Because we want your stay with us to be a very positive experience, we’re providing you with information to help your admission and treatment program go as smoothly as possible. Please let us know if we can help you in any other way by contacting your social worker. Our Mission Guided by God’s purpose and a commitment to exemplary care, we empower each mind, body and spirit to reach its fullest potential. Before Admission Tours Tours of the facility are provided Monday through Friday. The best times to tour are from 9:30 a.m.11:00 a.m. and 1:00 p.m.-3:00 p.m., as they allow you to see patients participating in therapy. So that we can have tour guides available, please call (214) 820-9330 to schedule the day and time that would be most convenient for you. If special arrangements need to be made for evening or weekend tours, please let us know. What to Bring With You Clothing We want you to be as active as possible during your stay, so we recommend that you wear nonbinding clothes during daytime hours. Shorts, sweat pants, and t-shirts with tennis shoes are especially comfortable. Dresses are not appropriate for therapy activities. We recommend that you bring at least six changes of clothing and an adequate supply of socks and undergarments. Pajamas, gowns, bathrobe and slippers are suggested for the evening, and swim suits are needed for aquatic activities. Please use an indelible ink pen to mark all clothing with your complete name for easy identification. Personal Items We recommend that you bring the following: • Brush & comb • Toothbrush & toothpaste • Shampoo • Deodorant • Shaving supplies (electric razor if you are on blood thinners) • Non-skid shoes (e.g. tennis shoes) • Eye glasses/contacts Additional items to include may be: • Hair dryer • Hair curlers • Radio/tape player (All electrical appliances must be checked by our engineering department before use and should be labeled with your name.) We ask that you leave all non-essential items at home. Room Assignment Your room will be assigned according to your medical need and bed availability at the time of admission. Most patients will be admitted to a semi-private room. Please feel free to bring personal items to decorate your room, including plants, flowers, etc. There is a bulletin board in each room for you to display cards, posters, and photographs. Family and Guest Housing For out-of-town family members, we have limited access to a variety of nearby, lower-cost housing facilities. These include hotels and motels that can be reserved for nightly, weekly or monthly stays. Please call 214-820-9330 for a list or ask your social worker. About Our Hospital Parking Parking lot #28 adjacent to the hospital on Gaston Avenue is designated for the family members of inpatients and for outpatients and participants in our community exercise and aquatic classes. The charge for this lot is $2.00. So that we can have adequate parking space for these persons, please ask friends and other visitors to park in lot #27 or #9, on the corner of Gaston and Hall. Parking for these lots is anywhere from $2.00 to $4.00 depending on the length of time in the lot. Parking is not allowed in the front and side driveways that are designated for patient transportation drop-off and pick-up. Please ask your family members and visitors to honor this request. Hospital Hours and Access The lobby entrance on Gaston Avenue and the Floyd Street entrance, near the Aquatics Center will be open from 6:00 a.m. to 6:00 p.m. The west entrance will be open from 6:00 a.m. to 9:00 p.m. Persons can exit all doors at any hour, but will not be able to re-enter the hospital after locking until 6:00 a.m. Telephone Calls Local telephone calls may be made from patient rooms by dialing nine (9), then the number desired. Metro numbers beginning with 817 and long distance calls may be made by dialing nine, zero, the area code and the number desired. These calls must be charged to a calling or credit card. Family members and friends may call directly to the room at any time. However, we recommend that calls not be placed to the room after 9:00 p.m. as a courtesy to patients who may have already retired for the evening. Smoking Smoking is not permitted in the hospital by employees, patients, family members or visitors. Baylor facilities and all outdoor areas of the Baylor campus grounds (including parking areas and sidewalks) are designated smokefree. Smoking is not permitted anywhere on our campus. This policy is for the protection of all who come to Baylor for their healthcare, to visit or to work. Laundry As a convenience to our patients and their family members, washers, and dryers are located in the basement. There is no charge to use the equipment, however, washing detergents are not provided. Patients who are not able to do laundry and who have no family members available can make special arrangements through the nursing department. Pets There is an Animal Assisted Therapy Program that is part of our inpatient treatment program. These animals have been through extensive obedience and temperament testing before being allowed into the hospital. We regret that personal pets are not allowed inside the hospital. If you would like to visit with your pet, please do so outside the facility. Media Coverage of Patients There are occasions when television, newspaper, magazine, and radio stations want to interview and/ or photograph patients. We are happy to accommodate media requests. We ask that family members do not schedule interviews directly with the media, but contact our media relations representative at 214-820-1817 for coordination of these requests. This will allow us to schedule the interview at a time that does not interfere with the patient’s therapy or other treatment services. Additionally, the Baylor media representative will be present during the interview to assist the news reporter and camera crew and help ensure a positive experience for the patient and family. Please refrain from taking personal photography or video during the therapy sessions. Before taking any photographs, please contact the marketing department at 214-820-9330 for approval. Dining Options The Truett Cafeteria is located on the Baylor University Medical Center Campus on the basement level of Truett Hospital. Family members and visitors can take the rear elevators to the basement level and follow the signs in the tunnel that lead to the cafeteria and bake shop. Daily food selections are provided by calling 2-MENU, inside the hospital or 214-820-MENU, outside the hospital. Hours of operation are 6:30 a.m. to 7:30 p.m. seven days a week. The BIR Bistro, located in the lobby, offers Starbucks coffee and “grab and go” snacks from Monday through Friday and is open 7:00 a.m. to 7:00 p.m. The Atrium Market is located in the lobby of Roberts Hospital on the Baylor University Medical Center Campus. It provides “grab and go” food and is open from 10:30 am to 1:30 am daily. Choices include a single serving size or family size meal that is hot and ready to eat or pre-packaged for heating later. Nourishment Centers are located on all patient floors. They are stocked with coffee and tea for family members and visitors. Snacks are available for patients on request from the nursing staff. Perishable food must be labeled and can be stored in the refrigerator on the floor. Food is thrown out each day at midnight. Foods that do not need refrigeration may be kept in patient rooms in a non-breakable container with an airtight lid. Soft drink machines are also located in the third and fourth floor patient dining rooms and the second floor occupational therapy gym. Chaplain and Worship Service Our Chaplain is available to meet with patients and family members to provide support and spiritual counseling. The Chaplain can be reached during the day at 214-820-9550. Chaplain services are provided on a 24-hour basis and can be arranged through the nursing staff. A worship service is held each Sunday beginning at 11:10 am in the Chapel located on the first floor of the hospital off of the left side of the lobby entrance from Gaston Avenue. The Chapel also provides devotional material and prayer requests. Dial-A-Prayer is available to patients and family members by calling 214-820-2333. What to Expect on Your First Day Admission After arriving at Baylor Institute for Rehabilitation, you and your family will be greeted in your room by the Patient Admissions Coordinator. The Coordinator will provide bedside admission and orient you and your family members to your room and the hospital. After this process is complete, your nurse will visit with you and do a medical assessment. On the day of admission, we ask that patients be accompanied by a family member. This is especially important if the patient is not able to complete the admission forms, verify information and sign the “Consent to Treat” form. If no family member is available at the time of admission, a family member must complete the admission documents within 24 hours. This can be done by checking in with the Admissions Desk on the first floor of the hospital, between 8:00 a.m. and 5:00 p.m., or with the Nursing Supervisor, after 5:00 p.m. weekdays and on weekends. We also ask that patients bring their insurance card, prescription drug card, Medicare card, driver’s license and social security card at the time of admission. Although financial information has been verified prior to admission, we will use this information to arrange for insurance assignment to the hospital, permitting direct billing to your insurance company. Treatment Program A rehabilitation hospital is very different from an acute care hospital. The types of activities that you will be participating in and the assistance that you receive will be different. We have developed this portion of the brochure to help explain these differences and why rehabilitation services are necessary to help your continued progress. What is the purpose of a rehabilitation hospital and why have you been referred to one? Following your injury, illness or surgery, you may have experienced loss of strength, endurance and/or the ability to perform activities in the way you once did. Your deficits may be in the areas of speech, thinking/ cognitive abilities, and movement/mobility. You have been referred to our facility because it offers a treatment program to help you regain independence and functioning. This is the goal of all our patients. Your Treatment Team Our interdisciplinary clinical team, led by physiatrists and including licensed physical therapists, licensed occupational therapists, speech-language pathologists, certified recreation therapists, neuropsychologists, certified rehabilitation registered nurses, social workers and other specialized clinicians, work with each patient to design a custom-tailored program of rehabilitation care. Physiatrist This is a physician who specializes in physical medicine and rehabilitation. He/she will be your attending physician, meaning the physician who is primarily responsible for your care. The physiatrist may also call in consulting physicians to address specific medical problems that may arise. Clinical Nurse Coordinator The clinical nurse coordinator works with all the people involved in planning and implementing your treatment program. The coordinator will help with issues related to financial resources and is the primary contact and source of information about your progress in the treatment program. Social Worker The social worker problem solves issues related to financial resources and insurance benefits as it relates to discharge options and services. She provides you and your family with discharge information and makes referrals for continued services for you upon discharge from the hospital. Rehabilitation Nurse The responsibilities and involvement of the rehabilitation nurses vary, depending on the level of care required by each patient. The nurse will monitor your physical condition, nutrition, bowel and bladder function, response to medications and vital signs, similar to the nurse’s role in an acute care hospital. However, nurses in our rehabilitation hospital will also help ensure that therapy recommendations are carried out and provide training to you and your family members regarding how to manage medical issues after discharge. Patient Care Technician (PCT) Like the nurse, the patient care technician provides varying degrees of support based on your level of independence. The PCT’s primary role is to assist you with self-care activities in your hospital room. As your progress in the treatment program and become capable of performing these activities more independently, the PCT provides less hands-on care. Occupational Therapist (OT) The OT works with you in the areas of self-care. This includes dressing, grooming, bathing, eating, and advanced self-care activities such as home management, cooking, cleaning, money management, and the use of upper extremities (arms). The OT may also work on assessing and treating vision deficits and prescribing assistive technology. Physical Therapist (PT) The PT works with you in the use of your lower extremities (legs), in the areas of mobility, including bed, or walking, with or without the use of assistive equipment, and/or using a wheelchair. The physical therapist will also make recommendations on your equipment needs in preparation for discharge. Rehabilitation Technician The rehabilitation technician is specially trained and carries out therapeutic activities prescribed by your license therapist to address your rehabilitation goals.. The rehabilitation technician is specially trained and supervised by a licensed therapist to supervise activities that address your physical rehabilitation goals. Speech-Language Pathologist The speech-language pathologist will help you improve communication skills, as well as cognition, in the areas of listening, speaking, reading and writing. He/she may also work on assessing and treating swallowing disorders. Neuropsychologist Neuropsychology provides assessment and treatment in cognition, which includes thinking, new learning, memory, emotional well-being (coping skills, mood), and behavior. Services are provided in individual, family and group therapy settings. Adjustment counseling and sexuality, including relationships, self-esteem, body image and fertility are also addressed. Therapeutic Recreation Specialist (TR) The TR works with you on the physical, cognitive, social and emotional issues which may interfere with leisure and community activities. The TR may adapt leisure activities so you can continue to participate in pastimes you enjoy. The TR can also help increase your awareness of leisure and community resources, which may include taking you on a community outing prior to discharge. Respiratory Therapist A respiratory therapist provides treatment and education to patients who are on a ventilator, have a tracheotomy or have any pulmonary dysfunction. Your Daily Therapy Schedule Patients will receive approximately three hours of therapy a day Monday thru Friday. Within a day or two of admission, you will begin a routine of therapy activities based on your individual needs. You will receive a printed therapy schedule daily to help you know what times you need to be prepared to participate in therapy. While most of the time your schedule will remain the same, sometimes it is necessary for changes to be made to it. This will occur if you progress into new treatment activities, diagnostic tests or medical procedures are scheduled, or one of your treatment team members is absent. A printed copy will be delivered to your room the evening before by nursing staff. If schedule changes occur during the day, a new schedule will be printed and put in place of the previous schedule. A copy of the therapy schedule is also available at the nurse’s station on each floor. Therapies The therapy gym and therapy offices are located on the second floor. Aquatic therapy is provided in the Aquatics Center on the first floor. Therapy is provided on both a one-on-one basis and in a group format. Hospital transporters are provided to take you to each therapy session. At the end of your therapies, you will be asked to wait near the second floor elevator for the transporter to take you back to your room where they can assist you back to bed as needed. As you progress, you will be asked to become more independent in your mobility. For example, you may be asked to propel your own wheelchair instead of having the transporter push it for you. Family members, who have been trained in transfers and who are able to help you get in and out of bed, may also transport you to and from therapies. Hospital Passes During your rehabilitation program the treatment team may recommend that you go out of the hospital on a pass. In addition to providing a break from being in the hospital, the pass will help you prepare for going back into the community and the activities that are important to you. Initially, you may be scheduled for an outing with the therapeutic recreation specialist. This will allow you to be in the community with the support of the Baylor Rehab staff. You may also be provided with a pass to leave the hospital with family members. Before this type of pass is granted, your family members must complete training with the PT on how to safely assist you and manage any equipment while out in the community. One training is completed, your physician will issue the order for a community pass. Patient Care Conferences Throughout your stay, patient care conferences will be held. During this conference your treatment team will meet to discuss your treatment plan, therapy goals, progress and anticipated discharge date. Your and your family members will have the opportunity to attend some of these conferences and will be notified of the day and time the conference will be held. Since space and time are limited, we ask that you be on time, suggest that you prepare your questions in advance, and that you limit the number of family members to two. This will help us accommodate all patients and families scheduled during the allotted time period. Family Observation and Participation It is our philosophy that support and involvement from family and friends enhance the rehabilitation process. Therefore, your family members and friends are invited to observe and participate in daytime therapies, evening activities and training sessions with therapists. Visitors are also welcome in your hospital room until 9:00 p.m. Observation Area, Room 307 For the welfare of patients, we limit the number of visitors in room 307 to two per patient, and children under age 12 are allowed only with special permission. Visitors are not allowed to enter room 307 when the door is closed for a procedure. Additionally, guests are not allowed to stay overnight. Overnight Guests In our private rooms, one adult over the age of 16 may stay overnight. We are not able to accommodate overnight adult guests in semi-private rooms except under the following circumstances: • The patient is critically ill • The patient is extremely agitated • The patient is under age 16 These exceptions are also contingent upon space requirements to perform patient care activities and roommate consideration. Overnight stay in a semi-private room must have staff approval and the roommate must be in agreement. Discharge Planning A discharge date is set for each patient at the first patient care conference and reassessed through the hospital stay. Your social worker will inform you and your family members well in advance of the date so that adequate preparation can be made. Your treatment team will make recommendations regarding plans for continued therapy or other care options following discharge. Recommendations will also be made for any home modifications, equipment, community resources and other services. Additionally, you may be scheduled for a follow-up visit with your physician in the Outpatient Services Clinic one month after discharge. At this time, your medical, physical and functional status will be evaluated and recommendations will be made for modifications in equipment, medication and/or home program. Discharge Day Your discharge plan, including the day and time, will be clearly communicated to you. We ask that you make advance arrangements with your family or caregiver to leave your room at the time specified by your treatment team so that we can make it ready for the admission of another patient. If you have any items in the Business Office vault, please make arrangements in advance to retrieve them.
