choose from 2 dental plans to supplement your health coverage.4 Kaiser Permanente Insurance Company (KPIC) Dental Insurance Plan DeltaCare USA This plan gives you the freedom to see any licensed dentist you choose, with no deductible for preventive and diagnostic services. When you visit a participating Delta dentist in California, all claims paperwork will be handled for you. With this dental HMO plan, you’ll select a dentist from the DeltaCare USA network. There are no claim forms to fill out and no deductibles. Most preventive and diagnostic services are available at little or no cost to you. As a Kaiser Permanente Federal Employees Health Benefits (FEHB) member, your benefit goes even further than your home service area. Save time and money by ordering prescription refills online, on your smartphone, or by calling the phone number on your prescription label. You can visit any American Specialty Health Plans of California, Inc. (ASH) participating provider with no referral. Chiropractic services are covered if they’re needed to diagnose or treat neuromusculoskeletal disorders, as prescribed and provided by an ASH Plans participating provider. You’ll get: Coverage for emergency care worldwide at emergency departments and urgent care centers. Care from any Kaiser Permanente facility in another region through the Visiting Member Program. Coverage for certain outpatient follow-up care and outpatient continuing care outside of Kaiser Permanente service areas through the FEHB travel benefit. g g Prescription refills can be mailed to your home at no extra charge. More services, like prescription drugs, now apply to your out-of-pocket maximum. 9 Kaiser Permanente Senior Advantage (HMO) for Federal Members If you’re eligible for Medicare, you have an option to choose Kaiser Permanente Senior Advantage (HMO) for Federal Members. If you decide to enroll in Senior Advantage (HMO) for Federal Members, you’ll get richer benefits than with your FEHB coverage alone. You’ll get all of your FEHB benefits, plus: Lower copays for office visits and for most other services, such as inpatient hospital admissions and most prescription drugs. Up to a 100-day supply of prescription drugs at Plan pharmacies for one copay (High Option only). Extra credit toward the purchase of eyeglasses and contact lenses.
