Medco Energi US LLC | 201 Rue Iberville, Suite 700 | Lafayette, LA 70508 (O) 337-456-7016 ___________________________________________________________________________________ CONTRACTOR SERVICE REQUIREMENTS Dear Valued Contractor / Service Provider: Medco Energi is pleased to inform you of our continued implementation of our Contractor Safety Management Program. This program is a result of and furtherance of Medco Energi’s SEMS Implementation and HSE Policy Statement and Corporate Values of protecting the Health and Safety of all personnel, including contractors, within our operations. PEC Safety will continue to assist Medco Energi in implementing its Contractor Safety Management Program. As a result, all Medco Energi contractors are required to complete the following requirements within 60 days of receipt of this letter. 1. Standardized Safety Questionnaire (SSQ) – To provide services or equipment on a Medco Energi location, contractors must provide safety records for review. To facilitate this process, all contractors must complete an SSQ in PEC Safety’s database. 2. Update SSQ Quarterly – Once the SSQ is completed, you will be required to update the SSQ quarterly: Enter statistical data for the previous quarter including man-hours contractor personnel worked on Medco Energi’s facilities (see # 3 below). Update any other SSQ information that has changed (for example: changes to services, training, or policies/procedures) The SSQ quarterly update for the General Safety Incident Statistics section (statistical data) will be available on the following dates each year: January 1st, April 1st, July 1st, and October 1st 3. Medco Energi’s Specific Quarterly Man-Hour Section – Medco Energi is requiring your company to report exposure hours and recordable cases related to work performed on Medco Energi facilities on a quarterly basis. This table is located in the SSQ under the section – Operator Specific Quarterly Incident Section. If your company did not have employees on Medco Energi’s facilities during a quarter, you are still required to complete the table by recording zeros for those quarters. 4. Document Submittal – Medco Energi will use PEC Safety to track and verify your company's required documents. Contractors performing work on a Medco Energi location must submit the following: Certificate of Insurance with Medco Energi as the certificate holder (Acord form in most states) Last three years of OSHA 300 and 300A forms Injury/illness summary forms Last three years of Experience Modifier Rate (EMR) documents received from your workers compensation insurance company EHS Manual/Safe work practices CNL revised April 21, 2015 Medco Energi US LLC | 201 Rue Iberville, Suite 700 | Lafayette, LA 70508 (O) 337-456-7016 ___________________________________________________________________________________ Training plan and/or matrix Management of Change (MOC) procedures, including a sample form JSA procedures including a sample JSA form Mechanical Integrity Plan (if providing critical equipment on Medco Energi premises) Complete Employee Training Records to include a training matrix, employee job titles, and course completion verification documents (enter in Training Tracker) Trainer qualifications and supporting documents for all training conducted. Master Service Agreement (MSA) Bridging Document (executed) To meet the Document Submittal requirement for Medco Energi, a contractor must purchase PEC Safety’s Verification Package annually. If your organization has already purchased PEC Safety’s Document Upload and Training Tracker Bundle, the upgraded Verification Package, or maintained a CAP membership in the past year to meet other customer’s requirements, you will not have to purchase an additional subscription until your renewal date. (Please see Attachment I for annual fee information) 5. Training – At a minimum, all personnel arriving on a Medco Energi location must have SafeGulf Orientation and meet the requirements in API RP 75 Section 7 as applicable. 6. Training Records Reviewed –Contractor employees who are assigned to perform work, services, or provide equipment at any Medco Energi location will have their training records periodically reviewed for compliance. To facilitate this process, please upload contractor employee training records, including certificates, into PEC Safety's Training Tracker (included in Verification Package). PEC Safety does offer multiple avenues to enter your existing training records into the PEC Safety system via data transfers and customized programming. If you have completed PEC Safety’s Core Compliance, PEC Safety’s Core Compliance Refresher, PEC Safety’s Basic/SafeGulf orientation, these courses will automatically be populated in the PEC Safety Training Tracker. In order to be considered for selection by Medco Energi during the contractor bid process, your company's subscription must be in place and all required information must be posted within 60 days from receipt of this letter. Medco Energi’s goal is to provide you valuable, easy to use tools and services that will help expand the safety culture within your organization and make this part of the bid process affordable and efficient.
