The Canadian Mental Health Association – Halton Region Branch (CMHA-HRB) is seeking a full-time
temporary Case Manager, Dual Diagnosis (DD) and Community Support Programs (CSP). The
position is a 37.5 hours per week role based in our Oakville, Ontario office. The position resides within
an integrated team of individuals who work closely together to provide meaningful programs to people
living with mental health and addiction issues who have a coexisting developmental delay (Dual
Diagnosis).
The Dual Diagnosis and Community Support Program strives to support people (who experience dual
diagnosis and mental health issues) to:
 Live full and meaningful lives in the community
 Enhance coping strategies, life skills and social skills
 Have a strong network of reliable and supportive relationships
 Have choices about where to live and work
 Have services that are accountable to the person and family supported and are available as
needed
The successful candidate will:
 Provide individual 1:1 support coordination to clients with significant mental health, addictions
and dual diagnosis issues
 Focus on strategies to reintegrate clients with mental health, addictions and dual diagnosis
concerns into the community
 Make referrals on behalf of clients to a wide variety of community programs and services
 Support individuals within the mental health and developmental system by empowering them,
identifying goals and strengths and successfully implementing formal and informal supports
 Promote the development of enhanced capacities and a holistic approach to recovery through
enhanced functioning, community involvement and healthier relationships
 Commit to supporting health, safety and wellness according to relevant legislation and agency
policies and procedures
Responsibilities:
 Responsible for all aspects of case management and maintenance of client records
 Developing and working in a comprehensive and potentially growing service for those with a
significant mental health and addiction issues and a developmental disability
 Facilitate skilled-based groups and organized activities in the areas of social and life skills







Work with individuals to create a holistic service plan, focusing on strengths and capacities,
service coordination and the needs of the individual
Engage in proactive planning with individuals with dual diagnosis; develop individual treatment
and service plans that address the individual’s specific needs which may include crisis plans,
support and diversion plans with criminal justice system as well as community support plans
Engage and support families where appropriate and needed
Be present at the site during peer run groups
Work collaboratively with community partners in a multidisciplinary approach
Mental health and dual diagnosis education and advocacy
Other duties as assigned
Qualifications:
 Bachelors degree in nursing, social work, occupational therapy or a relevant field
 Master’s degree and/or certificate in Dual Diagnosis are an asset
 Regulated health professional (registered social worker or registered nurse) an asset
 At least three (3) years experience working with individuals with complex mental health and
dual diagnosis concerns
 Experience developing group activities and programs
 Commitment to the Recovery philosophy of service provision
 Community outreach and knowledge base of community resources and services
 Able to work effectively independently or within a team environment
 Excellent oral and written communication skills
 Initiative, creativity, flexibility and an openness to change
 Non-violent crisis intervention, first aid, CPR and ASIST certifications are an asset
 Knowledge of CRMS database an asset
 Proficiency in MS Word and Excel
 Valid driver’s license and able to provide own transportation
 Ability to speak a second language would be considered an asset
