Note: A CQE does not guarantee a license or a job, but it may make the process easier.
Waiting period to apply for a CQE is 6 months for a misdemeanor or 1 year for a felony starting from time of
discharge from all supervision (probation, parole, etc.), not release from incarceration.

for each crime you have been
convicted of or pled guilty to
4. name, address & phone number of
employers from last five years (if applicable)
5. full name & phone numbers for
3 non-family references
6. full name & phone numbers for
3 family references
If you are seeking an occupational license, also include:
1. the name of the licensing board
2. documents that demonstrate your criminal record hinders from getting a job or license
