Student Portfolio Reflection Form - Ada Merritt K
ADA MERRITT K-8 CENTER
MIDDLE YEARS PROGRAMME
Portfolio Guidelines
At Ada Merritt K-8 Center, Portfolios serve a dual purpose. They make you an active participant
in the evaluation of your progress by having you reflect on your performance and on the work
you select for inclusion. They also provide you with an overview of your growth throughout the
completion of the program.
What goes in my Portfolio?
You will build your Middle Years Programme Portfolio during the three Middle School years at
Ada Merritt K-8 Center. In eighth grade, to receive the Ada Merritt International
Baccalaureate Certificate, the Portfolio must contain the following sections and documents:
1. Student Information
a. General Information
b. IB Learner Profile self-assessments and reflections
c. Approaches to Learning self-assessment
d. Grades Reflection Forms
2. Work samples: a minimum of 6 per year. You are encouraged to include work samples
that reflect a range of Global Contexts.
3. Extra-Curricular Activities Reflection Forms (a minimum of one per year)
4. Family Involvement:
a. Reflection Forms (a minimum of one per year)
b. Parent/Student portfolio review forms (a minimum of one per year)
5. Service as Action form
a. A minimum of two Service as Action forms per Year
b. Three Yearly Summary Forms (Service Log or student agenda record page)
c. Artifacts and/or photographs from Service and Action activities
What kind of work can I select?
You may select class work, home learning assignments, assessments, or projects. When work
samples are too large to fit in your Portfolio, a photo may be attached to a paper that describes
the project. Be sure the date is on all your work and don’t forget to complete and attach a
Student Reflection Form.
When do I place work in my Portfolio? Who will grade it?
Portfolios are kept in homerooms. You will select work during assigned times in your subject
classes or whenever allowed by subject teachers. All work taken from a classroom folder must
be replaced with a Student Work Removal form. Be sure to complete the form prior to removing
your work. You will place the chosen work samples in your Portfolio during Extended
Homeroom. You will self assess your portfolio by using the student checklist at the end of the
year. Your homeroom teacher will also evaluate it as a requirement for earning the Ada
Merritt K-8 Center International Baccalaureate Certificate in eighth grade. Be sure that all
required items listed above are included for each year. You will be able to take your portfolio
contents with you when you move on to high school.

We nurture our curiosity, developing skills for inquiry and research. We know how to
learn independently and with others. We learn with enthusiasm and sustain our love
of learning throughout life.
We develop and use conceptual understanding, exploring knowledge across a range
of disciplines. We engage with issues and ideas that have local and global
significance.
We use critical and creative thinking skills to analyse and take responsible action on
complex problems. We exercise initiative in making reasoned, ethical decisions.
We express ourselves confidently and creatively in more than one language and in
many ways. We collaborate effectively, listening carefully to the perspectives of
other individuals and groups.
We act with integrity and honesty, with a strong sense of fairness and justice, and
with respect for the dignity and rights of people everywhere. We take responsibility
for our actions and their consequences.
We critically appreciate our own cultures and personal histories, as well as the values
and traditions of others. We seek and evaluate a range of points of view, and we are
willing to grow from the experience.
We show empathy, compassion and respect. We have a commitment to service, and
we act to make a positive difference in the lives of others and in the world around us.
We approach uncertainty with forethought and determination; we work independently
and cooperatively to explore new ideas and innovative strategies. We are resourceful
and resilient in the face of challenges and change.
We understand the importance of balancing different aspects of our lives—
intellectual, physical and emotional—to achieve well-being for ourselves and others.
We recognize our interdependence with other people and with the world in which we
live.
