EdVenture Club-Summer 2015
Please complete the registration/payment and change form to register your child for EdVenture ClubSummer. Be sure to pay special attention to due dates to avoid late fees. If you have any questions, please
contact the EdVenture Club office at 763-241-3544. Additional copies of this form are available online at
www.728communityed.com. The deadline for the first week of attendance is Monday, May 18, 2015.
For your convenience, there is also an online registration/payment system accessible from any computer
or smartphone! Go to https://www.ezchildtrack.com/728communityed/ParentLogin.aspx. You will enter our
parent portal login page (this is not the same as the ISD 728 Parent Portal where you view report cards,
etc.). You will want to bookmark this page for future scheduling. If you used this online scheduling for
EdVenture Club during the regular school year, just enter using the “Returning Customer Sign Up.” If you
are new to the program, you will need to setup your account using “New Customers Open Account.” For

EdVenture Club-Summer will be Closed on July 3
Discount Available! Families that schedule the entire summer, attend a minimum of 9 weeks and pay for
the entire summer by credit card will receive a 10% discount on the total summer cost. The 10% discount
*if attending day of outing
will be processed during the last week of the program by refund to original form of payment. To receive the
discount, include the coupon from the EdVenture Club-Summer flyer along with your payment by credit card in full.

EdVenture Club-Summer 2015 Swim Lesson Registration Form (optional)
 Complete this form if you would like the 1-week swimming lesson option. You must attend every day of EdVenture Club-Summer to participate in the
swimming lessons (5 total swim sessions). The swim registration form is not available online; the regular attendance registration can be completed
online for those weeks, but use this form to enroll your child in swim lessons and select level.
 Swimming lessons are only available for the levels listed below. Check the level carefully; if your child is signed up for the wrong level, he/she may not
be able to participate. Every effort will be made to switch levels if space is available; however, lessons are usually full and it may not be possible.

American Red Cross Swim Lesson Descriptions
Level I: Intro to Water Skills – Helps students feel comfortable in the water and enjoy the water safely. Students learn elementary aquatics skills and
develop good attitudes and safe practices around the water.
Level II: Fundamental Aquatic Skills – Marks the beginning of true locomotion skills. Students explore simultaneous and alternating arm and leg motions
on the level adds to skills begun in Level I. Student must have passed Level I.
Level III: Stroke Development – Builds on the skills of Level II with guided practice. Students will learn to coordinate the front and back crawl, elements of
the butterfly, treading water and rules for head-first entries. Student must have passed Level II.
Level IV: Stroke Improvement – Helps students improve their skills and increase their endurance by swimming familiar strokes (front and back crawl) for
greater distances. Students continue to build the butterfly stroke and be introduced to the breaststroke, elementary backstroke, elements of the sidestroke
and basic turns. Student must have passed Level III.
Level V: Stroke Refinement – Coordination of refinement of all the strokes; front crawl, back crawl, butterfly, breaststroke, and sidestroke are covered.
Students will increase their distance and be introduced to flip turns. Students have passed Level IV.
Level VI: Lifeguard Readiness – Refines strokes of students swim with more ease, efficiency, power and smoothness over greater distances. This course
includes skills for lifeguarding, personal water safety and practices the pre-requisites for Lifeguard and Water Safety Instructor certification.

Field Trip Information:
The weekly field trip is included in weekly and part-time rates. All full-time and part-time students must attend the field trips. Families not wanting to
participate in a field trip can register their student to attend another site not going on a field trip for that day. Field trips are usually scheduled:
Wednesdays* – Otsego, Rogers, Parker
Thursdays* – Hassan, Twin Lakes, Westwood
Students will need to wear a program t-shirt on field trip days for group identification. One shirt will be provided per student. Families will be responsible for
care of the shirt. Additional or replacement t-shirts may be purchased upon request. Please indicate size(s) per child(ren):
