HAMMARSKJOLD HIGH SCHOOL 2014 – 2015 INTERNAL SCHOLARSHIPS BURSARIES AWARDS & GENERAL APPLICATION FORM Hammarskjold - INTERNAL AWARDS The awards in this booklet are presented to deserving Grade 12 students who are in their graduating year. However, only those students who have submitted a fully completed Hammarskjold Internal Awards General Application Form will be considered by the Hammarskjold Scholarship Committee. (Note: Previous graduates are ineligible to apply.) Please complete and submit the Application Form (along with the additional requested documentation) to Student Services by Wednesday, April 1, 2015. The Hammarskjold Internal Awards General Application Form is included as the final two (2) pages of this document Note: 1. List in alphabetical order the awards for which you qualify. Please print neatly and use a black/blue pen. Do not apply for awards marked “You cannot apply”/ “Do not apply”. 2. Remember to read the criteria thoroughly. Some awards require the completion of an additional application form. Other awards require identification and/or verification of specific details. The completion of the 2nd page of the Application Form will help ensure that all requirements are met. Please do not attach résumés or unrequested information. 3. Be aware of the deadline date! 4. Notify Student Services immediately upon news of your receiving any external awards so that appropriate arrangements/recognition may be included in the graduation exercises. 5. Do not forget to include your Social Insurance Number (S.I.N.) with your application form. If you don’t have a S.I.N., you may apply for one online. (Go to www.sdc.gc.ca/. Click on Forms.) 6. Remember that the availability and the amounts of all awards, bursaries, and scholarships are subject to yearly change.
