About CCNB
CCNB is a not-for-profit, non-government organisation. We have been providing community care in the Northern
Sydney Region for more than 20 years.
What do we do?
CCNB provides services to support older people and their carers to live independently, and stay connected to
their communities.
We also support people with a disability, people with a mental illness, people with dementia and their
carers.

We have more than 60 staff and manage a wide range of community care programs including
Home Care Packages, in-home podiatry services, healthy ageing programs, carers’ support,
dementia support plus many others.
All of our services are aimed at helping people who are vulnerable to maximise their
independence, wellbeing, and participation in the community.

WHY THIS GUIDE
WAS DEVELOPED
This guide was developed as part of the Better Health Care Connections Program,
Keep Me Healthy Longer (KHL). This Commonwealth Government funded program
supported people with a chronic condition to access resources and develop a
self-management plan to - you guessed it - keep them healthy longer. While
the pilot program has come to an end, we are making available the resources
and information developed by the KHL team during the program.
This publication will help people find programs and services related to their
condition.

WHAT IS A
CHRONIC CONDITION?
A chronic condition is an ongoing or recurring health issue that has a significant
impact on a person’s life. These will include conditions such as chronic pain,
asthma, arthritis, heart disease, cancer, anxiety, depression and diabetes. Most
chronic conditions do not resolve spontaneously, and are generally not cured
completely (World Health Organisation).

A diabetes educator is a registered nurse who has additional training in caring for and teaching people about diabetes. HOME

Learn about monitoring your blood glucose levels, the target levels to
aim for, which meter is best for you

A two-phase program to maximise physical and psychosocial well being to enable
