Use one form per student. Fill in all spaces below these instructions. Pre-registration is required to attend.
Check all sessions which you plan to attend. Course must be attended in its entirety for a certificate to be issued.
Please print clearly. Information on this form will be used for identification and to create certificates.

Class size is limited. If classes are full you will be advised by return email.

Fees & Expenses
Distributor personnel are required to pay a fee to a end any training session. Fees can be paid as a purchase order or by credit card.

processed upon receipt of your applica on. Confirmed registra ons cancelled within two weeks of the session are subject to a 50% of fee

Kidde and Badger welcome non-distributors and Authorities Having Jurisdiction. Those not factory authorized must be sponsored by an authorized distributor and approved in advance by Badger/Kidde. For non-distributors the course will include the sponsoring distributor’s name. Ask
your sales manager for additional information. For AHJ’s a certificate of attendance will be issued upon completion of the entire course.
