53rd Annual Report 2012
THE FEDERATION OF OBSTETRIC AND GYNAECOLOGICAL SOCIETIES OF INDIA 53 Annual Report rd and Statement of Accounts Website - www.fogsi.org FROM THE PRESIDENT’S DESK Dear FOGSIan’s, I am fully aware of the responsibilities involved in leading this mammoth organization. At FOGSI, the task is made easy with the active involvement of the THOUGHT LEADERS in FOGSI who are constantly INNOVATING. To match this, we also have the energy and commitment of YOUTH LEADERS in FOGSI who are enthusiastically helping us in IMPLEMENTATION. Dr. Hema Divakar President I invite you all to be a part of Team FOGSI 2013, “INNOVATION TO IMPLEMENTATION.” Helping Mothers Survive (HMS) – This initiative was launched jointly by FOGSI in association with the John Hopkins Programme for International Education in Gynaecology and Obstetrics (JHPIEGO). Our aim is to prevent needless maternal deaths. To express our urgency, this mission is christened as a fast track initiative. We offer training on prevention, recognition and treatment of PPH and PE/E (Postpartum Haemorrhage and Pre Eclampsia and Eclampsia), which are the leading causes of maternal mortality. The FOGSI HMS CHAMPIONS train lower-level medical assistants and staff and educate them in maternal healthcare in an aim to shift the task of preventing maternal mortality to the frontline. An innovative birth simulator — the MamaNatalie —is being used to train individuals such as nurses, health officials and even ambulance drivers, on fulfilling the most basic of maternal healthcare needs, hence expanding the base of healthcare providers to women giving birth. There are currently only around 27,000 gynaecologists in maternal healthcare in India. This is completely insufficient to handle the sheer volume of births that take place every day in the country. India’s Maternal Mortality Rate (MMR) is currently one of the highest in the world, at a rate of 212 deaths per 100,000 live births. There is a great need for expanding the base of skilled professionals who can help in maternal healthcare. To date, HMS programmes have trained 100 master trainers (25 from each zone) They have conducted more than 100 HMS programmes across India from February to May 2013. About 1200 Frontline Health workers have been trained so far. We are embarking on the monitoring phase, to study the impact of this programme. We believe that our pride will be in the number of lives saved, rather than the number of participants trained in the HMS programme. Keep Educating Yourself (KEY) : The fields of medicine and science never stop moving forward — and neither should physicians. KEY initiative with the mantra, “Each one teaches one”, has witnessed several CME programmes pan-India this year and you will see it being mentioned as a part of committee reports by our leaders. Speciality courses in Gestational Diabetes Mellites – GDM through FOGSI-DIPSI and WDF, have been rolled out in 25 centers with 1000 OBGYNs enrolled. This course would help them implement well-known interventions and fulfill our dream of setting up Diabetes in Pregnancy (DIP) clinics. YUVA - Towards Tomorrow (YTT) -The exam oriented FORCE programmes through FOGSI prepare young doctors to be successful in their exams. The YTT series of programmes will enhance technical skill development and prepare them for future challenges beyond the exams – in the technology training in infertility, Ultrasonography and Endoscopy and various skill drills. VISION 2022 - build a healthy generation ….. FOGSI’s VISION is not only about building an organization – it is also about building a Healthy Nation. The ABCDE project launched at the Bengaluru Congress in June 2013 will continue to strive on targeting the issues facing adolescents such as Anemia, Contraception, HPV Vaccine, Diabetes and Emergency Obstetric care. This is FOGSI’s call to Action … We need every FOGSIan to help us in our endeavors. You are our strength and will be the catalyst for the change we want to see in women’s healthcare in India. With your involvement, I have no doubt that we will Innovate, Implement and Impact. The Annual Report is our most important communication to our membership. We are confident that on reading it, you will be proud of the important work done by your parent federation. Dr Hema Divakar President FOGSI FROM THE FOGSI OFFICE “The achievements of an organization are the results of the combined effort of each individual.” Vince Lombardi Dr. Nozer Sheriar Secretary General Dr. Hrishikesh D. Pai Deputy Secretary General Dr. Jaydeep Tank Treasurer Dr.Vidya Bhat Joint Secretary The nature of an organization is largely characterized by the work it puts forth. FOGSI, a professional body of obstetricians and gynecologists is committed to the goals of saving mothers’ lives and improving reproductive health care. But, the nature of an organization is also portrayed in how it goes about getting to the goal and how inclusive it is in terms of members and society at large. An efficiently run organization is vital to good executive function. The FOGSI office aims to be this highly efficient entity. The governance in administration of and organization of 221 member societies and over 27,000 members is an important and a constantly challenging task. The FOGSI Office is run by a joint effort the honorary work put in by the office bearers lead by the Secretary General and the full time office staff dedicated to administration function and support. The office staff works with Office Superintendent Ms. Jasmine Kunder heading the team. The staff is assigned specific duties related to day to day activities, accounts and banking, ICOG activities, Journal activities, work related to special schemes and FOGSI programs. This year the functioning of the FOGSI office has been revamped with Operation Clean Up a complete sorting and classification of years of records, Human Resource function with staff appraisals, Risk Management with an electricity and fire audit and fire safety measures and many other initiatives. This has helped in improving efficiency, quality of work output and ease of achieving results consistently. The role of good governance cannot be overemphasized. Though a number of measures are in place to ensure that the organization runs professionally, some areas need further thought and guidelines to improve. Towards this end FOGSI is today a proud organization with a clear Vision and Mission Statement, Administrative Policy Guidelines and an Organizational Code of Conduct. Besides this the Conference Organization is being streamlined by the AICOG Conduct Committee and the most important burning platform issues are been addressed by the PNDT Cell, the Anti Violence against Doctors Cell and the Anti Violence against Women Cell set up during the year. As the organization grows, there is a need to expand its role in education and training. To address this, it was decided at the Managing Committee in April 2013 that FOGSI should have its own larger premises measuring about 10000 square feet, in accordance with the MCI for establishing a College. A suitable plot has been identified and the process of assessment and negotiations is under way. We thank you for your belonging to our great federation and request you to be involved and participate in whatever way works best for you. We would like to place on record our thanks to our office staff, Dr. Parikshit Tank and Dr. Ameya Purandare for the assistance in compiling this Annual Report. ANNUAL REPORT 2012-2013 27 REPORT OF THE YEAR’S ACTIVITIES FROM THE IMMEDIATE PAST PRESIDENT, DR. P. K. SHAH The theme of 2012 was ‘Let the Life of Every Mother & Neonate Count’ The thought behind this theme was to infuse a sense of urgency and accountability towards the most important issue that we face as professionals and as a professional organization. Maternal mortality rate is a proxy indicator to reflect the functioning of the health system of the nation. It is also a comment on the professional and ethical standards of the profession. The Federation of Obstetrics and Gynecological Societies of India (FOGSI) is the flagship representative of womens’ health care providers in the country. Maternal mortality is a burning issue that we face and though we are making some progress, there is a long way to go if we want to meet the MDG goals by 2015. The key areas where we function are: 1. Training and spreading knowledge through scientific discourse, conferences, symposia, and CMEs. A number of workshops titled Maternal Mortality Reduction Workshops are being held to involve FOGSI members and train Government officials. 2. Capacity building through training of maternity health care providers. 3. Advocacy with the Government on laws and administrative practices 4. Working closely with the Government to complement and supplement services 5. Partnering with other NGOs aligned towards improving the state of reproductive health Excellent work is being done on this matter by the various special FOGSI projects, namely the EmOC FOGSI Certification course, FOGSI SIDA project, FOGSI – India FIGO Gates and MNH Initiative and the National Initiative for Maternal Health. FOGSI has a deep sense of social responsibility as is clearly set out in its Vision and Mission statements. Perhaps the most important social issue Topic Venue Date that we face, as women’s health care providers is sex selective feticide. Much has been said, written and read about sex determination and selective female feticide. FOGSI has been proactive in this regard. In 2012, a number of societies have formed DOSST (Doctors Opposing Sex Selective Termination) Cells. The massive Government machinery is at work at all levels and on various fronts to check these practices. FOGSI supports the sentiment of these machinations. There have been some serious difficulties that FOGSI members have faced during these operations. FOGSI has done its best to represent the difficulties and ease these burdens. There is some headway in terms of clarifying the issue that no additional qualifications are required for gynecologists to do ultrasound. Other important area where progress is being made is relooking at the Act as a whole and especially the issue of graded punishment. The highlights of the other activities for the year 2012 are grouped into 1. 2. 3. 4. 5. 6. Academic events and conferences Workshop series Public forums and awareness activity Publications – FOGSI Focus Good Clinical Practice Recommendations development Postgraduate training: Teaching resource development – Universal Undergraduate and Postgraduate Textbook of Obstetrics, Gynecology and Family Welfare A brief overview of these important activities. One of the biggest projects undertaken is the development of the UNIFORM UNDERGRADUATE AND POSTGRADUATE TEACHING MODULE ALL OVER INDIA. This is a comprehensive set of three textbooks – Textbook of Obstetrics, Textbook of Family Welfare and Textbook of Gynecology. These will cover over 200 topics as per the MCI syllabus and will be accompanied by multiple choice questions, short and long questions. Teachers will also benefit from the powerpoint presentations which will accompany each topic. This is going to be a major teaching resource in our specialty. A full set will be presented to every major teaching hospital of the country. The project is on track, but has seen some delays, mostly logistic, for which I apologize. EmOC FOGSI certification course report for April, 2012to March 2013 by Dr. Sadhana Desai. FOGSI – ICOG – GOI EmOC certification course was in 7th year of its implementation in 2012 -2013. Although, 5 years contract with GOI was over in March, 2011, additional 2 years of extension was requested by GOI to complete training of Government MBBS doctors working in FRUs and to do refresher course for some of EmOC certified Govt. MBBS doctors who were not posted in FRUs after completing their training and passing the examination. In this year, Government reviewed the EmOC programme by nominating an independent NGO “Deloitte” to decide if the programme should be renewed for further period of 4 to 5 years. In the final report submitted by “Deloitte” NGO on 31st March, 2013, there is a strong recommendation for continuation of FOGSI – ICOG – GOI EmOC programme. Government of India is therefore in process of finalizing the details of contract for continuation of programme. This year FOGSI has set up 8 new district hospital training centres has trained 15 master trainers in medical colleges to fill the vacancy of those who have retired and has trained 23 additional district hospitals practical trainers. Besides, 193 New Govt. MBBS doctors took training this year. Current Position – Creating the EmOC Human Resource Total Upto March 7, 2013 Nodal Centres 6 Tertiary Training Centres 34 District Hospital Training Centres 235 Master Trainers in Medical Colleges 212 District hospital Practical Trainers 333 MBBS doctors enrolled for long course 1280 MBBS doctors trained in short course 104 FOGSI – ICOG have also decided to start EmOC certification course for those DGO, DNB and MS doctors who are interested in taking the training. The details of the programme are being worked out. 2. Report of FOGSI-FIGO Bill and Melinda Gates Mnh Initiative by Dr. C.N.Purandare Maternal Death Review Maternal Death Review (MDR) is an important strategy to improve the quality of obstetric care and reduce maternal morbidity and mortality. MDR provides detailed information on various factors at community, facility, and district, regional and national levels that influence maternal health outcomes. A key challenge is to analyze paper based data related to MDR, collected through facility and community based tools. The current database of 10500 records of maternal deaths in India continues to expand on a daily basis, with data elements repeated across tools, making analysis of data difficult to manage and inform program and policy. It is critical to have a standardized solution across the country avoiding duplication of data through use of various software solutions with different reporting structures making it impossible to link and follow a cohort of pregnant women as intended by the Government led Mother and Child Tracking System. The National MDR software was designed and developed under the leadership of the Ministry of Health and Family Welfare (Maternal Health division) supported by The Federation of Obstetric and Gynaecological Societies of India, Systems and Program Management, National 38 Informatics Cell, Statistics Division, and Mother and Child Tracking system team through funding from the International Federation of Gynecology and Obstetrics – LOGIC. Website for MDR software is www. nrhm-mdr.nic.in Maternal Near Miss Maternal Health (MH) Division of Government of India (GoI), Ministry of Health and Family Welfare, in 2010, commenced the implementation of Maternal Death Review (MDR) in all the States in India. MDR is an important strategy to improve the quality of obstetric care and reduce maternal morbidity and mortality. To serve one of the purposes of strengthening MDR, development and implementation of a uniform National Near Miss policy framework and program is critical, as it will give additional value to complement information being received from the reviews of maternal deaths implementation. Review of “near-miss” events provides valuable insight into risk factors and possible means of prevention, particularly in facilities where deaths are uncommon and where events associated with deaths may not be typical. As the woman survives, near miss reviews may be seen as less threatening than death reviews for the teams who report them. THE FEDERATION OF OBSTETRIC AND GYNAECOLOGICAL SOCIETIES OF INDIA Under the guidance of Government of India, Ministry of Health and Family Welfare (Maternal Health division), Federation of Obstetric and Gynaecological Societies of India, Mahatma Gandhi Institute of Medical Sciences and Avni Health Foundation, Development partners, National associations of Pediatrics/Nursing/Midwifery/Neonatology/Anesthesia, National Institute of Health and Family Welfare, National Health systems resource center and Six Medical colleges firmed up the MNM policy framework, definitions, tools and criteria for India. The project was funded by FIGO-LOGIC. EmOC Curriculum Maternal Mortality Ratio (MMR) in India has shown an appreciable decline from 398/100,000 live births in the year 1997-98 to 254/100,000 live births (2004-06) and to 212/100,000 live births (2007-2009). While several factors and initiatives have contributed to this decline, Ministry of health and family welfare, Maternal health (MH) division’s policy decision to build capacities of MBBS doctors to provide high quality emergency obstetric care services and operationalize approximately 2000 first referral units across India contributed immensely to the prevention and reduction in maternal morbidity and mortality. Maternal health division of the Ministry of Health and Family Welfare in partnership with The Federation of Obstetrics and Gynecological Societies of India embarked upon a nationwide ambitious program of building capacities and task shifting in Emergency Obstetric Care (EmOC) in India. MBBS doctors were trained in acquiring skills in the management of emergency obstetric complications including C Section to overcome the shortage of skilled manpower so that, women in semiurban India were equally served. Other key partners included JHPIEGO, Macarthur Foundation, Averting maternal deaths and disabilities and Avni health foundation. MH Division of the Ministry of health and family welfare felt the need to update the EmOC curriculum in 2009, as the curriculum which was being implemented since 2005 was outdated given that there was new scientific evidence on the management of maternal emergencies, other maternal health programs were already using these evidences as part of their program implementation plans and training, the EmOC curriculum books were bulky and spread across seven books, and lastly on field health teams attending different training program were getting one message from EmOC training and another one from other trainings e.g. BeMOC (Basic Emergency Obstetric Care), SBA (Skilled Birth Attendant), New Born Care etc. An urgent need was felt to not only update the EmOC curriculum but also synchronize it with other program training modules. As on date the final EmOC curriculum has been released for pilot implementation across all the 35 EmOC training center. The EmOC curriculum was updated under the guidance of the Ministry of Health and Family Welfare (Maternal Health division) supported by The Federation of Obstetric and Gynaecological Societies of India, Systems and Program Management and CSSMU & Queen Mary Hospital – Lucknow, through funding from the International Federation of Gynecology and Obstetrics – LOGIC. 3.
