Military Service and Pension Records CLASS HANDOUTS Awards and Decorations The issuance or replacement of medals, decorations, and awards should be directed to the specific branch of the military in which the veteran served. It is recommended that Standard Form (SF) 180 be used for such requests. You may obtain this form from most local VA offices; or by calling Fax-on-Demand (301) 837-0990 from a fax machine, using the handset. Follow the voice instructions, and request document number 2255. There is no charge except for any long distance charges you may incur. You may request SF-180 in writing from The National Personnel Records Center, 9700 Page Avenue, St. Louis, Missouri 63132. With computer access, you may download it from the internet: http://www.archives.gov/research_room/obtain_copies/standard_form_180.pdf Do not use the addresses on page 2 of SF-180 for awards, decorations and medal requests.
