Before you sign your visa application form, use this checklist to make sure your application is complete. 2. When you are satisfied that you have correctly completed your visa application form and all of the relevant documentation is included, you should sign and date your visa application form. 3. Include the completed visa application checklist when you lodge your visa application form and relevant documentation at the Australian High Commission. Important information Please be aware that the decision on your visa application may be based on the information and documentation that you include at the time of lodgement. Applicants should not make arrangements to travel to Australia until advised that their visa has been approved. You may be asked to provide information in addition to that provided in your application. If this is the case, you will be given a date by which you will have to provide the information. The provision of false or misleading information or documents may have an adverse effect on the outcome of the application and will delay processing. You must provide a certified copy of the documents, not the originals. Please be aware that any original documents you choose to provide may not be returned to you. If an original is required, the Immigration processing office will request this from you. All documents must be certified by the relevant authority. Documents not in English must be accompanied by accredited English translations. Lodgement of visa applications from Mauritius: - Tuesday and Thursday from 09.30 am to 12.00 Noon (by appointment only) Please contact this office by email on ‘immigration.portlouis@dfat.gov.au’ to make an appointment to lodge your visa application. Applications from Seychelles, Madagascar and Comoros must be lodged via courier to the Australian High Commission in Port Louis (please note that applications should not be sent by regular post). Complete this checklist and attach all documentation – in the order provided below. The checklist should be placed to the front of your application. Tick the documents you are submitting Official use only A completed and signed Form 157A – Application for a student (temporary) visa. Subclass 580: Student Guardian Visa – Complete Form 157G Application for a student guardian visa. Attach all necessary documents to the form. If you are not the student’s parent or legal custodian, you need to ask the parent or legal guardian to complete Form 157N Nomination of a student guardian. Whichever is applicable, a completed and signed: Form 956 - Advice by a migration agent/exempt person of providing immigration assistance or Form 956A - Appointment or withdrawal of Authorised Recipient Payment of the Australian Government Visa Application Charge at the Visa Counter or if the Visa Application Charge has been paid at a DIBP office in Australia, a copy of the receipt. Certified copies of the biographical pages of the current passports or travel documents of all people included in the application (these are the pages with the holder's photo and personal details and the issue/expiry dates). Include copies of all pages that contain visas and stamps. Note: your passport must have at least six months left before the expiry date. Two recent passport-sized photographs, no more than six months old, of you and any family members included in your application and travelling with you. A certified copy of birth certificates for you and any dependents included on the application. If you are married, a certified copy of your marriage certificate; The electronic Confirmation of Enrolment (eCoE) certificate(s) including any ELICOS or foundation courses, or a letter of offer Overseas Student Health Cover (OSHC). The OSHC should cover the full period of your intended stay in Australia and show the expiry date. The payment must have been made. Family OSHC: If dependants are accompanying the student then the OSHC must specify that the insurance is ‘Family cover’. The OSHC should cover the full period of your intended stay in Australia. See: www.immi.gov.au/students/health-insurance.htm English language: Depending on your Assessment Level, you will need to either declare your English language capability or provide evidence of an English language proficiency test undertaken in the last two years. See: www.immi.gov.au/Study/Pages/student-visa-assessment-levels.aspx THIS IS NOT AN AUSTRALIAN GOVERNMENT FORM STUDENT VISA (SUBCLASS 570-580) Page 2 of 4 Financial requirements: The department must be satisfied that an applicant, once in Australia, will have genuine access to the funds they claim to possess to cover the travel, tuition and living costs for themselves and their family members. This includes funds to cover yourself, your partner and your dependent children during your stay in Australia. Detailed information on defining acceptable sources of finance, calculating the amount of funds required and categories of expenses which need to be funded for living in Australia are available on the Immigration website. Go to the Find a Visa (on the www.immi.gov.au homepage) search tool and enter the details particular to your circumstances. See: www.immi.gov.au/Study/Pages/Study.aspx and www.immi.gov.au/students/student-visa-living-costs.htm Note: Non-cash assets such as property and shares are not acceptable sources of income. However, noncash assets may be liquidated or used as collateral on a loan, if they are provided by an acceptable source. Credit cards are not acceptable source of funds. We do not accept internet print outs of bank statements. All submitted bank statements must be original. Funds will be verified through the bank. Business accounts are acceptable sources of funding only if there is evidence of the business ownership and declaration from all business owners/directors stating no objection to the funds being used for student visa purposes. If you are Assessment Level 3: Corporate sponsorship is an acceptable source of funding only if the corporation is genuinely operating in the international arena; the applicant is an employee of the company and the proposed course of study is relevant to your role in the company. Evidence of education/qualification and work history up to the date of application. Applicant under the age of 18 - Necessary Requirements If you are under 18 years of age, your parents and/or anyone who can legally decide where you can live must give permission for you to study in Australia. If any of these people cannot sign your student visa application on your behalf, they must sign one of the following: a Sworn Affidavit made at the Supreme Court of Mauritius giving their permission. You must submit this declaration with your application. the consent form for children under the age of 18 years See: Form 1229 Consent form to grant an Australian visa to a child under the age of 18 years. Please note that any of the above documents should be signed before a Notary Public. Applicants under the age of 18 years must provide: If travelling without both parents, evidence that the student will be taken care of in Australia. Two forms of identification (with a signature) for any non-accompanying parent. Where one or both parents are deceased, proof that the remaining parent or the guardian has legal custody of the applicant Please note: Where there are custody issues, further documentation may be requested If there are minors included in the application Evidence of enrolment in a school in Australia for each child (aged 5 – 18 years) included in the application. Health You will be required to undergo a chest x-ray (form 160) and a medical (form 26) by a panel doctor nominated by DIBP.
