Medical Consent (for parent’s signature)
I hereby state that my child is in good
normal health, and has my permission to
participate in all camp activities. In addition, I
authorize the PVI Soccer Boys’ Camp Staff to act
for me in securing medical treatment for my child
in the event of injury and sickness. A registration
requires that a parent/ guardian sign below to
agree that in case of an accident involving their
child while attending the PVI Soccer Camp they
release the camp, the ownership, the counselors,
the directors, Paul VI Catholic High School, and
the Commonwealth of Virginia from any and all
liability.

PVI SOCCER CAMP
Our goal at the PVI Boys’ Soccer Camp
is to improve your athlete’s technical
and tactical abilities in a fun and
positive environment. We will
emphasize the importance of
fundamentals while developing
decision-making abilities. We want to
ensure each camper is on their way to
reaching their full potential while
having fun at the same time. For the
more advanced players, instruction
will be focused on maturing as a
player and steps to take to reach the
next level. Athletes will be given a
glimpse of what to expect when
joining a strong high school program.
CAMPER TO STAFF RATIO
Enrollment will be limited, so we can
ensure a 10-1 campers to staff ratio.
EQUIPMENT
Campers are expected to provide cleats,
shin guards, running or tennis shoes,
soccer ball and water bottle.
In Case of Rain
In the event of rain, camp will be held
inside the school cafeteria. Activities
will include applicable videos, small
games and interactive instruction. No
rain dates will be rescheduled and no
reimbursements will be given.
Lunch
Campers can bring their own lunch or
lunch will also be sold daily.

The past 6 seasons have been exciting
for the Panthers. In 2010, the squad
enjoyed one of their most successful
seasons in program history, finishing in
the top 4 in the WCAC and winning
their first ever Virginia State
Championship. PVI placed 3 athletes
on the All-State Team and numerous
all conference selections. In 2013, the
Panthers were ranked in the NCAA
Regional Ranking all year, defeated
the #21 ranked team in the country
and Co-Captain Tyler Phillips became
the first player from PVI to be named
VA State Player of the Year. This past
year, senior Co-Captain Jimmy Skipper
was named WCAC Player of the Year,
VA State Player of the Year,
Washington Post All-Met First Team,
and NCSAA High School Soccer
All-American (the first PVI player to
earn this honor).

Head Coach
Matt Leiva
Entering his 7th season at the helm of
the Paul VI Boys’ Soccer Program,
Leiva has already enjoyed a lot of
success in Fairfax. The Panthers have
established themselves as one of the
elite high school programs in the DC
Metro, winning over 60 games and
winning the 2010 VA State Title.
Known for taking a program and
turning it around, Leiva took his 1992
Real Mclean boys team from obscurity
to the First Division of the NCSL and
also winning the National Indoor
Championship. A select group of PVI
players won the 2013 edition of the
same National Indoor Championship
Tourney in January. During his years
of coaching, Leiva has had over 60
former players move on to play at the
collegiate level.
Camp Director
Chuck LaPorte
The former Head Coach at Bishop
O’Connell High School in Arlington,
Coach LaPorte joined the Panthers Staff
in 2012 as Assistant Coach and Director
of Soccer Operations. Chuck has been a
very successful high school coach over
the years, reaching the WCAC Final
Game twice. He currently assists in
practice planning, scouting and
managing day to day operations for the
soccer program.
