CHURCH USAGE AND HOLD HARMLESS AGREEMENT
We the undersigned authorized representatives of _________________________________

of the city of ________________________, state of_____________ shall be using the
building and grounds of Faith Lutheran Church
from
_Nov. 13, 2015__________ to
____Nov. 14, 2015___________ for the purpose of __Craft Fair & Quilt Sale ____________
herein referred to as the Activity.
We understand and agree that neither Faith Lutheran Church, nor its trustees,
representatives, employees, or agents may be held liable in any way for an
occurrence in connection with the Activity which may result in injury, harm, or other
damages to the undersigned or members of organization and guests, invited or not.
As part of the consideration for being allowed to use your facility, building, and grounds
as well as all appliances and fixtures in the Activity, I/We hereby assume all risk in
connection with participation in the Activity. I/We further release Faith Lutheran Church,
its trustees, representative, employees, or agents, for any damage which may occur while
participant in the Activity. I/We further agree to save and hold harmless
Faith Lutheran Church, its trustees, representatives, employees, or agents from any
claim by the undersigned member of the Organization, their estates, heirs, or assigns
arising out of or participation in any form or fashion in the Activity.
I/we also authorize Faith Lutheran Church, its employees or agents to render or obtain
such emergency medical care or treatment as may be necessary should any injury,
harm, or accident occur while participating in the Activity.
I/We further state that I/We are authorized to sign this agreement;; that I/We understand
the terms herein are contractual and not mere recital;; and that I/We have signed this
document of my/our own free act and volition. I/We further state and acknowledge that
I/We have fully informed ourselves of the content of the affirmation and release by
reading it before I/We signed it.

TERMS AND CONDITIONS • • • • • • • • • • INDOOR OR OUR OUTSIDE TENT Fee is $30.00 per 8 ft. table and is non-­refundable. Two chairs will be available for each vendor. The fee is non-­refundable. Please indicate on your application if you need a table set up (some vendors prefer working without a table). Note: This is an 8’ x 6’ area! If more space is needed, then please request a 2nd space. OUTDOORS AREA Fee is $40.00 per 10 ft. X 10 ft. space. A double space fee is $70. The fee is non-­
refundable. You are responsible for providing your own shelter (if desired) as well as tables and chairs. Parking is allowed directly behind your space! Crafters must remain until 3:00 p.m. and someone must be at your table at all times. This is strictly a craft fair. No flea market or “Made in China” items. No items are to be displayed that were purchased for RESALE. Please keep in mind the craft fair is being held in a church. We reserve the right to remove anything we consider inappropriate. We assume no responsibility for theft of or damage to merchandise, equipment, or personal property, or for personal injury from any cause. Please enclose a self-­addressed stamped envelope so we can mail your area assignment. Please make checks payable to: Faith Evangelical Lutheran Church. Crafters will be asked to unload their goods and then park on the grass area at the lower side of the church, to make room for patrons to park. No Smoking Indoors! A refreshment stand will be open all day. A donation of a handcrafted item would be appreciated for our Chinese auction. Your canceled check is your confirmation. You will be notified a month before the show of your assigned area.
