Green Valley Hospice is recognized as a program participant by National Hospice and Palliative Care Organization: Green Valley Hospice is a Kaiser, Medicare and Medi-Cal contracted agency. Green Valley Hospice Physician referral is necessary to provide hospice services. Our office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday. Telephone referral service and access to a nurse is provided 24 hours a day. We are honored to have the opportunity to serve you, members of your family, a loved one or your patients. About Our Agency A brief description of what we do: Green Valley Hospice is an agency which provides a valuable service to residents of the greater Sacramento area. At Green Valley Hospice, we provide quality care and comfort in the last phase of one’s life. Our team is far more than just employees, we are kind and caring people who understand and appreciate the significance and honor in supporting both the family and the patient during their final journey through life. How do we serve the Hospice community? Green Valley Hospice is a community based agency. Our goal is to be available to the community as a valued resource and to be as supportive both to the patients and their families in their time of greatest need. Mission Statement Green Valley Hospice is committed to providing compassionate, personalized and competent care for individuals as they approach the end stages of life. We look forward to working closely with patients and their families, to structure a plan of care which takes into consideration the unique needs of the patient, respecting the unique differences in lifestyles, values and wishes of each family member. Our goal is to emphasize the comfort and dignity of the patient, making every effort to being mindful of their spiritual and social needs. We wish our patients to live out their remaining life to the fullest, on their own terms. We Are Your Support Team A large majority of people associate hospice care with hopelessness doom and gloom. They are afraid of death and the process of letting go. This is completely understandable. We can say from experience that it is an honor and a privilege to be part of someone’s final weeks, days or hours. You have the opportunity to be part of your loved one’s final journey. The Hospice Team understands and supports your desire to care for your loved one in the living environment of their own home. We realize that this period of time is a very difficult one and sometimes, because “fear of the unknown” is greater than fear of the known, our desire is to be as honest and straightforward as possible in helping you through this period. The interdisciplinary team, led by the hospice medical director and your attending physician, will develop a plan of care focused on your individual needs. The Medical Director directs the hospice team and is a physician licensed in the state of California who is responsible for the overall medical direction of the hospice program. The Nurse is the case manager and coordinates the hospice plan of care. The Plan of Care is a written medical plan developed by the attending physician, the medical director, and the hospice team, addressing the needs of the patient and family admitted to the hospice program. The nurse communicates frequently to keep your physician aware of your condition and needs. The nurse supervises the home health aide services. A skilled nurse is on call 24 hours. The Social Worker role begins during the first week of admission. Social workers can talk to you about financial, physical, and emotional concerns you may have A social worker is an experienced counselor who assists patients and families during this time. Spiritual Counselor: Although Green Valley Hospice does not promote or advocate any particular religion, the hospice chaplain is available to help meet you and your family’s spiritual and bereavement needs. The chaplain may also help assist families in planning the funeral or memorial services. The spiritual counselor role extends for the 13 months of the bereavement process. Home Health Aide: Under the direction of the registered nurse, the aide will provide personal care such as bathing, change of linen, assisting with personal hygiene, and assisting with light housekeeping. Volunteers are very special individuals who give freely of their time to support hospice, the patients and their families. All volunteers participate in a volunteer training program and work under the direction of the Volunteer Coordinator. If you have experienced a loss of a loved one, we recommend waiting 12 months before joining our Volunteer Program. Bereavement services are for the surviving family members and friends for a period up to 13 months after the death of the patient. Services include an assessment of the needs of the bereaved family and the development of a care plan that meets these needs, both prior to and following the death of the patient. The Chaplain is the Bereavement Coordinator at the Green Valley Hospice. “You matter because you are you.
