PHC is a non-profit community-based healthcare organization that contracts with the State of California to administer MediCal benefits through local care providers. the Gap Bridging Being Ready When Opportunity Knocks NVHADS Key Part of Development of New Statewide Program 4 Dr. Robert Moore, the chief medical officer for PHC (and former chief medical officer of Napa’s own OLE Health), had identified that members with progressive, serious illnesses were often enduring prolonged hospitalizations, visits to the emergency department, and treatment for problems that could have been avoided. Moore knew that a community-based palliative care program could prevent many of these problems—such as uncontrolled symptoms, inadequate social support and a lack of communication—to help to keep members well and out of the hospital. The problem? Medi-Cal didn’t have an outpatient palliative care benefit. One Step Ahead In 2011, NVHADS had identified a gap in care delivery for those with chronic and progressive illness and launched a community-based program: Transitions Palliative Care. Since its inception, Transitions has cared for more than 550 patients in Napa County, providing an extra layer of expert support that prolongs independence and prevents unnecessary hospital admissions and emergency department visits for these individuals. In 2014, the State of California passed SB 1004, which paved the way for developing regulations and a reimbursement methodology for an outpatient palliative care plan for Medi-Cal recipients. In response, California Healthcare Foundation funded a study to identify and implement community-based palliative care. With NVHADS’ already-proven success in developing and delivering a palliative care program, PHC and NVHADS partnered on a proposal for the project and were selected to participate in the study. Learning from the Past for a Better Future “Other health plans have conducted pilot programs that have shown some level of success,” explains Moore, “but we learned from these and the success of Transitions how to deliver the most impact while remaining sustainable at this larger scale.” “The true measure of success in this effort will be 1) better patient outcomes, 2) improved patient and family satisfaction, 3) a plan of care that is aligned with the patient’s goals, and 4) a reduction in overall healthcare costs when comparing the cost of palliative care to hospitalization/emergency room costs. “The U.S. has the most expensive healthcare system in the world,” says Dr. Moore. “I think California is setting the stage for this new kind of care, and that the model Napa Valley Hospice & Adult Day Services is developing with us can be viable, sustainable and equitable for the future.” “Launching new programs such as this one and its Annual Palliative Care Conference has really put NVHADS on the statewide map as far as forwardthinking models and services. It really speaks to the leadership at NVHADS. Linda is a real visionary and I haven’t seen a leader of this caliber at any other hospice of this size,” says Dr. Moore. Hospital 2 Home – Pathway to Health Supporting Patients on the Road to Recovery After Leaving the Hospital Sometimes innovation is less about blazing new trails and more about finding new and better ways to connect existing ones. That’s what NVHADS did when it developed its Hospital to Home (H2H) program in partnership with St. Helena Hospital Napa Valley (SHHNV). NVHADS President & CEO Linda Gibson and SHHNV President & CEO Dr. Steven Herber were discussing the high number of hospital readmissions among certain populations due to conditions that could have been easily avoided. “We have expertise in home care. I saw no reason why we couldn’t successfully develop a program together to help reduce the number of readmissions,” explains Linda. The two organizations identified the patients most at risk for difficult recoveries or possible readmissions after leaving the hospital. What they found in common among these patients was that they lacked health education and proper supportive care. “These patients don’t need professional at-home help,” says Nia Lendaris, RVP of Patient Care for SHHNV, “but they do need help making sure they have the resources during their recovery to avoid unnecessary readmissions.” So Gibson approached SHHNV with an idea for a care transition program for these patients. The two organizations set up a process for NVHADS to provide a nurse navigator to: • meet with identified patients before discharge to discuss their needs and make plans for follow-up contact • visit the patient at home shortly after discharge to assess their health status and provide health information • make sure the patient has scheduled a follow-up appointment with their primary care physician • follow up with phone calls to support their progress to recovery The result? H2H significantly lowered the readmission rate among this at-risk population to nearly half the overall hospital readmission rate. The results were promising enough that in July 2014, Partnership HealthPlan of California asked SHHNV and NVHADS to participate in the development of a pilot program for its members to provide a care transition program and to develop a model that would be sustainable in rural communities. “The plan is straightforward and effective, allowing us to ensure our patients have the best outcome possible,” says Lendaris. “It extends our ability to care beyond the hospital walls.” Napa Valley Hospice & Adult Day Services was awarded the 2015 Compassionate Care Leadership Award by the Coalition for Compassionate Care of California—a statewide non-profit dedicated to the advancement of palliative medicine and compassionate end-of-life care. The award is presented annually to an organization that has distinguished itself through exemplary service, commitment and caring for those with serious illness or who are approaching the end of life. NVHADS was recognized for: • A commitment to providing person-centered care • An organizational culture enabling staff to better meet the needs of seriously ill patients • Significant contributions to the palliative and end-of-life care community in California “Napa Valley Hospice & Adult Day Services is a clear leader in caring for the seriously ill in California,” said Judy Thomas, JD, CEO of the Coalition for Compassionate Care of California. “Their ability to target services to underserved populations in Napa County, and strong community partnerships, make NVHADS a role model for compassionate care programs throughout California.” “Napa Valley Hospice & Adult Day Services is a clear leader in caring for the seriously ill in California. Its commitment to raising awareness and influencing attitudes about palliative care shows why this organization is truly deserving of this recognition.” - Judy Thomas, CEO, Coalition of Compassionate Care of California 5 Person-Centered Care Innovation at the Personal Level Sometimes the best innovations come from simply flipping an existing model or way of doing things—and for Adult Day Services’ director Celine Regalia, the inspiration came in the form of a random flier that landed on her desk. In adult day services, the traditional model is a program with schedules and activities that are often based on the participants’ diagnoses as a group, which can have an “if it’s Tuesday, it’s meatloaf” predictability. Structure and consistency are important for some participants, but these don’t always address an individual’s personal story, goals, needs, or interests, and can create frustration, a lack of engagement and an unequal relationship between caregiver and participant. While the ADS team strove to create engaging and rich programming, Regalia thought there had to be a better way to individualize a group program to each participant’s needs. “I wanted to treat each of them as a person, not as an illness or disability. I wanted to create programming that would honor the life they had lived, and the life they now lived, despite appearances or expectations,” she explains. 6 When she received a flier offering information and training in a new “Person-Centered Care” model, she wondered if it was answering the very question she’d asked. She and ADS Nursing Services Manager Karen Proteau attended Wisconsin Alzheimer’s Association’s conference to learn more and see the model in action at Luther Manor Adult Day Services. They left with a copy of Person-Centered Care in Practice, a guiding text written by Lyn Geboy and Beth Meyer-Arnold, who had presented at the conference. The program is built on the idea of using respect and empowerment to protect or, in some cases, restore participants’ personhood. Their life stories drive the program, so rather than placing individuals in existing activities or programs—putting the peg in an existing hole—Person-Centered Care modifies the hole to fit the peg. If Joan likes quilting, or Tom likes singing, programming is created around those activities in a way that allows those participants to help develop, and often facilitate, the activity. With Person-Centered Care in Practice to guide team members, they reimagined programming to enhance existing physical activity and mental stimulation, and monitor for loneliness and depression, practices shown to improve participants’ brain health. The result? Participants were almost immediately more engaged and empowered, but so were the staff and volunteers, which helped balance the relationship between participants and staff. Together, they built the program, and together, they reap the benefits of increased safety, neural plasticity, independence, and social connectivity. Evidence-Based, Person-Centered Care “We had found a way to remove stigma around attending an adult day center, by developing a rich program that treats participants with respect, as an active member of their own care team and their community,” said Regalia. The Person-Centered Care model, now used by Adult Day Services for three years, is more open and organic, designed to give clients a sense of purpose. Some of its practices include: Adult Day Services became one of the first adopters of Patient-Centered Care in California, and was approved by the California Association of Adult Day Services to train other providers from across the state in this new model, sharing best practices with other organizations. 1 “We watch for anything that can be a detractor for each person—how the facility is set up physically, or if there is discomfort due to unfamiliarity or a lack of interest, for example—so we can remove anything that is a barrier to engagement,” says Proteau. “We pay attention to not just the words participants use, but to their emotions or behavior. If someone has difficulty sharing thoughts or needs, behavior can be the next-best way to communicate. Someone acting in a way that could be interpreted as ‘aggressive’ or ‘anxious’ may be reacting to an unfamiliar or uncomfortable setting. Once we discover and remove the cause, the behavior often disappears, and as we get to know more about our participants and their stories, we’re able to better identify any issue quickly.” 2 ADS pairs or groups participants with similar interests to encourage self-directed activities. “For example, one participant had been a science teacher,” explains Regalia. “He loved to teach, so we worked with him to offer a science class for other participants. We also have an ‘armchair travel’ program, and when we found out that two participants were of Greek heritage, we brought them together to co-lead a ‘trip’ focusing on the culture and history of Greece. Both of these gave participants the opportunity to share their knowledge, and to develop personal connections with others, increasing social confidence. “Another participant was quite reserved. We learned that she loves to cook foods that reflect her Southern heritage, but is unable to do so because she lives in a board and care home. We made a batch of grits from her recipe to share with other participants, and as they enjoyed her dish, she reminisced about her life in the South, her family and cooking. She just opened up, even speaking into a microphone to share her stories, with a smile from ear to ear. “These kinds of activities can have a lasting effect and help to create a community among participants who then are invested in looking out for each other,” she says. In response, between 2012 and 2014, ADS doubled the number of programs available—in some cases adapting existing programs to be more person-centered—including horticulture therapy, participant-led activities, and quilting and sewing, and has received a grant to develop a fine arts program with participants. 3 Empowering participants means not only looking at their pasts, but looking to their future, so ADS works to help them achieve personal goals, incorporating more therapies— physical, occupational, or speech as needed—to help them accomplish those goals. “Person-centered care is not a destination, but an ongoing journey,” Regalia explains. “By empowering participants to build connections, use their interests, and meet goals, we celebrate their individuality and uniqueness, and that means each new participant enriches our program. “We measure success through how engaged each participant is, because when participants are socially engaged, they feel respected and valued, which is a birthright, no matter our age or condition.” 7 Finding the Middle Ground Giving Veterans With Serious Illness New Options for Staying Well and Independent The Veterans Home of California-Yountville, the largest veterans’ home in the United States, has more than 1,000 aged or disabled veterans calling it home. One of its goals is to provide veterans with a living environment that protects their dignity and contributes to their feelings of self-reliance and self-worth. One challenge has been helping those with advanced, chronic illnesses maintain that dignity and independence. “We had been trying for years to be more forward-thinking about quality of life for vets coping with advanced chronic illnesses,” says Julie Fields, MD, MPH, Chief of the Ambulatory Care Clinic. “So when NVHADS approached us to offer, free of charge, its Transitions palliative care program to our vets, we jumped at the opportunity.” 8 “The Veterans Home was once a hospital, complete with an ICU, so for many years there has been what I call a more aggressive approach to treatment of serious, long-term chronic illness. Our veterans are warriors— they have been trained to not back down quickly and of course they want to remain independent at any cost—but what we are finding is that more aggressive treatment does not always equate to independence. In the past there hasn’t been a middle ground to meet them between aggressive work-ups/ treatment and ‘giving-in’ and moving into skilled nursing care.” “But adding palliative care allowed us to create that middle ground. It has reduced participating vets’ health complications, increased their comfort and given them more of a voice in the kind of care they want. It allows them to stay independent for as long as possible.” “While our initial goal was for Transitions to allow those with serious, advanced illness who wanted to continue to live independently to do so, we’ve found that palliative care also gives vets the opportunity to determine what ‘quality of life’ means to them in the face of illness. They now know what the options are and it allows them to face these possibilities and to make informed decisions for the future. They’re more comfortable with planning to ensure their needs and wishes are honored.” “One of the operational benefits for the Veterans Home of bringing in Transitions was that we didn’t need to modify the existing care plan here to incorporate palliative care. Transitions Nurse Navigator Angela Kaufman worked with our doctors, nurses, social workers and section leaders to develop processes, creating more resources for staff and patients alike without adding another level of bureaucracy.” “It’s allowed both our staff and our residents to think beyond treating illness to how to create the best quality of life possible.” Legacy Society Bonny & Tom Thrower For many, making a gift to NVHADS is a personal way to say thank you for the care they or a loved one received from our organization. For Bonny and Tom Thrower, however, including NVHADS in their estate planning was about the future. “It’s self serving, in a way,” says Tom. “We hope to never need their services, but it is such an important part of the Napa Valley, and we wanted to help make sure it’s there in the future for anyone who needs it.” “My sister was a hospice social worker in Phoenix, St. Louis and Denver, and we’ve had friends who’ve used NVHADS’s services, so NVHADS is near and dear to our hearts,” says Bonny. Originally from Missouri, the Throwers moved to the Bay Area in 1975. In 1996, they built a second home in Yountville and moved here full time after retiring. They learned about NVHADS when their friend Valerie Peebles joined the board at NVHADS and invited them to attend the organization’s annual gala. The Throwers have continued to attend the gala whenever possible and support the organization with annual contributions, but last year they decided it was time, as Tom says, “to step up and make a worthwhile contribution,” by including a legacy gift in their estate plan. “We had our attorney rewrite our trust to add an addendum, we gave a copy of the addendum to NVHADS and it’s all done.” “So many of us put off major decisions, thinking, ‘I’ll do it someday.’ But the time to make these is while you’re healthy, so that you can make thoughtful, informed choices without the pressure of time or health concerns. “There’s nothing to lose by supporting such a worthy community service,” says Tom, “yet so much to gain. It was an easy choice for us to make.” “Art washes from the soul the dust of everyday life.” –Pablo Picasso And now, thanks to the generosity of renowned artist Ira Yeager, the lobby of the NVHADS offices offers that experience to guests and staff. Yeager, whose work is on display in galleries and museums around the world, donated an original 66” x 66” oil painting, “Floral Still-Life, 1785,” to NVHADS which now offers a warm and uplifting presence in the reception area. Payne Transforming one arch of gold manzanita branches, eight strands of icicle lights and 500 twist ties—not to mention 20 hours intertwining all of the above—into a fantastic entrance to the annual NVHADS Light Up A Life is just one of the creative ways Sue Payne has supported the agency over the past 25 years. Sue, an artist and professional window designer, is the creative force behind the Light Up a Life displays – from the early window displays and towering Christmas trees in downtown Napa to the twinkling manzanita arch entrance that graces our Light up a Life display at its current home, the V Marketplace, each year. But Sue has also generously and creatively supported NVHADS through another of her endeavors: the Sue F. Payne & Friends Annual Holiday Shopping Extravaganza, an art show event featuring the work of dozens of artists each December. Part of the draw of the show is the raffle Sue has organized. “I ask each artist to donate something to the raffle, and I package all of the items to create a lot worth at least $200. All raffle proceeds benefit NVHADS, and we usually raise around $2,000 for NVHADS each year.” Sue admits, “I am not good at asking for donations; I just don’t enjoy it. But whether for the extravaganza raffle or for the early Light Up a Life celebrations, I would say it was for Hospice, and people would just open doors, hearts and wallets.” “I guess because so many people, including me, have had loved ones that have benefitted from their services, they are happy to help.” “The staff are so dedicated and loving—they give any and every way they can. One thing I really appreciated when I had family members receiving services was how they work with a person’s beliefs and wishes—they genuinely want to know your perspective and work hard to meet your needs within that. Matsumoto Paulette Timmer with daughter Jennifer Hansen at the 2015 Volunteer Appreciation Dinner Volunteer Profile Paulette Timmer Twenty years is a long run with one organization even in a paid position, but Paulette Timmer has given two decades’ of time to volunteering with NVHADS— a long-term commitment that has allowed her to observe and participate in the agency’s growth from a small hospice program to a multi-program continuum of care. She’s been through its four moves to consecutively larger spaces, and has worked with each of its three current programs. Upon retiring in 1995, Paulette was looking for volunteer work. She had always had an interest in healthcare, and her daughter, then a hospice social worker in another community, suggested she look into the Napa hospice program. She did, and has never looked back. “We only had three RNs and two social workers when I started here. I’ve provided respite and caregiving, done office work, helped with events and fundraising—whatever needed to be done.” Paulette’s favorite work involves interacting with patients. For many years, she worked with Hospice patients and their families, a resource and relief to those she helped. “When I would work with a patient,” Paulette says, “I wasn’t there to help a dying person—I was there to help a living person going through a difficult time. Thinking Beyond the Bottom Line How Changing Energy Sources Can Also Improve Patient Care NVHADS completed the conversion of its facility from traditional electric power to 100% solar-energy-generated power—a move with significant, long-term benefits in terms of energy conservation and financial savings. The system, mounted on the roof of the facility, converts sunlight directly into electricity and is expected to generate approximately 158,550 kilowatt hours of emissions-free electricity each year. system and provided supplemental financial support; and NVHADS Board Chair Steve Silva and Director of Development and Communications Joanne Sutro co-managed the project. “With this switch to solar, we are freeing up thousands of dollars—approximately $450,000 over the next 20 years—that can be invested in programs, client care, caregiver support and community services instead,” said NVHADS CEO Linda Gibson. The project was possible thanks to a collaborative effort: Sustainable Napa County and Napa County Energy Watch conducted a facility-wide energy audit in 2013; the Peter A. and Vernice H.
