OFFICIAL South Texas College (STC) transcript of records. Copy of HESI Admission Assessment (A2) scores within the last two years. Copy of current Paramedic license. To expedite your application process, kindly comply with the following procedures: 1. See Clinical Compliance Specialist, in NAH R-1.602 and get clearance with item #6 in the application form. “Students must complete and provide documentation to Clinical Affairs Specialist for clearance on their Background Check, Hepatitis B series or titer, and other requirements prior to applying to the Associate Degree Nursing. Please see Clinical Compliance Specialist in NAH R-1.602 for clearance on all other requirements.” 2. Mark a (√) in all the areas in the application form which apply to you. If you answered “NO” to questions #1-7, you must schedule an appointment with a faculty advisor before submitting your application. 3. 4. Highlight all pre-requisite and co-requisite courses to be credited on your transcript of records. Compute your GPA and total acceptance points by following the formula on page 2 of the application form. Complete and sign the application form. All pre-requisite courses completed must be credited onto your STC transcript. 3. Students must be TSI complete or TSI Exempt via State approved exam scores. Students must earn a cumulative GPA of 3.0 on a 4.0 scale in all pre-requisite and co-requisite courses as listed on the ADN degree plan. 6. Qualified applicants will be ranked from highest to lowest points. You will be notified by email to your Jagnet address of your acceptance/denial status.
