Attendance Policy Contract Form
Attendance Policy Contract Spring C 2015 Last Name/Family Name First Name/Given Name Class attendance is a requirement of the U.S. Citizenship and Immigration Services (USCIS). It is also required by the ELI because we feel that you cannot learn English if you do not come to class regularly. To help you stay focused on your studies and to maintain your VISA status, the ELI has the following attendance policy: All full-time students must attend 286 hours or more of their classes (85%). See receptionist for part-time limits. Percentages are the same. New and Non-Probationary Students: I cannot miss more than 43 hours (15%) of classes, or fail any of my classes, or I will not receive a semester certificate. If I miss more than 43 hours (15%) of class and up to 72 hours (25%) of class, I will be allowed to return to the English Language Institute the next semester ONLY on a probationary basis. If I miss more than 72 hours (25%) of absences, I will not be allowed to return to the ELI the next semester. If I miss more than 86 hours (30%) of absences, I can no longer attend ELI classes and my I-20 will be terminated. Probationary Students: If I miss more than 43 hours (15%) of classes or fail any of my classes, I will not get a semester certificate and I will not be allowed to return. If I miss more than 72 hours (25%) of class, I can no longer attend ELI classes and my I-20 will be terminated. If I miss five days in a row without informing the English Language Institute prior to or immediately after the five days, my I-20 can be terminated. I have read the above policy and understand that I must be a student in good standing. Signature Date
