CPP Evaluation, EOY SY2014-­‐2015 ALD4ALL END-­‐OF-­‐YEAR PROJECT EVALUATION QUESTIONNAIRE Thank you for taking the time to fill out this ALD4ALL project evaluation questionnaire. It designed to provide constructive feedback to the project team as they seek to continuously improve Professional Learning services to schools and programs. Your individual responses will be kept strictly confidential within the ALD4ALL project team and Center for Positive Practices, the project's external evaluator.
