Quillin, the parent(s) and/or legal guardian(s) of the minor participant named below agreed: 1. The parent(s) and/or legal guardian(s) consent(s) to and will instruct the minor participating in any member club activity or event regularly thereafter, that he or she should inspect the facilities and equipment to be used, and if he or she believes anything in unsafe, the participant should immediately advise the instructor of such condition and refuse to participate. 2. Participant shall be instructed to and shall carefully review and follow all Safety Guidelines. 3. I/we fully understand and will instruct the minor participant that: a. There are risks and dangers associated with participation in gymnastic events and activities including but not limited to those of bodily injury, partial and/or total disability, paralysis, and death; b. The social and economic losses and/or damages, which could result from those risks and dangers described above, could be severe; c. These risks and dangers may be caused by the negligence of the participant or the negligence of others; and d. There may be other risks not known to us or not reasonably foreseeable at this time. 4. I/we accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death, however caused or alleged to be caused in whole or in part by the negligence of coaches, instructors, owners and lessees of the premises used to conduct the event or activity and each of them, their officers, directors, agents, and employees. 5. Main St. Cortez, CO 81321 atcgymnastics@gmail.com (315) 209-5846 Fees and Policies Payment Cycle Attendance Policy 4 Week Month. To simplify the billing process, ATC operates on a 4-week month. (There will typically be 4 Absentee Policy. Students will be charged on a 4-week monthly basis for classes in which they are enrolled, regardless of attendance. If you know a student will be absent please notify ATC in writing in advance and ATC MAY credit the fees. classes in any given month. Some months will have 5 classes, and 1-2 months may only have 3 classes. There are more 5week months than 3-week months.) Holidays. No classes will be held on the following major holidays: Labor Day, Thanksgiving, Christmas through New Years, and Memorial day. Fees 1) All fees are subject to change. 2) Annual Insurance Fee. There is a $25 per-studentper-year insurance fee. This fee is nonrefundable. Class Cancellation Policy. If ATC cancels a class for any reason, no participants will be charged for that class. If students have already paid for the class, their accounts will be credited. This will happen very rarely and, whenever possible, prior notice will be given. 3) Class Fees. Standard classes are $7.50 per hour. Prices for special classes or private tutoring may vary. 4) Family/Multi-Class Discount. We provide a Family/Multi-Class Discount. The discount is based on the number of students enrolled and/or the number of classes per week enrolled. 2 student from one family, 1 class = 2 Classes OR 1 Student , 2 classes per week = 2 Classes Withdrawal. If a student decides to withdraw from the program, please notify ATC at once. Without notification, the student will continue to be charged for classes as unexcused absences. Payments Payment Procedure. To expedite the collection process, please: 1. Fill out provided Payment Slips 2. Wear Proper Clothes. Clothes should be loose enough to allow full range of motion, but tight enough to keep from getting in the way. Remember, students will often be up-side-down. Make sure body parts that should be covered remain covered in all positions! Girls may want to wear a tight-fitting shirt or leotard under looser shirt. Foot Wear: Students may either wear socks, bare feet, or gymnastics slipper. Please No street shoes. Do not wear any jewelry (watches, necklaces, and rings included), belts, with buckles or clothes with large zippers. These items can cause serious injury, either to the student, or to instructors and/or other students. If any of these items are worn to class, the student may be asked to remove them. ATC Gymnastics and the instructor are not responsible if any such item is lost as a result. Remove all items from pockets before class. Items such as keys, change, toys, etc. can pose serious hazards, either by being landed on while in the pockets, or when they fall out and are on the tumbling floor. We are not responsible for any items lost or damaged as a result of being in pockets during class. Ponytails or braids required for long hair. Long hair is not only a nuisance in gymnastics, it is also potentially dangerous because it can catch on thing, blind the student's vision, and be ripped out. To limit such problems, hair any longer than shoulder-length must be pulled back. Please do not use stiff hair ties, plastic or metal barrettes, etc. as these items are very uncomfortable when doing many gymnastic activities. Workouts 1. 2. 3. 4. Workouts will consist of the following parts: Warm-up. This will consist of warm-up activities, stretching, and conditioning activities. This section is perhaps the most important because it gets the body prepared for the other activities and helps to reduce the chance of injury. Please be sure not to miss this section and to try all activities to the best of your ability. Basic Skills. These skills, which will vary depending on skill level of class, are target skills which all students in the class are expected to master. These will be reviewed from week to week until mastery is achieved. Feature Activity. These are special activity and skills that will vary from week to week. In this section new stunts will be introduced, certain basic skills will be worked on in greater depth, or demonstrations and learning games may be conducted. Near Review time, this will be the time when program routines will be rehearsed. Cool-down. The last few minutes of class will be devoted to cooling down. This may include post stretching or other such exercises. Conduct in Class 1. 2. 3. 4. 5. 6. 7. 8. 8/26/2013 Always have fun while learning. Learning takes place best when students are having fun. There will be times when gymnastics may be hard work and not as much fun, but if a student is consistently not enjoying the class, please notify the instructors and we will see what we can do to make it more interesting and fun. This goes for all aspects of the program, too. If you have suggestions, complaints, or comments about the classes, please feel free to discuss it with us. Make Friends. Advancement in gymnastics depends to a large degree on trust and interaction between participants. The more friends you make and the more confidence you build in your instructors and classmates, the faster and easier you will learn. Work hard. Most moves are not learned overnight; mastering a skill may take weeks, months, or even years. Don't get discouraged or give up if a skill is not mastered in the first few tries. The harder you work, generally, the better you will become. Listen carefully to instructor. This is crucial to learning and is important to prevent injury. All other activity and talking should cease while the instructor is demonstrating and instructing. Use proper equipment. Some skills may require special safety equipment in order to master, such and skills cushions and landing mats. Others may simply require tumbling mats. Do not attempt moves without the proper equipment. Don't try moves unless instructed. Do not try new moves at home, at school, or with anyone else unless the instructor says you have mastered them well enough. Don't allow unqualified people to spot you for moves. An instructor may make spotting look easy, but an unqualified person may not know what to do in certain situations, and you could get seriously hurt. No horseplay. Gymnastics should be fun, but it can also be extremely dangerous if it is not approached properly. Fooling around not only puts you at risk, it also distracts and endangers other students. No eating, drinking, or chewing gum. Besides making a mess of the equipment, there is also a very strong possibility of choking on these things.
