N Nuurrssiinngg EEdduuccaattiioonn IInniittiiaattiivvee A Apppplliiccaattiioonn CChheecckklliisstt The Nursing Education Initiative is a tuition reimbursement program funded by the Ontario Ministry of Health and Long‐Term Care. Grants are available to support nurses in continuing education to improve their nursing knowledge and professional skills. Important Application Information The following MUST be included in your completed application or it will not be processed: 1. Completed and signed Application Form 2. Proof of payment of education to be reimbursed (base tuition fees / registration fees only) 3. Proof of successful completion of education to be reimbursed (i.e. transcript, grade report, certificate, etc.) 4. Void cheque or direct deposit information (i.e. financial institution name and number code on bottom of cheque) Note: It is the applicant’s responsibility to ensure that the information contained in the application is true and complete. The applicant is to understand that any false or incomplete information submitted in support of an application may invalidate that application. Nursing Education Initiative PLEASE BE ADVISED: Funding is not guaranteed. Only completed applications will be accepted. The Application Form must be signed. Incomplete applications will not be processed. Completed applications must be submitted within 90 days (3 months) of the education finish date. The applicant will not be notified to confirm receipt of application. The applicant will be notified of the application’s status within a minimum of 4‐6 weeks after submission.
