Admission to the Programme of study, for which I have applied.
I further declare that entries made by me in this Form and the documents submitted in
support of the information furnished by me in the Application Form are true in all respects and
in case any entry or information or document are found to be false, this shall entail automatic
cancellation of my admission besides rendering me liable to such action as the University may
deem proper. Further, that my admission to the University and my continuance on its rolls are
subject to the provisions of the Statutes of the University. In the event of my selection/joining
the University, I will pay the required tuition fee and other charges and shall not ask for any
financial assistance or waiver in fee during the tenure of the Programme of Study.

Duly attested copies of Senior School/Secondary Certificate/ Bachelor’s Degree, or any other Degree.
Candidates who have passed the qualifying examination from a foreign University should submit all
the relevant information/documents which may justify the equivalence of their qualifications;
Marks cards/Degrees in a language other than English should be got translated into English
language and a copy of the same duly attested be attached;
A character certificate from the Head of the Institution last attended;
Details of bank account;
Bank Receipt/Details of net-banking payment of Professing Fee;
A certificate from the Bank of the applicant to the effect that sufficient funds are available in the
account of the applicant to sustain his/her studies in India for the duration of the Programme;
Copy of passport;
Medical insurance document/s. Candidates are required to submit medical insurance documents for
the duration of the programme at the time of admission/registration;
A brief write-up mentioning the reasons for applying for the Programme/s and how it is going to be
useful for the applicant.
