Comparative Listing Of Medical Billing Software Packages
Introduction
Healthcare industry in the United States is undergoing rapid changes with a
continued emphasis on privacy regulations like HIPAA (Healthcare Insurance
Portability and Accountability Act). This coupled with the ever-increasing stress on
improving the efficiency of healthcare operations to minimize the healthcare costs is
forcing the providers, insurance companies, clearinghouses, and billing companies
equally to evaluate alternative processes and systems.
Medical billing is a very important portion of the whole equation affecting the
healthcare industry. Medical billing firms ensure that the right payment is routed
properly from the right patient’s insurance company/companies to the
practitioners/hospitals in a timely manner. Medical billing has become a complex
endeavor due to the sheer number of diagnostic codes, procedure codes, and other
requirements that change year after year increasing the chances of billing error.
There are several hundred medical billing software packages, which try to make the
billing process more efficient and generate electronic claims. Medical billing software
also helps providers manage financial information and reduce errors on claims
submitted to over 3000 insurance companies and the Medicare. Medical billing
software can be a standalone function or integrated with other aspects of medical
practice such as patient medical records, appointment scheduling system, etc.
Several practice management packages are increasingly eyeing for a vertical
integration in terms of offering billing, patient scheduling and medical records
including the patient history etc.
Since billing firms are mostly independent entities handling the claims processing for
the practitioners, it is becoming more and more necessary to have a seamless flow of
data from the software package used by the practitioners to the billing package used
by the billing firms. Having several hundred software vendors in the industry does
not help to this problem and indicates a definite need for further consolidation in the
industry and a spread of interoperability standards among existing packages. This
will reduce the switching cost when the practitioners want to move to a different
medical billing vendor and will increase the overall efficiency of billing operations.
In this paper we have attempted to compare several popular billing packages in
terms of their features, technical capabilities, cost and interoperability compatibility.
This paper is an attempt to provide useful information for small and medium medical
practices, which would allow them to make suitable selection of software depending
on the fluid needs of their client base.
The following packages were included in the evaluation:
1. AdvancedMD
2. Altapoint
3. Centricity
4. ClaimGear
5. elligence
6. Healthpac
7. MedicsElite
8. NDCLytec
9. NDCMedisoft

Comparative Listing Of Medical Billing Software Packages
The paper describes each of the above software packages in detail and a combined
comparison chart is included at the end of the paper. Sample screen shots are
included to get a real feel of the software user interface and capabilities. Important
as well as unique features of all the packages are noted. The information presented
in the paper is collected from varied sources including but not limited to company
website, brochures and other marketing material, direct interaction with company
executives, support professional etc.
Billing software can be a stand-alone solution or a part of the larger practice
management package. In either case, the industry trend is toward more powerful
interactive financial tools that fully integrate with practices' administrative and
clinical software and databases.
However we strongly recommend that prior to implementing any new technology,
practices should evaluate their technology and processes to determine how they are
related before making any changes to either. Technology is a great means to manage
operations, but only if the operations support the technology.

Centricity – by GE Healthcare Systems
Overview
Since its inception in 1993, Centricity Physician Office Practice Management
was built from the ground up as a 100% pure Microsoft-based solution.
The Centricity system, now installed in more than 3,000 office-based
physician practices, automates administrative functions, such as scheduling, patient
information, billing, insurance claim processing, accounts receivable and collections,
managed care, and inventory control. Each month, more than 3.6 million
transactions are processed for more than 12,000 physicians with the use of GE’s
Centricity™ Physician Office Practice Management system (formerly called Millbrook
Practice Manager).
Centricity Physician Office - PM is built to interface with thousands of other
Windows applications, which allows easy integration of custom features. Centricity
works with practice sizes of 1-150 physicians and is capable of handling over 70
specialties. Currently it is being used in 50 states all over US.
Features
Centricity Physician Office - PM offers tools to manage everything from scheduling to
collections to financial analysis. These are some of the capabilities:
•
Resource-based scheduling that accommodates multiple providers in multiple
locations
•
Patient information management accessible from any screen
•
Fast, accurate billing for any payer, including any kind of managed care
contract
•
Built-in electronic claims, remittance, statements, and eligibility
•
Accounts receivable and collections tools for better cash flow
•
Flexible reporting, including both built-in reports and compatibility with
Microsoft Excel, Microsoft Access, and Seagate Crystal Reports
•
Inventory control
•
Scanning for insurance cards, lab reports, and more
•
Compatibility with dozens of EMRs, appointment reminder systems, and other
specialty software
Because Centricity was built on the Microsoft Windows platform and the HL7
standard, it is compatible with any other Windows-based software. Centricity runs on
Microsoft/Intel-based computer hardware. It takes full advantage of industry
standard protocols to move data between disparate systems like outside labs and
internal equipment. It uses HL7, for example, the standard for communicating health
data among disparate systems. This lessens the need for expensive, proprietary
interfaces. It is accessible to Microsoft Word, Excel, and Access, and to Crystal
Reports, and further to other reporting tools using ODBC.

Ledger, so each credit can be processed.
Document Designer
• Customize and design your own insurance forms, statements, superbills, recall
notices, marketing mailings, surveys and other documents and
correspondence.
• Work with a number of pre-defined documents, including the CMS 1500

EDI Overview
• Manage the flow of electronic healthcare transactions to and from your
practice, through a unified, centralized system.
• Verify the validity of all outgoing and incoming EDI transaction files.
• Transmit and process files, just by pressing EDI Manager's "Send/Receive"
button.
• Maintain all EDI transactions specifications and requirements, to ensure strict
ongoing compliance with HIPAA.
Electronic Claims Submission
• Build an electronic claims file, based on user-defined selections, of charges
that have been entered into the system.
• Based on a library of rules that is pre-defined in elligence, an Exceptions
Report is displayed that flags claims from which information is missing or
inaccurate.
• Hyperlinks in the Exceptions Report take the user to the exact place in
elligence where the data must be corrected.
• Pre-Processing Report displays all charges that will be billed electronically.
• Claims are processed, then placed in the EDI Manager's Outbox for
transmission.
Electronic Remittance Advice (ERA)
• Replaces the time-consuming process of manually posting multi-page EOBs
with an automated, streamlined method - more effective personnel utilization.
• Enables significantly faster updating of accounts receivable.
• Automatically posts payments, maintains a history of allowed amounts and
adjusts write-off amounts.
• Highlight any patients and/or charges that have any claim or charge
adjustment codes, as reported by the insurer.
• Claims that do not meet expected payment amounts, or have other errors
that cannot be corrected immediately from the Exceptions Report, can be
posted manually later.
Electronic Statements
• Generates a batch of statements, and then transmit it to the statements
processing service bureau.
• Create a back-up copy of the statement batch in HTML or PDF format, so you
can retrieve the statements at a later time and address inquiries by the
patient.
• The service bureau will send an acknowledgement report, indicating they have
received your batch, the number of statements included, etc.
• The file is processed, data is double-checked, and addresses are bar-coded.
• Your statements are printed.
• Undeliverable addresses are separated & corrected.
Technology
elligence is designed specifically for Microsoft Windows, and other Microsoft
technology (such as SQL Server 2000). Data for all programs is stored in standard
Microsoft SQL Server 2000 for easy transfer of information. The application is
deployable on a Local Area Network (LAN), Wide Area Network (WAN), Virtual Private
Network (VPN). Also, it may be deployed on a peer-to-peer network for fewer than
five concurrent users.

Healthpac –
by Healthpac Computer Systems, Inc.
Overview
Healthpac Computer Systems, Inc, with corporate headquarters in Savannah,
Georgia, has been in the market for 23 years with clients in over 30 states across the
U.S.
Healthpac Computer Systems, Inc. offers their software package H2000 that
is HIPAA ready and has many powerful features such as: full patient billing, electronic
claims filing, electronic remittance posting, and extensive insurance claims
"scrubbers".
Features
H2000 is a quick, easy medical billing software system offering fast tracking of
demographic background, and insurance information. Healthpac can be used for a
large multi-user system, or a single-station full management system, or simply to
process and file claims electronically. Through the use of user definable "rules
tables," H2000 can be customized to the requirements of individual practice.
Like all Patient Information and Insurance Claims Processing systems on the
market, H2000 is also designed to do the basic functions of:
•
•
•
•
•
•
Reducing the average age of receivables
Improving cash flow
Automating insurance claims, rejections, re-filing, and follow up procedures
Providing a better in-house collection system
Identifying bad debt situations before they happen
Capturing and controlling all information relative to the patient and all
services rendered
Healthpac offers a package that can operate on Client/Server as well as ASP
software architecture. An Application Service Provider (ASP) version of Healthpac
H2000 is available for a low monthly cost per workstation. Under this plan, all a client
needs is a PC connected to the internet. Healthpac maintained programs and client
data are available 24 x 7 via a secure internet connection. This feature is a good
option for new startup Billing Companies, doctors’ offices and small DMEs with 1 or 2
workstation requirements.
Healthpac H2000 comes with the following features Patient Information:
• User Defined “Claim/Data Entry Edits” by Doctor, Insurance Company and
Procedure Code
• Ticklers Available Each Time You Make a Note on An Account For Follow Up
• Scanning Linked By Account/By Line Item
• Can Automatically Load Fee Schedules That Are In ASCII/Excel Format
• The system can scan insurance cards, driver’s license, PHI disclosure release
and other documents and fed directly into the system for future reviews.

The pre-programmed "checks and balances" system forces the crossreferencing of data thereby verifying the accuracy, before filing the claim.
Security:
• The system is set-up for user specific log in and password-protected entry.
Every movement is stamped with the user, date and time information. Every
transaction can be tracked and reported.
• The system can be set to auto-log off users after a determined period of time.
This is called a "sleep mode" and the user must log back into the program
with their password upon return. This is another step in HIPAA compliance.
• User access can be defined locally by management/administrator. With a
simple “yes” or “no”, management can allow a user to have access to
demographics but not all of them the ability to view financial/account
information and more.
Practice Management:
• The system has the ability to "scrub" the claims before they are submitted,
verifying that the required information is on the claim necessary for
processing and payment.
• The system automatically prints an (computer generated) EOB from the
primary payment to send with the claim to the secondary. As the HCFA forms
print, any secondary claims will have an accompanying EOB - sorted.
• One can scan the posted EOB for future retrieval and review. The system is
designed for the user to stay at their station, recalling or printing whatever
they need to be as efficient as possible. It is also possible to auto-post an
electronic EOB.
• System handles multi-tiered pricing for each CPT Code to permit automatic
posting.
• Ability to suppress claims based on CPT Codes for procedures not covered by
insurance.
• Group CPT Codes for reporting and analysis
Reporting:
• Use Ad Hoc Report Generator to export data into Excel or Other Programs
• Compare Current Year Month End to Prior Year Month End totals using
Healthpac’s Practice Management Reporting, which displays on screen, your
requested information with additional pie or bar chart that can be used to
create impressive presentations.
• Practice Management Reporting allows managers to analyze production
information by “drilling down” to supporting data by clicking on the figures
being analyzed.
• The Employee Production Report provides for the analysis of detailed
Employee Information supporting educated personnel related decisions
Technology
Operating Environment – H2000 runs on Windows, NT, and UNIX systems. Clients
requiring LINUX should call Healthpac to discuss support options.

Comparative Listing Of Medical Billing Software Packages
Conclusion
The healthcare industry has evolved greatly with the advent of medical billing
software as now more and more claims are being submitted electronically. The
software helps providers manage financial information and reduce errors on
submitted claims. It facilitates better accounts receivable management, improved
business analysis and efficient medical practice. It is easy to link patients’ insurance
data, x-ray reports, laboratory test reports and other information to their medical
records with such software.
All the applications have the standard features that are essential in medical billing
software, with some additional features that differentiate each from one another.
Some software packages are based on Client/Server architecture, some Application
Service Provider (ASP) based, while some are available in more than one models. If
one package has extensive scheduling facility, then the other has better reporting
capabilities. Nevertheless, all these applications fit into the bracket of solutions for
small and medium business practices with low to moderate price range.
Overall medical billing software industry is currently in a highly segregated state with
over 500 players offering competing packages. Most of the medium to large players
also offer complimenting packages to serve medical practices’ ancillary needs. In the
near-term future we foresee a trend of greater consolidation in the industry with a
couple of major players offering comprehensive software packages to meet the needs
of a medical practice. Already many of the compared packages above offer modules
for appointment scheduling, billing, invoicing, patient data management which are
seamlessly integrated with each other. As the industry evolves to a higher maturity
level, there will be a higher vertical integration in terms of some of these packages
acting as MERP (Medical Enterprise Resource Planning) packages and will offer the
clearinghouse services as well.
One of the other forces pushing the healthcare software community towards
consolidation would be the rapid globalization. There will be an increased need for an
intra-package seamless sharing of patient data to meet the needs of an evershrinking world as the patients move from state-to-state and eventually from
country-to-country and medical practices at different geographical locations be able
to share these critical pieces of life-saving information. It will be upto the industry
regulatory bodies to enforce the standards that would allow an error-free data
transfer from one package to the other. Some of the technologies like eXtented
Markup Language (XML) already exists which makes it feasible to transfer the data
from one-package to the other; Its only time which will make it practical to use such
and other emerging technological standards to make the industry more efficient and
the world more healthy.
This paper aims to provide a reference for choosing an appropriate billing software
for a medical billing enterprise which is under a constant pressure to serve several
medical providers who in-turn use different practice management packages.
