Change in Status Events Terminating a Domestic Partnership During this difficult time, we want to make updating your benefits as easy as possible, so you can spend more time adjusting to recent events. Making a change in as easy as 1-2-3. 1. Review the benefit plan information below to understand how this life change may affect your benefits. Benefit Plan information is also available on the CarMax Benefits website, available through the CarMax World or at carmax.com/benefits from any computer with Internet access. 2. Log on to mykmxhr.com and click on the appropriate actions under “Benefits” in the blue area on the left side of your screen. Make the necessary changes to your current benefits. 3. Confirm that your elections are correct on the Confirmation Statement you will receive and that your benefit plan deductions are correct on your future pay stubs. You are responsible for making any changes to your benefit plan elections as a result of this life event. Please call the MYKMXHR Service Center at (888) 695-6947, option 0 for assistance and information. Medical, Dental, and Vision Plans Remove your former Domestic Partner and any ineligible children from your Medical, Dental, and/or Vision Plan coverage on mykmxhr.com. The coverage change will be effective the first day of the month after you complete the election. Individuals who lose coverage will be eligible to continue coverage under COBRA and will be notified accordingly. If you were covered under your former Domestic Partner’s plan(s), you may elect coverage under the CarMax plans. Coverage will be effective the first day of the month after you complete your election to enroll. Flexible Spending Accounts HealthCareFlexibleSpendingAccount(HCFSA) If, as a result of the termination of your domestic partnership, you expect your health coverage or out-of-pocket health expenses to change for you and/or your remaining eligible dependents, you may elect to enroll, waive, or change your future contributions in the Health Care Flexible Spending Account. Coverage will be effective the first day of the month after you complete the election. DayCareSavingsAccount(DCSA) If you will no longer be using day care services for any formerly eligible children, you may stop or decrease your contribution to the Day Care Savings Account Program. The change will be effective the first day of the month after you complete the election. Life Insurance If you wish to update your beneficiary for your CarMax Life Insurance coverage, complete a new Beneficiary Designation Form. The form is available on the CarMax Benefits website under Your Money – Life Insurance Plan. DomesticPartnerLifeInsurance If you are enrolled in Domestic Partner Life Insurance coverage, cancel coverage in mykmxhr.com as soon as possible to avoid paying for coverage for an ineligible Domestic Partner.
