Central Intake phone - 604-453-1960 fax - 604-291-7350 •Referrals are accepted through Central Intake from family physicians, other health professionals as well as from individuals, family and/or significant others. •Referrals are screened daily and will typically require 2-3 days for processing. Assertive Case Management (ACM) phone - 604-453-1960 fax - 604-291-7350 •This team offers intensive case management for individuals experiencing a serious and persistent mental illness that causes significant and enduring functional disabilities. •The team aims to prevent re-hospitalization through enhancement of community support services, advocacy, education, social, recreational, housing, and other personal needs. Adult Community Support Services (ACSS) phone - 604-453-1930 fax - 604-453-1929 •Serves primarily individuals who are experiencing major mental illness requiring longer-term case management in the community. •Assists in achieving and maintaining necessities such as adequate housing, nutrition, financial resources, medical and psychosocial care. ASTAT/Group Therapy phone - 604-453-1900 fax - 604-453-1929 •Provides adult short term assessment and treatment for mental health issues. •Program consists of individual counselling, Perinatal Depression and Concurrent Disorders. •Program provides specific interventions, including Cognitive Behavioral Therapy to deal with acute mental health issues. ASTAT/Group Therapy phone - 604-453-1900 fax - 604-453-1929 •Clients are assessed by a clinician and may receive brief solution focused one-to-one and/ or psychiatric consultations. •Services include individual and group therapy. •Some of the groups are Rapid Access, Depression, Anxiety, and Family Support. Concurrent Disorders phone - 604-453-1900 fax - 604-453-1929 •A Concurrent Disorder Therapist provides consultations, education, treatment, planning, referral services, support groups and counselling to individuals who experience difficulties with both mental illnesses and addictions. •Family members of Concurrent Disorders clients are welcome as well. Community Geriatric Mental Health Services phone - 604-453-1900 fax - 604-453-1929 •Works with older adults 65+ who experience depression, anxiety, memory problems, behavioral changes and disturbed thoughts. •Works in partnership with caregivers and service providers to enhance quality of life, improved level of functioning and promote emotional well-being. Community Residential Program phone - 604-453-1900 fax - 604-453-1929 •Program offers assistance to Burnaby Mental Health clients as they learn the skills to live independently. •Provides a continuum of support ranging from 24-hour licensed care facilities to support and rehabilitation for clients living in their own apartments. •Referrals to this program must come through the Community Mental Health Teams.
