DFA Office of Consular Affairs – Consular Records Division since our marriage (Copy of the
Report of Marriage is hereto attached as “Annex B” and shall form an integral part hereof);
3. That we wish that our marriage be reported and registered at the Consulate General of the
Philippines in Dubai, United Arab Emirates and the Civil Registrar-General (Philippine Statistics
Authority)
