This form must be submitted to Mrs. Grana’s office no later than Tuesday, April 14 at 8 a.m.
STUDENT RESPONSIBILITIES:
● The Ladue student is responsible for the guest and must notify the guest of all school rules.
● Only one guest per student. All guests must be in high school or under the age of 21. Guests in high school must
obtain a principal’s signature on the bottom of the form. Guests not in high school must have a parent sign the
form.
● No guests will be admitted to the dance alone; all guests must accompany a Ladue student and present a picture ID.
● Any guest forms received after the deadline above are subject to refusal. If a guest form is refused, your ticket
money will be refunded.
● Attendance at the dance is a privilege, not a right. This privilege may be revoked at any time.
● This guest form is not a ticket. Tickets must be purchased separately.
