How do I sign up for a
Patient Portal Account?

 Login: choose this option if you have an existing FMH
account with a healthcare organization.
OR
 Create an Account: choose this option if you do not have
an existing FMH account with any other healthcare
organizations.
Complete the demographics screen with the required
information.
You must accept the Terms of Service.
You also have access to the Privacy Policy on this step. Clicking
the link will open the Privacy Policy.
Select your account login method
This login verifies your identity to FollowMyHealth™ (FMH).
None of your information will be shared from FMH to them or from
them to FMH. FMH does not store your login credentials and does
not have access to your password.
Please write down your username for future use. For security
purposes, Greensboro Orthopaedics does not have access to your
username or password.
You will be prompted to request a connection to area healthcare organizations who participate in FMH.
You can search by zip code, provider, or organization for matches. FMH will search by zip code by
default. To Connect with Greensboro Orthopaedics, search by zip code and enter zip code 27408.
Once you have chosen to request a connection to Greensboro Orthopaedics, your request will be
processed in five (5) business days. We will receive confirmation of connection or a notification if we are
unable to connect you to our organization.
