EAST RIDGE HIGH SCHOOL NATIONAL HONOR SOCIETY 2014-2015 SELECTION PAPERS (Turn in to Ms. Bockman’s room #1007 by 3:15 p.m. on Friday, April 10th, 2015) You will type out your selection papers using the following guidelines: 1. The form must be typed or computer generated. Please use one-inch margins, size twelve font and Times New Roman font. 2. Selection is based on the following: Ø Cumulative G.P.A. of at least 3.800 – please provide an unofficial transcript from Infinite Campus as well as your current class schedule Ø Service/Volunteer Activities Ø Co-curricular activities Ø Character Ø Leadership Ø Essay Responses Ø Teacher recommendation form 3. Submitting your application does not guarantee acceptance into National Honor Society. 4. Make sure your entire application is neat, complete, and free of grammatical errors. 5. You must turn in two complete copies of your selection papers. 6. Do not attach any additional materials. 7. Page one will be a cover sheet with the following information: - Full Name - Address - Primary Contact Number (Home) - Secondary Contact Number (Cell) - Email Address - Current Cumulative GPA - Current Grade Level at ERHS This is the only place on the application where your name will appear. *Each application is assigned an ID number for anonymity. The Faculty Council, consisting of five teachers, will read and score each application according to the above criteria. All Faculty Council decisions are final. Page two Your application should include the following labeled sections and information: 1. Leadership – List three elected or appointed leadership positions held in school, community, or work activities. Only those positions in which you were directly responsible for directing or motivating others should be included. For example: elected class or club officer, committee chairperson, team captain, newspaper editor, work area manager or community leader. Use real-life examples as to how you demonstrate the following qualities: - Leadership in the classroom, work, or community - Being thoroughly dependable - Contributing to ideas - Being a positive influence Please provide name of activity, grade of involvement, description, and name of advisor Example Activity: Key Club Grades: 9 and 10 Description: Secretary-assisted with meetings, recorded minutes and delivered them to the entire club via email. Advisor: Mrs. Tollefsrud 2. Service activities – List service activities in which you have participated. These can be service projects done with a group either in or out of school, or done as individual projects performing service. Generally speaking, service activities are those which are done for or on behalf of others (not including immediate family members) for which no compensation (monetary or other) has been given. Please provide name of service, location, date(s), supervisor's name and hours earned and a contact number for the adviser. Example Hospital volunteer at Woodwinds Hospital from August 2013- present; 2 hours per week totaling 54 hours. Contact Anna John at annajohn@woodwindshospital.org or 651.555.5555. 3. Extra or Co-curricular activities – List all school-sponsored activities in which you have participated during high school. Include clubs, teams, musical groups, etc. and major accomplishments in each. Please provide name of activity, grade(s) of involvement, accomplishments/description, advisor or coach name. Example Activity: School Newspaper Grades: 10 and 11 Description: Contributed to the creation of the Literary Magazine Advisor/Coach: Mrs. Evans 4. Other activities - List all other activities in which you have participated that are not reflected in the application and any major accomplishments in each. These activities could include clubs sponsored outside of school such as Boy/Girl Scouts, church involvement or community arts endeavors. Please do not repeat participation already listed above. Please provide name of activity, grade(s) of involvement, accomplishments/description, advisor or coach name. Example Activity: Girl Scouts Grades: 9 and 10 Description: Helped to lead meetings Advisor/Coach: Mr. Mentor 5. Written response questions Your application should also include your answers to the following essay questions: a. Describe a service project that you have participated in over the last year. Tell about the impact that this project has had on your life (150 words or less). b. Identify and describe a problem or a need at our school that you would like to see resolved. How would you, as a member of the National Honor Society, go about improving this situation? Be very specific in both your description and in your plan (200 words or less). 6. Transcript & class schedule After your written responses, attach your transcript (from Infinite Campus) and your current class schedule. Please black out your name using a Sharpie for anonymity. 7. Teacher recommendation form Print the teacher recommendation form and have one teacher complete the form and turn into Ms Bockman’s office mailbox by April 10, 2015. A five-member faculty panel will evaluate membership requests. The method of selection is as objective as possible.
