What is the Home & Settle
service?
Being admitted into hospital is not
something that we want to do, however
on occasions it is necessary. Once you
are admitted we recognise that you will
want to return home as soon as you are
medically fit for discharge.
The Home & Settle Service can assist
you with practical support when you are
discharged from hospital.
It also helps you to adjust as smoothly
as possible from being in hospital to
moving back home.
How does it work?
Although
you
may
be
quite
independent, you may find you could
benefit from some extra support and
practical assistance when you leave
hospital.
Whilst you are still on the ward our
Project Administrator, who is based at
the Newham University Hospital, will
visit you to discuss your individual
requirements. They can also arrange for
one of our dedicated Home & Settle
Support Workers to drop in to see you.
for approximately 2 weeks following
your discharge from hospital.
Who can refer to the service?
How can it help me?
Our service can help you in many ways,
if you are 60+ we can:
 Check that you have enough food
and drink available and that your
home is warm and welcoming
 Help you with domestic chores
e.g. washing, ironing, making
beds etc.
 Collect your
prescriptions
shopping
and
We manage an open referral system,
this means we will accept referrals from:
 You directly
 Your family and friends
 Nurses
 Doctors
 GP’s
 Social Worker
 Call in for a regular chat
 Discharge Team
 Call back to the hospital to collect
your medication which saves you
waiting in a hospital bed for
medication to take home
 Occupational Therapist
 Support with other nonmedical/nursing requirements
 Support you if you are going
home in a taxi
We hope that this kind of support will
help you return home and feel
supported.
Please note shopping and taxi’s etc.
will need to be paid for
 Physiotherapists
